Education
Required a degree in physical therapy (Bachelor's or Master's)Experience
Previous experience in home health care of similar setting is preferred.Must have reliable transportation.Must have current driver's license.Minimum 1 full year Adult Home Health ExperienceMust have recent Home Health experience.Must be willing to travel up to 75 miles from home clinic.Licenses/Certifications
Must hold a valid state license to practice physical therapyAHA BLS certification required.Key Responsibilities
Patient Assessment: Evaluate patients' physical function and limitations within their home environment to develop individualized treatment plans.Treatment Implementation: Provide therapeutic interventions aimed at improving mobility, strength, balance, and pain management.Education: Educate patients and caregivers on diagnoses, safety precautions, equipment use, and home exercise programs.Collaboration: Work with an interdisciplinary team, including nurses and occupational therapists, to create comprehensive care plans that address the patient's physical, emotional, and social needs.Documentation: Maintain accurate records of patient evaluations, treatment plans, and progress according to healthcare standards.
