Nurse Aide Job Summary: Works with professional personnel to provide direct personal care for patients in the home. Essential Functions: Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives. Follows the Aide Assignment and provides personal care/ADLS such as: bathing, changing of bed linen, shampooing, shaving, mouth care, etc., as assigned. Maintains a clean, safe, and healthy environment. Assists with ambulation, transfers, and/or normal range of motion and positioning as assigned. Obtains vital signs as assigned. Reports any changes in patient/client condition and needs to the Case Manager and communicates with other members of the care team as needed. Maintains patient confidentiality and demonstrates courteous, polite, respectful behavior. Observes basic infection control techniques at all times and respects patient privacy. Documents any missed visits or reasons why a patient refused care. Documents and completes visits notes on paper or in the computer in a timely manner.
