Position Overview
The Clinical Case Manager provides case management and clinical services to HomeRise clients. The primary emphasis of this position is to provide case management, therapy, crisis, and assessment services to adults with mental health, substance use disorder and homelessness-related issues in supported housing settings.
Primary Duties and Responsibilities
· Provides individual behavioral health counseling and case management.
· Provides group therapy and rehabilitation designed to support residents.
· Coordinates with other providers to support residents in accessing primary medical care.
· Provide outreach to HomeRise residents who may otherwise not access services by traveling to their supportive housing locations and providing on-site services.
· Refers residents as needed for mental health and/or substance use disorder treatment.
· Conducts initial psychosocial assessments and develops individualized treatment plans and adjusts as needed.
· Documentation quality and timeliness must meet agency standards.
· Provide culturally competent services using harm reduction and trauma informed principles.
· Provide consultation to staff regarding best practices, symptom management, mental health conditions and crisis intervention strategies.
· Responds to mental health crises; provides immediate access to services, de-escalates, and follow-up care.
· Maintain a productivity target of 16 hours of services per week.
· Participate in agency and outside meetings, committees and workgroups.
· Performs rotating on-call and other duties as assigned.
Minimum Qualifications
· California Licensed or License Eligible LCSW, LMFT, Psy.D., ASW, AMFT, LPCC, APCC
· Two years clinical experience working with adults with a history of homelessness or mental health/substance use disorder.
· Knowledgeable about San Francisco community resources that serve our population preferred. Strong practical and theoretical foundation in clinical MH/SUD evidence-based best practices. Must possess exceptional verbal and written communication skills.
· Proficiency in electronic medical record systems and Microsoft office platform.
· Willingness to work non-traditional hours as the job necessitates.

Title: Residential Counselor
Available Shifts: Day, Swing, and Overnight
Locations: Baker Street House, Fostel House, Robertson Place, San Jose Place
Facility Type: Licensed, DOJ and CDSS Background Clearance Required
Position Overview
The Residential Counselor provides 24-hour support at the residential facility they are assigned to support, while offering counseling services to an adult population with psychiatric, substance abuse and/or HIV-AIDS related disabilities. Residential counselors advocate for and respond to clients’ treatment and rehabilitation needs. All agency services are provided from the philosophy of social rehabilitation, which integrates teaching daily living skills in a safe and healthy environment.
This is a Full-Time, Union position with openings organization wide.
Primary Duties and Responsibilities
Perform intake interviews and assist with client treatment plan development, implementation and discharge planning.Assess the clinical needs of resident clients.Evaluate clients for potential crisis situations and determine appropriate intervention.Conduct individual and group counseling sessions as assigned.Monitor and document clients’ progress toward achievement of individual goals.Record clinical notes and hourly rounds to meet Medi-Cal standards and licensure requirements.Maintain shift coverage and primary supervision and support of residential clients.Act as a role model - guiding clients and facilitating appropriate behavior about daily living skills, self-care, personal interaction, social relationships and constructive time management.Facilitate and supervise clients with residential house chores, meal preparation, and group activities.Ensure that clients are linked with a primary care provider and assist with identifying and accessing community resources.Participate in staff and shift change meetings.Maintain professional boundaries with clients and staff.Maintain agency 30 hour per year staff development training requirement.Ability to work at a San Francisco worksiteOther duties as assigned.Minimum Qualifications
All direct care staff shall have graduated from high school or possess a GEDMinimum of one (1) year of full-time experience, or its part-time equivalent, working in a program serving people with mental disabilities.Direct experience in providing services to clients.The following Baker Places, Inc. treatment programs require a DOJ clearance: Hummingbird Valencia, Hummingbird Potrero, Leland House, Ferguson Place, CDSS clearance; Fostel House, Robertson Place, San Jose Place and Baker Street House.Physical Requirements:
Must be able to lift 20 pounds.Must be able to physically respond to clients' needs in the event of a crisis situation.Must be able to do extensive charting and documentation.Must be able to have a valid covid-19 vaccination card.Salary and Benefits:

**This is an on-call relief position, with a maximum of 20 hours per week, Non-Benefited, Non-Union role.
