    POSITION SUMMARY


    This is a part time/weekend position (AM Shifts Saturday & Sunday)


    The Nurse is part of the team concept in a clinical setting for the delivery of healthcare. This position oversees client medications via monitoring, assessing clients for behavioral health and drug and alcohol withdrawals, building rapport with clients, and following up on medications and doctor appointments.

     

    ESSENTIAL DUTIES, RESPONSIBILITIES & CORE COMPETENCIES

    The essential functions include, but are not limited to, the following:

    Follow physician's instructions on medication management protocol and procedures.Monitor client’s vital signs for signs/symptoms of medication reaction.Write notes and reports, as protocol and procedures indicate.Document and report on the client's status to team members.Conduct client observations/physical checks per program guidelines. All observations shall be documented within the required timeframes.Complete nursing assessment on clients admitted to a detoxification and residential treatment program.Complete transcription of doctor’s orders pertaining to modified protocols.Manages the Medication Administration Record, documenting self-administration by the patient, efficacy, and first response.Documents nursing incidents and medication errors.Facilitates waived testing procedures.Assist clients with medications via monitoring as directed by the appropriate health care professionalObservation, documentation, audit functions, education, and record keeping of allobserved/inventoried/destroyed/prescribed medications.Facilitates waived testing protocol per manufacturer’s guidelines and company policy.Attend job-related meetings, in-service training, and staff development training; participate in team problem-solving methods; building rapport; respond to requests; and maintain a cooperative relationship among the health care team by communicating information.Administering and documenting vital signs, CIWAs, COWS, fall assessments/precautions, seizure precautions, nutritional assessments, pain scales, anxiety scales, craving scales, standard and infection control precautions.Maintains nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies, and informing the supervisor so that they may place order(s) for supplies.

    POSITION SUMMARY

    Responsible for coordinating and providing individualized individual and family therapy, group therapy and bio-psychosocial assessment services including group and family counseling for clients within the facility. The Therapist will demonstrate expertise in psychotherapeutic services, development of client resources, vocational counseling, discharge/continuing care planning, and placement of clients in collaboration with the case manager. The Therapist has the primary responsibility of maintaining communication with the referral source and other service providers as needed, as well as act as a liaison with the UR Department on all aspects of client care.     ESSENTIAL DUTIES & RESPONSIBILITIES

    ● Responsible for completing the initial treatment plan, master problem list, master treatment plan, master treatment plan reviews, aftercare recommendations, and discharge summaries in a timely manner.

    ● Completes the suicide risk assessment within the required timeframe.

    ● Screens individuals for trauma, abuse, neglect and exploitation.

    ● Provide crisis intervention and referrals for clients in cooperation with the health care practitioner and established agency guidelines.

    ● Attend, participate, and when necessary, facilitate case consultations, special events, such as seminars, lectures, retreats, trips, workshops, etc.

    ● Initiates “discharge summaries, aftercare plan; during the preliminary sessions with clients and closes client charts completing all required documentation.

    Position Summary

    The primary role of the Behavioral Health Technician (BHT) is in providing direct and support services to clients suffering from substance abuse or a co-occurring disorder. Specifically, the main functions of the BHT are 1) supervision of the clients 2) ensuring their participation in treatment related services and 3) facilitating activities of daily living 4) overseeing client safety and daily activities 5) monitoring and/or transporting clientele.  **MUST HAVE VALID DRIVERS LICENSE**

     

    Essential Duties, Responsibilities & Core Competencies

    The essential functions include, but are not limited to the following:

    ● Transport residents and participants to outside meetings, appointments, and outings on time according to the schedule. (Employee may only perform driving duties if they have a valid/unexpired CA driver’s license and are covered under the company’s insurance policy)

    ● Remain in communication with the supervisor regarding drives when needed; ensure that the interior portion of the company vehicles are clean at all times; monitor clients at all times during shift and communicate regularly with the clinical team.

    ● Responsible for following department protocol for checking-out vehicles, proper use of equipment and maintaining contact with the supervisor.

    ● Conduct searches when needed; inventory client personal property with clients upon intakes; utilize good communication skills in working with clients, their families, and significant others on a daily basis and while in crisis.

    ● Ensure that the client activity and treatment schedule is adhered to; must be a role model at all times, and if applicable, share personal experience, strength, and hope and act as a sobriety role model.

    ● Engages clients in therapeutic activities as appropriate.

    ● Provides appropriate support and structure for clients in dealing with their recovery; interacts continually with clients.

    ● Provides a brief overview of the housing program in regard to program rules, procedures, and services; with each new client.

    ● Ensure resident/participant, staff, volunteer, and vendor safety is adhered to at all times.

    ● Maintains order and ensures compliance with rules and regulations; ensures that clients conduct themselves in an orderly manner.

    ● Responsible for the cleanliness of assigned property inside and out by re-stocking items, following up client chores, assisting them, and if necessary, completing client chores.

    ● Ensures client wake-up and bedtime, escorts residents on shopping trips, recreation, meetings, medical appointments, etc.

    ● Enforces program rules at all times.

    ● Distributes daily mail to clients and observes confidentiality standards.

    ● Records accurate and up-to-date reports on every client and continually updates the bed list(s) to ensure that all admissions and discharges are recorded in a timely fashion.

    ● Reports to the supervisor of any discrepancies in the client's whereabouts.

    ● Responds appropriately to all medical and other emergencies.

    ● Conducts search of all client bags when being admitted to the facility. Confiscates and secures any contraband found and notifies the appropriate supervisor on duty in their absence.

    ● Randomly inspects all rooms, common areas, hallways and bathrooms every thirty minutes and performs a thorough search for contraband, no less than weekly.

    ● Perform drug and alcohol testing per protocol when necessary.

    ● Diligently monitor clients at all times during shift and overnight

    ● Monitors and document’s clients for signs of medical distress and/or behavioral concerns on an ongoing basis.

    Position Summary

    The primary role of the Behavioral Health Technician (BHT) is in providing direct and support services to clients suffering from substance abuse or a co-occurring disorder. Specifically, the main functions of the BHT are 1) supervision of the clients 2) ensuring their participation in treatment related services and 3) facilitating activities of daily living 4) overseeing client safety and daily activities 5) monitoring and/or transporting clientele. **MUST HAVE VALID DRIVERS LICENSE**

     

    Essential Duties, Responsibilities & Core Competencies

    The essential functions include, but are not limited to the following:

    ● Transport residents and participants to outside meetings, appointments, and outings on time according to the schedule. (Employee may only perform driving duties if they have a valid/unexpired CA driver’s license and are covered under the company’s insurance policy)

    ● Remain in communication with the supervisor regarding drives when needed; ensure that the interior portion of the company vehicles are clean at all times; monitor clients at all times during shift and communicate regularly with the clinical team.

    ● Responsible for following department protocol for checking-out vehicles, proper use of equipment and maintaining contact with the supervisor.

    ● Conduct searches when needed; inventory client personal property with clients upon intakes; utilize good communication skills in working with clients, their families, and significant others on a daily basis and while in crisis.

    ● Ensure that the client activity and treatment schedule is adhered to; must be a role model at all times, and if applicable, share personal experience, strength, and hope and act as a sobriety role model.

    ● Engages clients in therapeutic activities as appropriate.

    ● Provides appropriate support and structure for clients in dealing with their recovery; interacts continually with clients.

    ● Provides a brief overview of the housing program in regard to program rules, procedures, and services; with each new client.

    ● Ensure resident/participant, staff, volunteer, and vendor safety is adhered to at all times.

    ● Maintains order and ensures compliance with rules and regulations; ensures that clients conduct themselves in an orderly manner.

    ● Responsible for the cleanliness of assigned property inside and out by re-stocking items, following up client chores, assisting them, and if necessary, completing client chores.

    ● Ensures client wake-up and bedtime, escorts residents on shopping trips, recreation, meetings, medical appointments, etc.

    ● Enforces program rules at all times.

    ● Distributes daily mail to clients and observes confidentiality standards.

    ● Records accurate and up-to-date reports on every client and continually updates the bed list(s) to ensure that all admissions and discharges are recorded in a timely fashion.

    ● Reports to the supervisor of any discrepancies in the client's whereabouts.

    ● Responds appropriately to all medical and other emergencies.

    ● Conducts search of all client bags when being admitted to the facility. Confiscates and secures any contraband found and notifies the appropriate supervisor on duty in their absence.

    ● Randomly inspects all rooms, common areas, hallways and bathrooms every thirty minutes and performs a thorough search for contraband, no less than weekly.

    ● Perform drug and alcohol testing per protocol when necessary.

    ● Diligently monitor clients at all times during shift and overnight

    ● Monitors and document’s clients for signs of medical distress and/or behavioral concerns on an ongoing basis.
