    Case Manager / PAS & Crisis Coordinator (Full-Time)

    Prairieland Service Coordination, Inc. – Decatur, IL $24.04–$25.96/hour Monday–Friday | 8:00 a.m.–4:30 p.m. Full Benefits | In-Office Position

    Prairieland Service Coordination, Inc. (PSCI) is looking to welcome a Case Manager / PAS & Crisis Coordinator to our Decatur, IL office. This position focuses on completing PAS assessments, responding to crisis needs, coordinating services, and advocating for individuals with developmental disabilities and their families.

    This is an in-office position based at PSCI's Decatur office. The position requires travel throughout PSCI's service area as needed to complete job responsibilities.

    This position provides the opportunity to make a meaningful impact by helping individuals access necessary services and supports while ensuring compliance with Illinois Department of Human Services (DHS) requirements.

    What You'll Do

    Complete the PAS process in accordance with DHS PAS Manual requirements • Complete crisis-related activities, E-Packets, DHS forms, and required documentation within established deadlines • Assess individual needs and coordinate services through communication, provider linkage, service monitoring, and advocacy • Advocate for individuals and families to ensure access to eligible and preferred services • Schedule and complete Discovery and Personal Plan processes as part of the PAS process • Maintain accurate individual files and documentation according to DHS and agency guidelines • Provide problem-solving consultation with provider agencies following DHS Conflict Resolution guidelines • Complete daily billings and ensure accurate time reporting • Participate in staff meetings and required trainings • Communicate concerns, trends, and service gaps to the Administrative Program Manager • Provide coverage and support throughout PSCI's 27-county service area as needed

    Required Qualifications

    Bachelor's degree in a human services–related fieldOne year of cumulative experience providing direct services to individuals with developmental disabilitiesExperience may be gained through employment, volunteer work, or family caregiving

    Prairieland Service Coordination, Inc. (PSCI) is looking to welcome a new Housing Navigator to our Prairieland family in Maryville, IL, supporting individuals with developmental disabilities across our service area.

    This position offers a consistent weekday schedule, excellent benefits, and opportunity to make a meaningful impact by supporting individuals to secure and maintain safe, stable, and affordable community-based housing.

    What You'll Do

    Coordinate access to safe, affordable housing by identifying options, assisting with applications, and leasing processes • Educate individuals with IDD and their families on supportive housing opportunities and how to navigate the process • Collaborate with staff to assess housing needs and develop individualized housing and support plans • Build and maintain relationships with landlords and educate them on working with individuals receiving services • Assist clients with tenant readiness skills, including being a good neighbor and handling landlord-tenant concerns • Complete required referrals, maintain accurate records, and track housing progress and outcomes • Coordinate apartment outreach, showings, and support clients through the move-in process, including securing needed resources • Communicate and coordinate regularly with internal teams, service providers, and community partners • Monitor ongoing housing stability and connect clients to additional or long-term supports as needed
