    Summary of Position: 

    Responsible for carrying out the physician’s orders regarding administration of medications – Methadone.
    This individual is also responsible for the completeness of all medical aspects of the medical record – including any paper documentation and all entries to the electronic health record system. The individual in this position is responsible for provision of services in the opioid treatment program; and ensures compliance with local, state, and federal regulations and CARF standards regarding the medical treatment of opioid dependency/addiction.


    Role and Responsibilities:
    1. Ensures appropriate laboratory studies have been performed and reviewed, as needed.
    2. Ensures that a medical evaluation, including a medical history and physical examination, has been performed at admission and annually and compliance with federal, state and DEA regulations, prior to initial methadone dose.
    3. Completes physical examinations, that include a medical history, an examination of body systems on all patients; and presents medical forms for the clinical physician’s evaluation, consultation, and signature.
    4. Ensures evidence of current physiologic dependence, length of history of addiction, and exceptions to criteria for admission are documented in the patient’s record before the patient received the initial methadone dose.
    5. Completes follow up medical appointments and supports the physicians/medical director in most areas outside of dosing determinations for methadone. All initial methadone orders and dose change orders supervised by the physician.
    6. Provides education to patients on the risks and benefits of opiate treatment and medical issues surrounding the use of methadone.
    7. Follows the guidelines for the safety and treatment of high-risk patients (e.g., pregnant women, patients with HIV, and IV drug users).
    8. Coordinates care with clinical staff and other medical providers.
    9. Assists patients in referral services for other medical issues (co-occurring illnesses) and other medical needs.
    10. Continuous quality improvement (participation at in-service education and patient education).
    11. Ensures medication orders comply with State/DEA Regulations and HIPAA/CARF Standards.


    Education, License/Certifications and Experience
    - Current, Active Unrestricted License to practice medicine in the State of Colorado
    - 1 year experience as Nurse Practitioner or Physician’s Assistant in medication assisted treatment in an opiate treatment setting.
    - Must pass criminal background check.
    - CPR certified within 3 months of hire for Nurse Practitioner.
    - Current DEA License.

    - Knowledge of Opioid Treatment and Medication, ASAM Criteria.
    - Judgment: Uses good clinical judgment in resolving medical issues related to opioid treatment and medication.
    - Communication: Able to express ideas concisely and logically to ensure patient management, care, coordination, and safety.

Porch Light Health is a low barrier/high access outpatient medical clinic that treats all substance use disorders. This position entails working side by side with the medical providers to provide support to our patients. We have multiple sites, some of which are operated by mobile health units and some are embedded within community partners in rural areas. Our therapist serve all these sites through telehealth services and in person.

    The Medical Director is responsible for providing clinical leadership, medical oversight, and direct patient care services within the Opioid Treatment Program (OTP) and Medication-Assisted Treatment (MAT) programs. This role ensures the delivery of safe, effective, evidence-based care for individuals with substance use disorders while maintaining compliance with all applicable federal, state, accreditation, and regulatory requirements.



    The Medical Director serves as a member of the leadership team and provides supervision, consultation, and guidance to medical and nursing staff. This position combines administrative leadership with direct patient care responsibilities, including assessment, diagnosis, treatment planning, medication management, and ongoing medical monitoring of patients receiving addiction disorder treatment services.



    Essential Duties and Responsibilities

    Clinical Leadership and Oversight

    Provides overall medical direction and supervision of the organization's OTP and MAT services.Maintains responsibility for the quality, integrity, and effectiveness of medical and nursing services.Ensures compliance with applicable federal, state, DEA, SAMHSA, CARF, and other regulatory requirements.Collaborates with organizational leadership to establish, implement, and evaluate medical policies, procedures, and clinical protocols.Provides clinical consultation and guidance to providers, nursing staff, counselors, and interdisciplinary treatment teams.Participates in quality assurance, utilization review, risk management, and performance improvement initiatives.Serves as the medical subject matter expert regarding substance use disorder treatment and evidence-based practices.


    OTP-Specific Responsibilities

    Oversees methadone induction, stabilization, maintenance, and medically supervised withdrawal protocols.Reviews and approves dosing determinations consistent with regulatory requirements and clinical best practices.Determines eligibility for take-home medications and approves take-home schedules in accordance with federal and state regulations.Ensures proper documentation and authorization of medication orders and verbal orders.Monitors and manages the treatment of high-risk or medically complex patients, including pregnant individuals and patients with significant medical comorbidities.Ensures all required medical assessments, laboratory testing, and ongoing evaluations are completed in a timely manner.



    Supervision and Staff Development

    Provides clinical supervision and oversight to medical providers, nursing personnel, and other assigned staff.Participates in recruitment, onboarding, performance management, coaching, and professional development of medical staff.Promotes a culture of safety, accountability, collaboration, and continuous improvement.Assists with staff training related to substance use disorder treatment, MAT best practices, regulatory compliance, and clinical standards.



    Regulatory Compliance

    Maintains current knowledge of applicable regulations, accreditation standards, and evidence-based treatment practices.Ensures all medical services are delivered in accordance with applicable laws, regulations, and organizational policies.Participates in audits, surveys, accreditation reviews, and regulatory inspections as needed.Supports corrective action planning and implementation when compliance concerns are identified.



    Education, Licensure, and Experience

    Required

    Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.Current, unrestricted license to practice medicine in the State of Colorado.Current DEA registration with authority to prescribe controlled substances.Minimum of five (5) years of post-residency clinical practice experience.Minimum of three (3) years of experience working in an Opioid Treatment Program (OTP).Experience supervising or leading clinical teams.Demonstrated knowledge of federal and state OTP regulations, SAMHSA requirements, DEA regulations, and ASAM criteria.



    Preferred

    Board Certification in Addiction Medicine, Psychiatry, Internal Medicine, Family Medicine, or a related specialty.Additional experience providing Medication-Assisted Treatment (MAT) services utilizing buprenorphine and other evidence-based medications for opioid use disorder.Experience in behavioral health, addiction treatment, or integrated care settings.CARF accreditation experience.



    Knowledge, Skills, and Abilities

    Strong knowledge of substance use disorders, addiction medicine, and evidence-based treatment approaches.Thorough understanding of OTP operations, methadone treatment, MAT services, and recovery-oriented systems of care.Demonstrated leadership, supervisory, and team development skills.Excellent clinical judgment and decision-making abilities.Strong verbal and written communication skills.Ability to collaborate effectively with multidisciplinary teams.Commitment to patient-centered, trauma-informed, and culturally responsive care.Ability to maintain confidentiality and professionalism in accordance with applicable laws and ethical standards.

    Nurse works under the supervision of the Director of nursing and the clinical Manager.Nurse is responsible for the safe and the efficient dispensing and administering of Methadone to opioid dependent clients.:Documentation of medical and clinical records and observations, records keeping regarding Methadone inventory and control. Works in close coordination with other treatment team members.Interested candidates should have a strong interest in working in substance abuse field, be a team player, be organized, and have a strong interpersonal communication skills and be dependable.



    Requirements:

    LPN or RN from an accredited university nursing school licensed through the Colorado Board Of Nursing.1-2 years experience as a Methadone Dispensing NursePhlebotomy experience and/or certification

    Qualifications and Skills: 

    The ideal candidate will possess the following qualifications and skills: 

    Proficiency in Google Suite: 

    Comfortable using Google Docs, Sheets, and other Google applications.

    Experience with EMR Systems: 

    Previous experience with Electronic Medical Records (EMR) systems is preferred (at least 1 year).


    Ability to Navigate Multiple Patient Medical Record Platforms: 

    Capable of learning and adapting to various patient management systems.

    Strong Communication Skills: 

    Able to communicate effectively with patients, insurance companies, providers, and coworkers in a professional manner.

    Time Management Skills: 

    Ability to prioritize tasks and manage time effectively in a busy clinic setting.

    Dependability and Reliability:
