As a Residential Care Assistant/Direct Support Specialist, you’ll provide compassionate support to help residents live their best lives. Your role includes assisting with daily activities such as hygiene, meals, and medication, creating a comfortable home environment, and fostering meaningful family and community connections.

    KEY RESPONSIBILITIES:

    Engages residents in activities of daily living to the extent of their ability and supports individual needs in areas of shopping, meal preparation, cleaning, bathing, dressing, toileting, feeding, grooming and hygiene. Supports individual health and service plans (HSP/ISP) or other goals and objectives; regularly observe, identify, document and report changes in health and / behavior. Perform household chores as needed and assist residents to determine social, recreational, or educational activities of choice. Adhering to standard health precautions, ensure well-being of residents by monitoring individual health, administering medication as prescribed. Inspects individual equipment and furnishings for proper functioning and reports malfunctions to supervisor.Ensure that safe and reliable transportation is available to residents. Drives van or other vehicle; escorts residents to scheduled programs or other activities, recreational opportunities, or medical appointments as assigned by supervisor. Prepares and communicates written progress notes relating to medication administration, individual daily activities, medical emergencies and other related areas as required by supervisor. Interacts with residents’ family members and friends, facilitate communications and engagement within the community. Attending and participating in staff meetings may assist residents to participate in house meetingsPerforms other related tasks as required by Group Home Manager Maintain a clean and safe working environment; rotation of household chores and adhere to standard health precautions. Adhere to all PILLAR policies and procedures.

    Qualifications:

    Serves as an advocate for all Pillar Care Continuum Day Program Consumers; setting an example to staff that protecting and promoting dignity, respect, independence, health, and safety is our primary commitment and responsibility. As guided by Senior Day Program Staff, serves on team within designated area of responsibility to determine daily program activities for everyone. Assists consumers to achieve the greatest degree of independence by supporting participation in program activities, always providing opportunities for choices; may provide tutorial attention to consumers in pursuit of IHP goals and objectives. Participates in community activities with consumers as assigned by Senior Habilitation / Vocational Specialist. Assists consumers with personal needs including transferring, toileting, cleaning, grooming, and feeding. Collects and records data as directed, including completing daily DTR’s within the Electronic Client Record. Responsible for recording daily attendance within Electronic Client Record.     Summary: Seeking Manager with at least 2 years DDD Management Experience.     Job Summary:

    The Early Intervention Physical Therapist provides specialized services to infants and toddlers (birth to three) who have developmental delays or disabilities. The therapist works collaboratively with families and a multidisciplinary team to promote the child's physical development in natural settings, such as the home, childcare, or community environments.     Key Responsibilities:

    The Scheduler is a critical operational role responsible for building, managing, and optimizing staff schedules across Pillar Care Continuum's adult residential group homes (19 sites) and, as directed, the Day Programs and Schools. This position works directly within ADP Workforce Management, leveraging its scheduling tools to maximize coverage efficiency, reduce overtime costs, and ensure consistent staffing levels that meet program, regulatory, and individual service plan requirements.

    Reporting to the Senior Director of Operations, the Scheduler serves as the central point of coordination between residential leadership, day program managers, HR, and direct support staff to ensure schedules are accurate, proactive, and aligned with the organization's operational goals.

    Key Responsibilities

    Schedule Development & Management

    Maintain weekly and bi-weekly staff schedules for all adult group home sites (19 homes) and day programs as assigned, using ADP Workforce ManagementEnsure all shifts are filled in advance, minimizing last-minute gaps and reactive overtimeCreate and maintain recurring schedule templates by site, shift type, and staff role (FT, PT, per diem)Manage open shifts, proactively filling vacancies with qualified staff before they escalateCoordinate schedule changes, call-outs, and coverage needs in real time, escalating unresolved gaps to the appropriate Assistant Residential Director or Day Program Manager

    ADP Workforce Management — System Administration

    Pillar Care Continuum (PCC) is seeking a dynamic and compassionate Director of Nursing (DON) to provide strategic clinical leadership and elevate the quality of care across our Residential Group Homes and Day Programs.     The Director of Nursing oversees all clinical services, ensuring the delivery of high-quality, compliant, and person-centered care. This role plays a critical part in strengthening clinical outcomes, optimizing efficiency, and empowering nursing and direct support teams.


    Key Responsibilities

    Strategic Leadership & Operations

    Partner with senior leadership to define and execute strategic initiatives that advance clinical excellence.Collaborate with Operations and HR to support onboarding, training, and staff development aligned with PCC’s mission and values.Analyze data, audits, and stakeholder feedback to identify trends, improve performance, and enhance engagement.Maintain and continuously improve policies, procedures, and clinical practices to ensure compliance with NJ Department of Health, DDD, HCBS, and DHS regulations.Lead quality assurance initiatives, including chart audits and regulatory readiness activities.

    Clinical Leadership & Oversight

    Provide leadership and oversight of all clinical operations across residential and day program settings.Supervise, mentor, and support RN and LPN staff to ensure safe, high-quality, evidence-based care.Oversee medication administration protocols and maintain clinical authority across all sites.Serve as a liaison between nursing staff, direct care teams, families/guardians, and external providers.Ensure timely, accurate clinical documentation and compliance with regulatory standards.Lead continuous quality improvement efforts to enhance care delivery and outcomes.

    Clinical Case Management

    Conduct comprehensive assessments and develop individualized care plans.Review and approve health and nursing components of Individualized Service Plans (ISPs) in accordance with DDD standards.Provide direct clinical care when needed, including wound care, IV therapy, and medication management.Participate in on-call rotation to support after-hours clinical needs.Oversee medication administration, monthly reviews, and error resolution.Manage training and certification for medication administration across residential programs.Ensure coordination and documentation of medical appointments across all programs.Regular travel (daily/weekly) throughout New Jersey to provide clinical oversight and meet supervisory visit standards across multiple group home locations

    Qualifications

This role plays a vital part in fostering student independence, socialization, and self-esteem while supporting instructional activities and therapeutic interventions.

    Job Summary:

The SLP will assess, and treat speech, language, cognitive communication.     Key Responsibilities:
