We are looking to hire a dynamic Physical Therapist to join our Clinical Team of Professionals!The Physical Therapist is responsible for ensuring quality total patient care through assessment, planning, interventions, and evaluation of patient needs through nursing care plans for each assigned patient; responsible for the effectiveness of measures implemented, under the direction of the physician, to resolve active problems of the patient and to provide stronger self-care techniques.Responsibilities:Performs initial and ongoing assessments to determine the level of functioning, including OASIS assessments at appropriate time pointsDevelops and revises the plan of care in consultation with the physician and other care-team membersProvides therapeutic treatments and evaluation of equipment needs as applicable; coordinates with physician and DME provider to obtain assistive devices to ensure safety and rehabilitationAdvises and consults with caregivers, family, and other agency personnel as applicableEstablishes and instructs the patient, the caretaker, or agency staff involved in assisting the patient in home exercise programs, when applicableInstructs the patient and caregiver, when applicable, in the care and proper use of equipment and devices, such as wheelchairs, braces, crutches, canes, prosthetics, and orthopedic devicesAccurately and completely prepares all clinical documentation per agency policyIdentifies and communicates changes in the patient's condition; informs the physician and obtains new orders as neededMaintains communication with the patient, family and other members of the home care team to ensure coordination of services and episode management that meets patient goals in a cost-effective mannerEvaluates outcomes of care through performance improvement activities, clinical outcomes, and adverse event reviews, clinical record reviews, and other assigned dutiesSupervises Therapy Assistants and Home Health Aides as appropriateParticipates in the care conference, staff meetings, and at least quarterly, in-service programsQualifications:Must be a Physical Therapist licensed in the state(s) of operationMust be a graduate of physical therapy curriculum approved by the American Physical Therapy Association, or a four-year degree program approved by a state department of educationMust possess sound knowledge of rehabilitation principles, procedures, and standards of practice as determined by state and federal regulationsA minimum of one year of licensed professional therapy experience preferredExperience in community or home health preferredWe are an equal opportunity employer and consider all applicants without regard to gender, marital status, race, religion, age, sexual orientation, citizenship status, veteran's status, disability or any other protected characteristics.Job Type: Full-timeBenefits:401(k)Dental insuranceEmployee assistance programFlexible scheduleHealth insurancePaid time offVision insuranceExperience:Home health: 1 year (Preferred)License/Certification:CPR Certification (Required)active MD Physical Therapist License (Required)Work Location: In person
