Job Description Job Description Nurse Practitioner Value-Based Primary Care in Indianapolis, IN making $140k Compensation: Base: $130,000+ (Flexible based on experience) Bonus: Up to $10,000 annually Sign-On Bonus Available Schedule MondayFriday, 8am5pm No nights, no weekends Limited call: 1 week, 34x per year First call handled by call center NP is rarely contacted after triage Practice Model Value-Based Outpatient Primary Care Geriatric population Small patient panel 1216 patients/day Dedicated Care Team: MA, Scheduler, Referral Specialist, Social Worker, Pharmacist Additional specialty support: Endocrinology Podiatry Role Overview The Nurse Practitioner will provide comprehensive primary care to an older adult population in a value-based model. This role involves clinic-based care with occasional home visits and telehealth support. The NP will manage chronic conditions, develop treatment plans, educate patients, collaborate with an interdisciplinary team, and maintain thorough documentation aligned with risk-adjustment expectations.
