The agency serves low-income families and individuals in the County of San Diego in two (2) strategic areas with a high density population of Filipinos/Asian and other low-income, uninsured individuals — National City (Southern San Diego County) and Mira Mesa (North Central San Diego).

    JOB PURPOSE

    Under the direct supervision of the Clinic Manager, this position provides administrative and secretarial support for the Clinic Manager and the clinic. In addition to typing, filing and scheduling, performs duties such as insurance verification, coordinating direct mailings, and working on special projects. Also, it answers non-routine correspondence and assembles highly confidential and sensitive information. Deals with a diverse group of important external callers and visitors as well as internal contacts at all levels of the organization. Independent judgment is required to plan, prioritize, and organize a diversified workload and recommend office practices or procedures changes.

    QUALIFICATIONS

    High School Diploma or GED 1-2 years’ experience in a physician office or hospital setting as a Front Desk Receptionist or registration preferred. Experience with different insurances Bilingual (English/Spanish/Tagalog).Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills. Work involves sitting, talking, hearing, using hands to handle, feel or operate objects, tools, or controls, and reaching with hands and arms. The employee may be required to push, pull, lift, and/or carry up to 20 pounds. Reasonable accommodations may be to enable individuals with disabilities to perform the essential functions. Ability to read and write, good verbal and written communication, time management, and interpersonal skills.Prioritize, meet deadlines, and exercise sound judgment.

Job Purpose The Medical Assistant plays a key role in supporting clinical operations by providing direct clinical and administrative support to the assigned Provider across multiple clinic locations, including Rancho Peasquitos and City Heights. This position follows the Provider between sites and ensures continuity of care, efficiency, and consistency in daily clinic operations. Under the supervision of a Nurse Practitioner or Physician, the Medical Assistant assists with patient preparation, basic clinical procedures, rooming, documentation support, and other duties as directed. The role requires flexibility, strong organizational skills, and the ability to adapt quickly to different clinic environments while maintaining high standards of patient care and professionalism. Essential Duties and Responsibilities Administration: Responsible for preliminary preparation of health center to include exam room, medical supply and equipment inventory. Keep exam rooms clean according to clinic Infection Control Protocol. Sterilize instruments as needed. Screen patients for vital signs, chief complaints, allergies, lab requests, smoking status, Behavioral Health, medications, and family, social, and medical history as part of intake. Prepare rooms and instruct patients regarding specifically indicated medical procedures. Perform EKG's, administer injections, and assist with medical exams as needed. Documents injections in government websites eg. SD112 San Diego immunization Registry. With Laboratory Technician's permission and supervision, perform lab procedures, including venipuncture and blood withdrawal, HCT, urines, and cultures. Administer standing orders as they pertain to patient concerns. Checking documents and monitoring vaccine refrigerator and freezer temperatures twice a day (AM/PM) and reporting out of range temperatures to supervisor or vaccine coordinator. Interpret for patient and physician as needed. Assist with follow-up for all patients, with supervision from R.N. Scan the necessary lab, x-ray, emergency room forms, or other important documents within the medical chart for the physician to be evaluated during the examination. Check paperwork in charts and encounter forms for completeness and accuracy. Keep accurate records. Participate in in-service training as requested by Clinic Manager. Assist in all areas of clinic as directed by supervisor. Assist in patient discharge from clinic and discharge instruction. Work as a member of the medical team, and assist other members of the team as deemed appropriate by the Clinician or Clinic Manager. Key data into computer to maintain office records, patient records, make appointments for follow-up and make referrals to specialist. Assist to provide patient encounter responsibility at front office as needed. Translate between the physician and the patient by explaining diets treatments or special instructions to be followed by the patient. Translations must be verbatim to what has been stated by the physician, unless instructed otherwise. Call no-show appointments and re-schedules the appointment and documents on CH. May rotate or be assigned to other areas in the clinic as needed at the discretion of the clinic manager. Utilize the Primary Care Medical Home model to provide coordinated Team Care that addresses current diseases and provides panel management for preventative and health maintenance follow-up for the maximum number of patients per day. Medical Assistants must be available to rotate Saturday shifts with their colleagues, as applicable. Qualifications Knowledge, Skills, and Abilities Must have a high level of organizational skills.     POSITION OVERVIEW

    Opsam Health is seeking a compassionate, clinically strong, and detail-oriented Registered Nurse Telephone Triage Nurse to support our Federally Qualified Health Center outpatient clinics. This position plays a key role in ensuring patients receive timely, appropriate, and safe clinical guidance during clinic hours.

    The RN Telephone Triage Nurse assesses patient symptoms by phone and electronic communication, determines the appropriate level of care, provides patient education within nursing scope, and routes urgent or emergent concerns appropriately. The nurse also supports clinical operations through medication refill workflows, prior authorization assistance, EMR messages, provider communication, and follow-up tasks in accordance with Opsam Health policies, procedures, standing orders, protocols, and regulatory requirements.

    This role requires excellent clinical judgment, strong documentation skills, professionalism, and the ability to work collaboratively with providers, medical assistants, front office staff, referral staff, and care coordination teams in a fast-paced FQHC environment.

    ESSENTIAL DUTIES AND RESPONSIBILITIESTriage incoming patient calls, portal messages, and clinical concerns using nursing judgment, approved triage protocols, and Opsam Health policies.Assess patient symptoms, identify urgent or emergent conditions, and direct patients to the appropriate level of care, including same-day appointment, provider review, urgent care, emergency department, or 911 when indicated.Document all triage encounters accurately and timely in the electronic medical record, including assessment, disposition, education provided, escalation, and follow-up plan.Provide patient education and self-care instructions within RN scope of practice and approved protocols.Escalate clinical concerns to providers, site leadership, or clinical leadership when symptoms, risks, or patient needs require additional review.Assist providers and clinic teams with EMR messages, telephone encounters, clinical task management, and patient follow-up.Support medication refill workflows by reviewing refill requests, gathering relevant clinical information, checking protocol criteria, identifying safety concerns, and routing requests appropriately.Assist with prior authorization tasks by collecting clinical documentation, supporting medication or service authorization requests, and communicating with providers, pharmacies, referral staff, and payers as needed.Follow up with patients regarding lab results, imaging results, medication issues, care instructions, and provider recommendations as assigned and within scope.Coordinate with care teams to support chronic disease management, preventive care, transitions of care, and patient access.Recognize high-risk situations, including abnormal symptoms, medication safety concerns, behavioral health concerns, and barriers to care.Maintain patient confidentiality and comply with HIPAA, FQHC requirements, nursing standards, and Opsam Health compliance expectations.Provide limited in-clinic nursing support when needed, including patient assessment, clinical procedures within RN scope, vaccine or medication-related support, and provider assistance.Participate in quality improvement, patient safety, access improvement, and workflow standardization efforts.Communicate with patients in a professional, respectful, culturally sensitive, and patient-centered manner.Follow all Opsam Health policies, procedures, standing orders, triage protocols, refill protocols, documentation standards, and escalation workflows.Perform other clinical duties as assigned to support safe and effective clinic operations.REQUIRED QUALIFICATIONSActive and unrestricted California Registered Nurse license.Minimum of 3 to 5 years of recent experience in telephone triage, ambulatory care, outpatient clinic, urgent care, emergency department, primary care, or similar clinical setting.Strong clinical assessment and critical thinking skills.Ability to identify urgent and emergent symptoms and take appropriate action.Experience using an electronic medical record system.Excellent verbal and written communication skills.Strong documentation, organization, and follow-through skills.Ability to work independently while collaborating with providers and care teams.Professional telephone etiquette and strong customer service skills.Ability to remain calm, respectful, and effective when handling difficult or urgent patient situations.Knowledge of nursing scope of practice, patient confidentiality, and healthcare compliance expectations.PREFERRED QUALIFICATIONSExperience working in a Federally Qualified Health Center, community health center, or safety-net outpatient setting.Experience with eClinicalWorks or similar EMR.Experience with medication refill protocols, prior authorization workflows, referrals, and managed care processes.Bilingual ability preferred, especially in languages commonly spoken by Opsam Health patients.Experience working with Medi-Cal, Medicare, uninsured, and medically underserved populations.Familiarity with chronic disease management, preventive care measures, and team-based care.KNOWLEDGE, SKILLS, AND ABILITIESSound nursing judgment and ability to make timely clinical decisions.Strong ability to prioritize multiple patient needs and clinical tasks.Ability to clearly explain care instructions to patients with varying levels of health literacy.Ability to work effectively with providers, medical assistants, front office staff, care coordinators, pharmacies, and external partners.Strong attention to detail when documenting, routing, and following up on clinical concerns.Commitment to patient safety, access to care, and high-quality service.Understanding of culturally responsive care in a diverse and multilingual patient population.Ability to follow standardized protocols while recognizing when provider escalation is needed.PHYSICAL AND WORK ENVIRONMENT REQUIREMENTS
