Position Summary
NPORT is seeking a dedicated and compassionate Occupational Therapist (OT) to provide high-quality rehabilitative services at a skilled nursing and rehabilitation center in Middletown, NY. The OT will evaluate residents and develop individualized treatment plans designed to restore, reinforce, and enhance performance in activities of daily living (ADLs), functional mobility and overall independence.
Key Responsibilities
- Evaluate residents' physical, cognitive, and psychosocial functioning.
- Develop individualized treatment plans with measurable goals
- Provide skilled occupational therapy interventions in accordance with physician orders and regulatory guidelines.
- Train residents in ADLs, adaptive techniques, and use of assistive devices
- Collaborate with interdisciplinary team members to coordinate care
- Educate residents, families, and caregivers regarding therapy plans and progress.
- Maintain accurate and timely documentation in compliance with state, federal, and facility requirements.
- Ensure compliance with Medicare, Medicaid, and third-party payer guidelines.
Qualifications
- Graduate of an accredited Occupational Therapy program — Current New York State OT license in good standing
- Strong clinical assessment and treatment planning skills
- Excellent communication and interpersonal abilities
- Experience in skilled nursing or rehabilitation settings preferred
Physical Requirements
- Ability to lift, transfer, and assist residents as needed
- Ability to stand, walk, bend, and perform repetitive tasks throughout the workday.
