    Position Summary

    We are seeking an experienced Licensed Practical Nurse (LPN) for a full-time Triage LPN opportunity in Huntingdon, PA. The Triage LPN will provide telephone and clinical triage, assess patient needs, coordinate care, and ensure timely communication between patients, physicians, and the healthcare team.

    This is an excellent opportunity for an LPN with strong clinical judgment, excellent communication skills, and the ability to remain organized and professional in a fast-paced medical environment.

    Key Responsibilities

    Conduct telephone triage and obtain accurate patient medical histories.Assess patient symptoms and determine appropriate next steps using established clinical protocols.Communicate patient concerns and clinical information to physicians and other healthcare professionals.Respond to patient calls regarding test results, medication refills, post-operative instructions, and general medical questions.Handle complicated prior authorizations and medical clearance requests.Initiate appropriate diagnostic or therapeutic measures according to standing protocols.Coordinate patient care needs with physicians and other departments.Provide clear patient education regarding medications, treatments, testing, and follow-up care.Maintain accurate and timely documentation in the EMR.Protect patient confidentiality and maintain compliance with HIPAA and organizational policies.Provide compassionate and professional service to patients and families.Perform additional clinical duties as assigned.

    Requirements

    Graduate of an accredited LPN program.Current and active Pennsylvania LPN license.BLS certification required; ACLS preferred.3+ years of related medical or clinical experience preferred.Previous triage or telephone triage experience preferred.Strong clinical assessment and critical-thinking skills.Excellent communication, organization, and problem-solving abilities.Ability to work independently and make sound clinical decisions.Proficiency with EMR systems and basic Microsoft Office applications.
