    Neighborhood Health Centers of the Lehigh Valley is seeking a dedicated Family Medicine Physician to join our mission-driven team in a part-time/per diem capacity. This role is ideal for a physician who is passionate about community health and enjoys providing thoughtful, consultative care to patients with chronic and complex medical needs.

    Role Overview

    We are looking for a physician who preferably has experience in OB and Prenatal Care and is comfortable serving in a consultative role. You will help guide and oversee the care of our Chronic Complex patients, ensuring they receive comprehensive, coordinated support.

    Project HOME is seeking a compassionate Family Practice Physician to join its integrated health care team. This role provides comprehensive primary medical care and health education to meet the healthcare needs of the Project HOME community, including residents, neighbors in the surrounding community, individuals experiencing homelessness, and Project HOME staff. The Family Practice Physician plays a critical role in advancing Project HOME s mission through patient-centered, whole-person care.

    What You ll Do

    As a Family Practice Physician at Project HOME, you will:

    Provide primary care medical services for children and adults, and pregnant women.Have competency in basic office procedures and LARC placement.Teach medical students and precept residents as required.Collaborate with Project HOME staff and behavioral health staff on the healthcare needs of Project HOME residents.Coordinate care for all patients, with emphasis on obstetrics and gynecologyProvide occasional health education programs and community health screenings for patients, Project HOME residents, community neighbors, and staff as needed.Work independently and collaboratively with a team

    Support and demonstrate the Mission and Values of Project HOME

    What You Bring

    Minimum Qualifications

    Completion of residency in Family Medicine and Board Eligible or Board CertifiedCPR certification

    Preferred Qualifications

    Over 1 year of experience providing direct patient care in a primary care setting. Preference will be given to practitioners who have worked in an underserved area (e.g., FQHC, free clinic) and/or who have served patients who have significant behavioral health and social challenges.Experience in providing obstetrical and gynecological care.Ability to establish and strengthen relationships with the community, medical partners, and Project HOME staff across the organization.Excellent written and verbal communication skills.Strong computer and organizational skills.
