With Mt. Hood National Forest and Clackamas River recreation area as its backyard, Estacada is only a 45-minute drive to downtown Portland. The city is known for its thriving arts culture, friendly downtown shops, local farmers market, and abundant outdoor recreation opportunities.

     Responsibilities:AssessmentConduct comprehensive patient assessments, including history taking, physical examinations, and evaluation of health status.Perform clinical procedures such as phlebotomy, EKGs, spirometry, wound care, and vaccine administration.Identify patient needs, barriers to care, and social determinants of health that may impact treatment plans.DiagnosisUtilize clinical judgment to identify patient health concerns and prioritize interventions.Recognize acute and chronic conditions that require coordination with providers and external specialists.Identify gaps in care and opportunities for preventive health interventions.PlanningDevelop strength-based, individualized care plans in collaboration with patients, providers, and the care team.Establish patient-centered goals for disease management, health promotion, and preventive care.Contribute to clinic quality metrics by ensuring adherence to evidence-based guidelines and best practices.ImplementationProvide direct nursing care, including triage, patient education, and chronic disease management.Coordinate with specialists, behavioral health clinicians, community health workers, and patient support staff to facilitate comprehensive care.Participate in morning huddles with the clinic team and RN team to optimize workflow and patient outcomes.Engage in community health initiatives, including patient education sessions and local outreach programs.EvaluationMonitor patient progress toward health goals and adjust care plans as needed.Gather patient and team feedback to improve care delivery and complex care management.Track and report outcomes related to quality metrics, chronic disease management, and patient satisfaction.Participate in quality improvement initiatives focused on quality metrics and improving effectiveness in comprehensive clinical care.Leadership & Professional DevelopmentSupport onboarding of new RNs, providers, behavioral health clinicians, community health workers, and patient support specialists, ensuring they understand the RN’s role in primary care.Participate in ongoing professional development, including continuing education and annual conferences, supported by a CME allowance.Contribute to the development of leadership pathways within the RN team.Required Qualifications:Active, unencumbered RN license in the appropriate state.Minimum of two years of RN experience, with preference for primary care, emergency medicine, or case management.Current BLS certification.Strong clinical skills, critical thinking, and ability to work both independently and within a team.Work Environment:While in the clinic, team members may be exposed to viruses, disease, and infection from patients and specimens in the working environment. Team members may be required to work for extended periods of time at a computer. Team members use standard office equipment, such as computers, phones, and e-fax.Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. This position requires intermittent physical activity, including standing, walking, bending, kneeling, stooping, reaching, and crouching, as well as lifting and supporting patients. Against the backdrop of thick evergreen forests, Welches is part of the Villages at Mt. Hood. The city serves as a year-round hub for outdoor enthusiasts with many great restaurants, annual events, activities, local artisans, and endless opportunities to ski, hike, and sightsee.The schedule for this position is Monday – FridayResponsibilities:Greet and room patients with a warm and friendly attitudeDocument an accurate Patient History according to the guidelines you have been given, specifically Family History, Social History, Health History, and Surgeries. At follow up visits, verify and update this information to note changes that have occurredAccurately record patient vitals including weight, height, blood pressure, heart rate, pulse oximetry, respiratory rate, head circumference, body measurements if indicatedAccurately record patient medications and input patient information into their electronic health record, and update the medication list accurately at each visitProcess prescription refills per your level of medical knowledge under supervision of a provider, following prescription refill standing ordersPerform blood draws and process patient labs according to protocols. Knowledge of Centrifuge use is required. Management of Urine Drug Screens per protocol.Knowledge of autoclave is required. The area is known for year-round sunny weather, having over 500 miles of hiking and biking trails, and being surrounded by waterfalls, hot springs, and numerous other outdoor adventure opportunities.The schedule for this position is Monday – Friday / 8:00am - 5:00pmResponsibilities:Prepare for the day’s patients by verifying insurance, scrubbing for missing information, and preparing documents before we open so that the check in process is streamlinedWarmly and professionally greet patients as they check into the clinic for their appointmentsCollect and record necessary patient intake paperwork and any patient copaysAnswer phone calls, schedule patient appointments, answer patient questions, and be able to identify when patients need to discuss questions with a medical professionalHandle incoming faxes by passing them off to the appropriate staff member and/or uploading documents to patients’ electronic health records systemCalmly and professionally respond to healthcare crises as the first line of interaction. Appropriately involve supervisor(s) and healthcare staff as neededParticipate in community events, promotional efforts, and healthcare activitiesPresent the clinic in a positive light at all times to members of the communityMaintain professional boundaries in the community at all times especially regarding PHI, HIPAA, and other sensitive informationNightly cleaning of the waiting room to ensure it is clean and presentable with fresh coffee, water, cups, tissue, business cards, stickers, magnets, etc.Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement and creating a welcoming and safe environment.Providing initial outreach and engaging patients in their care plan goals specific to Social Determinants of Health (SDOH) via ongoing follow-up and support.Providing ongoing follow-up, basic motivational interviewing and goal setting around identified SDOH needs with patients and their families.Participate in an integrated team to support chronic care management and behavioral health needs.Participate as an active member in the patient's care team in coordinating services and developing care plans.Conduct outreach efforts for newly assigned patients and patients losing insurance coverage to help patients stay covered on OHP and establish care at Orchid.Be knowledgeable about community resources appropriate to the needs of patients/families.Provide resources and referrals to community agencies and follow-up to support successful connections are established.Build and maintain relationships with community organizations and partners.Community engagement and community organizer.Follow-up on all CHW referrals and document all patient contact in the EHR.Record work performed in the EHR via cases and encounter notes no later than 24 hours after patient contact.Exhibit excellent working relations with patients, visitors, staff and community partners, effectively carrying out Orchid’s mission.Attend regular staff meetings, training(s) and other meetings as requested.Other duties as assigned.Required Qualifications:Minimum of high school diploma or GEDOne (1) year relevant experience working with underserved populations with medical and social needs in a social service and/or community-based settingUnderstand rural communities served, preference via lived experience.Experience working in medical settings and interacting collaboratively with healthcare teams preferredBasic computer skillsAbility to initiate and maintain positive working relationships with patients, staff and other organizations.Good communication skills, such as listening well, and using language appropriately.Ability and willingness to provide emotional support, encouragement and motivation to patients.Valid Driver’s LicensePreferred Qualifications:Certification as Traditional Healthcare Worker (THW) or Community Health Worker (CHW); however, Orchid will compensate for necessary job related training upon employment.EHR ExperienceWork Environment:While in the clinic, team members may be exposed to viruses, disease, and infection from patients and specimens in the working environment.     Sandy is considered one of Oregon’s highest need areas when it comes to healthcare, and taking this position will be making a huge difference in a community that truly needs and appreciates its healthcare providers.The schedule for this position is Monday – Friday (up to 1 remote day).Located at Sandy High School, Orchid Health’s Sandy Clinic welcomes all individuals in need of healthcare no matter what point of life they are in. A 45-minute drive from Portland, Sandy is known as the western gateway to the Mt. Hood Corridor, with easy access to natural areas, year-round outdoor recreation, regional arts and culture festivals, and great established restaurants.Responsibilities:Facilitate a “shared goal model” within the care team to achieve coordinated high-quality care.Consult and collaborate on a multidisciplinary team. This includes providing integrated on-site care, recommendations, and feedback to medical providers and allied staff.Utilize solution-focused, brief-intervention behavioral health visits while working as part of the primary care team to effectively identify, treat, and manage physical, mental, and behavioral health concerns.Maintain a schedule that offers flexibility to respond to same-day behavioral health needs.Participate in our complex care management team.Meet patients during their initial intake visit when possible and perform a brief interview to determine the need for behavioral health support.Other duties as assigned.Education and Experience:A Master's in Social Work or Masters of Counseling from an accredited college or university.Clinical Social Work Associate (CSWA), Personal Counselor Associate (PCA) or Licensed Marriage and Family Therapist (LMFT) required.A minimum of one (1) year post-graduate (Master's) experience of providing behavioral health services (i.e. assessment, diagnosis, or treatment of an individual's behavioral health needs) in the behavioral health field required.Candidates who have completed their Oregon clinical licensure or actively engaged in the process of obtaining Oregon clinical licensure strongly preferred.Pediatrics and integrated healthcare experience preferred.Work Environment: While in the clinic, team members may be exposed to viruses, disease, and infection from patients and specimens in the working environment. Team members may be required to work for extended periods of time at a computer. Team members use standard office equipment, such as computers, phones, and e-fax.Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The employee may be regularly required to use hands and fingers to operate a computer, telephone, and keyboard. This position requires intermittent physical activity, including standing, walking, bending, kneeling, stooping, reaching, and crouching, as well as lifting and supporting patients. Computer work is required for in-clinic positions.
