  We are seeking a compassionate, reliable, and experienced Direct Care Worker to provide high-quality, person-centered care to clients in their homes. This role involves assisting clients with Activities of Daily Living (ADLs), meal preparation, medication reminders, and safe transfers using a Hoyer lift. Click this link or paste in your browser to complete the application: Key Responsibilities: Assist clients with Activities of Daily Living (ADLs) including bathing, grooming, toileting, dressing, and mobility. Prepare and serve meals according to clients dietary needs and preferences. Encourage and remind clients to take prescribed medications as scheduled (Note: No medication administration). Use a Hoyer lift safely and correctly to transfer clients as needed. Perform light housekeeping duties to maintain a clean and safe environment. Monitor and document clients condition, behavior, and physical/mental status. Communicate effectively with family members, care managers, and other care team members. Follow the clients personalized care plan and agency policies at all times. Maintain client dignity, privacy, and respect at all times. Qualifications: Minimum 1 year of experience in a similar home care or direct care role. Proficient in using a Hoyer lift and trained in safe client transfers. Knowledge of proper body mechanics and client safety techniques. Strong communication and interpersonal skills. Ability to work independently and manage time effectively. Must be dependable, trustworthy, and compassionate. High school diploma or equivalent preferred. Current CPR/First Aid certification (or willingness to obtain). Background check and references required. Preferred Qualifications: Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) . Experience working with elderly or disabled individuals in a home setting. Work Environment: Work is performed in private homes and may include exposure to pets.   RN/LPN Needed – ODP Home Care Client – Bucks County, PA
  
  Prolific Homecare is seeking a compassionate, skilled, and dependable RN or LPN to provide one-on-one nursing care for a Developmental Disability (ODP) client in Bucks County, PA.

  Client Care Responsibilities:Provide tracheostomy care, including suctioning as neededManage G-J tube care for medication administration, feedings, and hydrationDeliver complete dependent patient care and assist with all activities of daily livingMonitor patient condition and maintain a safe, comfortable environmentDocument patient care accurately and communicate effectively with the interdisciplinary teamQualifications & Requirements:Active, unrestricted RN or LPN license in PennsylvaniaExperience with trach care and G-J tube management preferredValid driver’s license and reliable transportation requiredProfessional, reliable, compassionate, and patient-focusedAbility to provide attentive, hands-on care in a home settingWorking knowledge of 55 Pa.
