Role: Compensation: Base $125,000 + up to 10% of base salary in bonus Primary Care Outpatient Start date: New center opens May 2025 Value Based Care Model Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.) Average patients per day: 10-12 Panel size: Max of 650 Shifts: Monday-Friday 8a-5p Call Schedule: Based on number o providers in the market (if 32 physicians in market--call 1:32wks) Nurse Practitioners have their own care team with MA/scheduler/ referral specialist and full support from leadership Benefits : Health, Vision, Dental (including family options) paid parental leave program Pre-tax or post-tax Roth contributions available.
