    PACE (Program of All-inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

    Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. PACE participants receive in home care services and transportation to a PACE center for primary medical care, social and recreational activities, and other senior care services.

    POSITION SUMMARY: Under the direction of the Home Care Manager, the Home Care Registered Nurse offers direct skilled care as well as supervision of care to participants receiving home care services.

    DUTIES / RESPONSIBILITIES:

    Clinical Practice: Evaluates, implements and develops a home care treatment plan for participants. Monitors participant changes and adjusts treatment plan accordingly. Provides skilled nursing care as needed.Supervision and Training: Instructs and supervises geriatric aides and LVNs in carrying out the home care treatment plan. Teaches and counsels participants and family members regarding participant's care and safety.Interdisciplinary Teamwork and Communication: Participates actively in the interdisciplinary team by offering clinical expertise in team planning and problem solving. Communicates effectively with participants, family and team members.Other related duties as assigned.

    QUALIFICATIONS (knowledge, skills, abilities):

    Graduate of an accredited college or university with either an associate or baccalaureate degree in nursing.Possess a current California Registered Nurse license. A California Public Health Nurse certificate is desired.Experience in public health nursing desired.One-year experience working with the elderly or frail population required.Ability to work as part of multidisciplinary team.Two years of acute care experience required (ICU, ER or Critical care) or homecare experienceCA Driver's license and be WILLING to drive and travel.CPR certificate.

    POSITION SUMMARY: Under the direction and supervision of the Director, Business Planning and Regulatory Affairs, interpreter/translator provides interpreter and translation services for participants,
    as requested or necessary.


    DUTIES / RESPONSIBILITIES:

    1. Provides interpretation services, including medical terminology
    2. Regular document translation of non-legally binding documents, e.g., training, medical appointments, and participant communication
    3. Works directly with team members at On Lok and participants
    4. Coordinates scheduling at different centers where services are needed
    5. Fulfills all administrative requirements, including record keeping, participating at meetings, satisfaction surveys, etc.
    6. All other reasonably related jobs as assigned

    QUALIFICATIONS (knowledge, skills, abilities):

    1. Bilingual, in either Cantonese, Mandarin, or Spanish

    POSITION SUMMARY: The Caregiver provides personal care services to On Lok participants receiving home care and/or day center services.

    DUTIES / RESPONSIBILITIES:

    Provides personal care/escort assistance according to the individual care plan (i.e. grooming, dressing, peri-care, showers, bathing, toileting, housekeeping, meal preparation/feeding, laundry) and reports observed changes in the participant's condition to the interdisciplinary team.Demonstrates safe practices, by utilizing proper body mechanics, adhering to universal precautions, and maintaining environmental safety.Interacts and provides services to participants, families and interdisciplinary team, in a professional manner (i.e. encourage participation in conversation and activities).Provides onboarding and ongoing training for other Caregivers using manuals and formal training materials as a guide.Active participation in departmental meetings and family meetings as needed.Other related duties as assigned.

    POSITION SUMMARY: On Lok has been a pioneer in delivering programs and services for the elderly, including founding the Program of All-Inclusive Care for the Elderly (PACE). Through On Lok Enhanced Care Management (ECM), On Lok serves the following populations of focus: Adults aged 21 and over at risk of long term care institutionalization, Adult nursing facility residents transitioning to the community, Adults At Risk of Avoidable Hospital or ED Utilization, and at-risk seniors using coordinated care for OR care management and targeted interventions. The Lead Care Manager 1 will be responsible for coordinating health care resources,, providing education, and facilitating access to care for a panel of members . The Lead Care Manager 1 serves as a clinical advocate for members, an active interdisciplinary team member, and a liaison with other health and social service providers in the community. Staff in this role may work with members who have mental health conditions and substance use disorders.

    Roles and Responsibilities:

    MEMBER PANEL MANAGEMENT & GROWTH

    •Manages an assigned panel of member(s) and maintains complete privacy of member personal information and health information.
    •Conducts initial and routine home visits and assessments with members in order to develop, implement and update individual care plans and interventions, and makes appropriate timely referrals.
    •Performs a minimum weekly on-site visit to On Lok facilities, based on program or operational requirements.
    •Performs a minimum weekly in-person outreach visit based on member's care plan.
    •Outreaches to new candidates and actively participates in the growth of the program.

    CARE PLAN DEVELOPMENT & MANAGEMENT

    •Identifies comorbidities, hospitalizations, prescriptions, and other factors that could impact the member's health.
    •Identifies care plan goals and objectives by working in conjunction with members.
    •Oversees and supports the implementation of the member's care plan, connecting with member via phone or in-person to facilitate engagement, follow-up, and education/training to address the care plan.

    RESOURCE & CARE COORDINATION

    •Acts as a liaison and advocates between members, their physicians as appropriate, outside healthcare and social service agencies, and other community providers as needed.
    •Collaborates with the healthcare providers to help members meet their goals.
    •Connects members to other social services and supports they may need.

    DEVELOP MEMBER RELATIONSHIPS (PATIENT ENGAGEMENT) & HEALTHCARE RECORDS MANAGEMENT

    •Builds trust with members and their families and/or caregivers by providing support and care.
    •Communicates with members about their care plan and engages other care team partners in the discussion when necessary.
    •Accurately documents all member interactions, care plans, and relevant information in the electronic health record (EHR) system in a timely and compliant manner.

    Bachelor's Degree in Social Work, Behavioral Health, or Public Health.3 Years experience working in case management and/or social service setting.Minimun 1 year Working with high-risk or vulnerable populations, serving a frail or low-income population.Knowledge and understanding of HIPAA regulations.
    Experience in facilitating positive behavioral changes in members.
    Demonstrates excellent oral and written communication skills, including proficiency in motivational interviewing and active listening, while providing consultations via phone, email, video, and in-person as needed.
