Position Purpose The Chief Clinical and Operations Officer (CCOO) provides executive leadership for the clinical, medical, and operational functions of Paragon Behavioral Health Connections. This role ensures that services are clinically sound, operationally efficient, financially sustainable, and aligned with the organization's mission, vision, and values. The CCOO carries direct executive accountability for utilization management, behavioral health outcomes reporting, and the clinical navigation of high-acuity and complex cases and drives the standardization of patient communication and person-centered care delivery across all programs. The CCOO oversees service delivery, program operations, and quality outcomes while advancing innovation, integration, and value-based care, and serves as (or directly oversees) the organization's designated Privacy Officer, ensuring the protection of patient information across clinical and operational systems. The CCOO partners closely with the Chief Financial Officer (CFO) to connect clinical and operational performance to financial strategy, translating service line, payer mix, and utilization data into growth planning, investment priorities, and long-range financial forecasts. This is accomplished through active collaboration and engagement with Center leadership, staff, community, and stakeholders. The CCOO partners with the Chief Medical Director to connect medical and prescribing services with clinical operations to ensure coordinated care and services and aligned operational practices. Organizational Structure & Direct Reports To consolidate clinical and operational accountability under a single executive and strengthen the line of sight between clinical quality, compliance, and patient experience, the following positions transition from reporting to the CEO to reporting directly to the CCOO:Chief Clinical Director leads clinical service delivery, provider practice standards, and clinical program design.Quality & Outcomes Director owns outcomes measurement, outcomes/UM reporting, and performance dashboards and licensing compliance.Product Owner prioritizes and builds the clinical, UM, and outcomes-reporting technology platforms in partnership with the CTO.PR & Communications Specialist executes standardized, patient-centered communication across all patient touchpoints, as well as external communication materials. The Chief Financial Officer, Human Resources Director and Chief Technology Director continue to report directly to the CEO. Given the technology dependencies of utilization management and outcomes reporting, the CCOO, Chief Medical Director and Chief Technology Director maintain a formal, matrixed working relationship. Education, Knowledge, Skills & Experience Required Master's or doctoral degree in a behavioral health field, with active, unrestricted license required.Ten years of progressively responsible management experience, including at least five years in community behavioral health management, spanning behavioral health administration, clinical practice, and/or other administrative experience relevant to this role.Demonstrated experience with utilization management programs, medical necessity/level-of-care review, and payer authorization processes.Proven track record building and interpreting behavioral health outcomes reporting, quality dashboards, and data-driven performance management.Working knowledge of HIPAA privacy and security requirements and experience coordinating and ensuring compliance with healthcare privacy regulations.Strong knowledge of financial principles and the diverse funding streams in community mental health organizations, with demonstrated experience partnering with finance leadership on budgeting, forecasting, and growth strategy.Demonstrated ability to implement, maintain, and lead others to a high level of clinical quality, efficiency, and compliance with administrative and best-practice standards.Excellent interpersonal, writing, and presentation skills, with the ability to communicate clinical and operational performance to boards, payers, and regulators.Experience with relevant clinical, reporting, and business computer applications (EHR, UM platforms, reporting/BI tools).     Job Description:Crisis Stabilization Clinician


    Position Overview
    We are currently seeking licensed therapists and social workers, as well as those pursuing licensure in Colorado, to join our multidisciplinary crisis stabilization team. As a Crisis Stabilization Clinician at Paragon BHC, you will be responsible for providing urgent behavioral health interventions to individuals under mental health holds and those requiring support as they transition back into the community. This team will also facilitate crisis stabilization programming using an evidenced based model. This team will transition clients to ongoing services that are conducted by our in-home clinical teams as appropriate. In this position you will work as part of a multidisciplinary team and will serve as a team leader. This position will report to a licensed clinical supervisor of the acute crisis stabilization team. This position requires in-person presence, but residing in nearby towns is acceptable.


    Key Responsibilities
    Assess urgent behavioral health needs, safety issues, and risk factors for clients and families.
    Develop well-integrated clinical formulations and safety plans in collaboration with the multidisciplinary team.
    Provide crisis stabilization interventions, mental health holds, substance use interventions, and other evidence-based therapies tailored to individual needs.
    Collaborate with law enforcement, emergency responders, hospitals, and community organizations to address safety and crisis response needs.
    Utilize Brief Cognitive Behavioral Therapy interventions for suicide prevention, and other evidence-based modalities.
    Ensure accurate and timely documentation for clinical accountability, patient safety, regulatory compliance, and reimbursement.
    Participate in Crisis Prevention and Intervention training and respond to behavioral health emergencies as needed.

    Qualifications
    Masters degree in Counseling, Social Work, or related field.
    Colorado licensure or eligibility for licensure (LCSW, LPC, LMFT, licensed clinical psychologist, or equivalent).
    Experience working with culturally diverse individuals and families, trauma, group facilitation, and crisis stabilization supports.
    Knowledge and experience in mental health and substance use assessments and consultation.
    Ability to provide interventions within diverse home and community settings.
    Strong written and oral communication skills.
    Familiarity with HIPAA, 42 CFR, protected health information, and confidentiality.

    Education:

    Paragon is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to serve as a frontline crisis care provider at our 24/7 Walk-In Crisis Center (WIC). This position plays a central role in our mission to function as a 'no wrong door' access point for individuals experiencing behavioral health crises.

    You will deliver immediate triage, medical screening, stabilization, and coordinated care to adults, youth, and families presenting in crisis including those with mental health, substance use, and co-occurring conditions. You will work as part of a multidisciplinary team operating across three 12-hour shifts per week, with one shift falling on a weekend day.

    This role is primarily in-person to ensure the highest quality of direct client care, though remote/telehealth options are available for a portion of your workweek. When scheduled for after-hours on-call, you must be reachable and ready to provide telehealth consultation or evaluation within 510 minutes of contact.

    Crisis Assessment & Triage

    Provide immediate walk-in access triage for individuals experiencing self-defined behavioral health crises

    Complete crisis screening and assessment within required timeframes per Colorado BHA Chapter 6 standards

    Conduct suicide and homicide risk assessments and administer validated tools (PHQ-9, GAD-7, PCL-5, Columbia Protocol)

    Facilitate crisis intervention, de-escalation, and brief stabilization services

    After completing required Colorado training, initiate M-1/27-65 emergency mental health holds when clinically necessary to ensure client safety


    Medical Services & Nursing Care

    Conduct comprehensive health assessments and ongoing monitoring for individuals receiving crisis services

    Monitor and document vital signs, perform basic health screenings, and administer point-of-care testing (urine drug screens, pregnancy tests, glucose monitoring)

    Administer medications including those used in Medication-Assisted Treatment (MAT/MOUD); monitor for side effects and treatment effectiveness

    Administer and track long-acting injectable medications as directed by prescribing provider

    Perform withdrawal assessment and monitoring using validated tools (CIWA, COWS); escalate appropriately

    Administer naloxone; support harm reduction interventions and naloxone distribution

    Under provider direction, enter laboratory and toxicology orders; track completion and follow up on results

    Coordinate with pharmacies; complete prior authorizations and support PDMP compliance

    Initiate emergency medical response and facilitate transfer when acute medical needs exceed WIC capacity


    Substance Use Crisis Services

    Screen and assess individuals for substance use disorders using evidence-based tools

    Conduct intoxication assessment and management per clinical protocols

    Evaluate for MAT/MOUD initiation and continuation; coordinate bridge prescriptions when clinically indicated

    Support Emergency Commitment (EC) evaluations and processing

    Coordinate warm handoffs to detoxification, withdrawal management, and substance use treatment providers


    Care Coordination & Disposition

    Determine appropriate level of care in collaboration with the multidisciplinary team

    Provide warm handoffs and referrals to outpatient behavioral health, community stabilization, Crisis Stabilization Units (CSUs), Behavioral Health Crisis Centers (BHCCs), inpatient psychiatric services and, via Provider support, to ER

    Coordinate transportation to appropriate levels of care

    Connect individuals to housing, food, case management, peer recovery, and social service resources

    Coordinate with school systems, criminal justice, and law enforcement as appropriate


    Follow-Up & Ongoing Coordination

    Conduct or support follow-up contact within 24 hours of crisis service delivery

    Confirm successful linkage to ongoing services; support appointment scheduling and referral follow-through

    Provide MAT continuity support and bridge services as needed


    Documentation & Team Participation

    Maintain accurate, timely, and compliant documentation in the electronic health record per agency and regulatory standards

    Participate in daily team meetings and contribute to individualized crisis and treatment planning

    Facilitate or co-facilitate psychoeducational groups focused on wellness, recovery, and harm reduction

    Triage client needs to appropriate team members; support provider scheduling

    Promote trauma-informed, recovery-oriented, and culturally responsive practices in all interactions


    Qualifications


    Required

    Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Colorado

    Valid Colorado driver's license and access to reliable transportation

    Current CPR/BLS certification

    Up-to-date vaccinations in accordance with agency and public health guidelines

    Minimum one to two years of experience working with individuals with serious mental illness, substance use disorders, or in crisis/emergency settings

    Ability to respond within 510 minutes when scheduled for after-hours on-call telehealth duty


    Preferred

    Bachelor of Science in Nursing (BSN) for RN applicants

    Experience in community-based, psychiatric, crisis, emergency, or integrated care settings

    Familiarity with the Assertive Community Treatment (ACT) model, trauma-informed care, and harm reduction principles

    Experience with MAT/MOUD administration and monitoring

    Familiarity with Colorado 27-65 emergency mental health hold procedures

    National certification such as Psychiatric-Mental Health Nursing (PMH-BC) or Certified Addictions Registered Nurse (CARN)

    Schedule & Work Structure

    Three 12-hour shifts per week (days/evenings/nights as assigned by team lead)

    One scheduled shift must include Saturday or Sunday

    After-hours on-call availability via phone and telehealth when scheduled must be accessible within 510 minutes for telehealth consultation and evaluation (Two or three 12-hour on call overnight shifts per week)

    Preferred: in-person at the Walk-In Crisis Center for all or most shifts

    Remote/telehealth work available for a portion of the workweek, per operational guidelines

    Part of a rotating team supporting 24/7/365 coverage

    Position Summary

    The Child & Family Care Manager at Paragon provides compassionate, family-centered, and flexible care coordination and direct support services to children, adolescents, and families within a community-based behavioral health model. This position goes beyond traditional care management Care Managers partner with families both in the home and in the community to address immediate needs, build skills, and strengthen family functioning. In addition to being trained in High Fidelity Wraparound, Care Managers provide hands-on support with resource access, parenting strategies, and youth skill development, ensuring families are empowered and supported in meaningful, practical ways.

    Position Overview

    Paragon Behavioral Health Connections is seeking a master's-level behavioral health clinician to join our growing community-based behavioral health team. This position offers a diverse clinical role combining outpatient therapy, home and community-based behavioral health services, and Jail Based Behavioral Health Services (JBBS) within the San Miguel County Jail.

    Approximately 12 hours per week will be dedicated to providing behavioral health services through Colorado's Jail Based Behavioral Health Services (JBBS) program. The remainder of the clinician's schedule will consist of outpatient therapy, child and family services, community-based behavioral health treatment, crisis follow-up services, and other clinical programming based on organizational needs and clinician expertise.

    This role is ideal for clinicians who enjoy variety, autonomy, and working across multiple service settings. The clinician will have opportunities to work with children, adolescents, adults, and families while developing expertise in serving justice-involved individuals and individuals with complex behavioral health needs.



    Essential Responsibilities



    Community-Based Behavioral Health Services (Approximately 70%)

    Provide outpatient therapy to children, adolescents, adults, and families.Deliver services in homes, schools, community settings, and office locations.Complete biopsychosocial assessments, treatment plans, and clinical documentation.Provide individual, family, and collateral therapy sessions.Utilize evidence-based practices appropriate to client needs.Coordinate care with schools, primary care providers, child welfare agencies, probation, and other community partners.Participate in multidisciplinary treatment planning and case consultation.Maintain compliance with agency, payer, and regulatory documentation requirements.Participate in clinical supervision, staff meetings, and ongoing professional development activities.Support additional community-based behavioral health programming as assigned.

    Jail Based Behavioral Health Services (Approximately 30%)

    Provide approximately 12 hours per week of in-person behavioral health services within the San Miguel County Jail.Conduct mental health and substance use screenings and assessments.Complete GAIN assessments and other JBBS-required documentation.Conduct suicide risk assessments and crisis intervention services.Provide brief individual counseling and therapeutic interventions.Develop individualized treatment and transition plans.Coordinate psychiatric evaluations, medication continuity, and MAT referrals as appropriate.Collaborate with jail staff, medical providers, probation officers, courts, and community treatment providers.Facilitate discharge planning and community reentry coordination.Complete Civicore documentation and JBBS reporting requirements.Participate in JBBS program meetings, quality improvement initiatives, and compliance activities.


    Required Qualifications

    Master's degree in Counseling, Social Work, Marriage and Family Therapy, Psychology, or a related behavioral health field.Colorado behavioral health license or license eligibility (LPCC, SWC, LMFTC, LPC, LCSW, LMFT, LAC, or equivalent).Experience working with individuals experiencing mental health and/or substance use disorders.Strong assessment, treatment planning, and documentation skills.Ability to work independently and collaboratively within multidisciplinary teams.Ability to provide services in community, home, office, and correctional settings.Valid Colorado driver's license, reliable transportation, and proof of automobile insurance.Ability to successfully complete CJIS background screening and jail security clearance requirements.

    Compensation
