  COMPENSATION: Salary $250k + Incentive Bonus + Signing Bonus up to $50k

Transportation is provided to patients that need it at no charge.

  Role:
  

  Compensation: Base $250k-$270kBonus: Up to 10% of Base Salary.Partnership: No Buy-in. .18-22 patients a dayHours: Monday Friday work schedule; Clinic Hours 7:30a 5:00p (last patient is scheduled at 4:15)Work/Life BalanceSmall Patient Panel 450 patients maxAdministrative time included in scheduleNo nights or weekends100% outpatientDedicated Care and support team (have your own MA/Scheduler/Referral specialist)Patient Population: Seniors with 3-5 chronic diseasesValue Based Care

  Requirements:

  Residency Trained IM/FMBoard Certified/Board Eligible in: FM, IM or Geriatric Medicine
