This role requires frequent travel between our Orlando & Daytona offices to ensure consistent patient care and effective coordination among healthcare providers. The ideal candidate is adept at patient assessments, treatment planning, and fostering collaborative clinical environments.
Responsibilities
Conduct patient assessments to gather medical histories and vital informationDiagnose medical conditions and develop appropriate treatment plansManage medication administration and adjustmentsEducate patients and families on diagnoses, treatment options, and care plansMaintain accurate and thorough clinical documentationCollaborate effectively with inpatient care team membersCoordinate patient care across multiple facilities considering travel requirements
Preferred Qualifications
Two or more years of clinical patient care experienceStrong clinical assessment and diagnostic skillsExpertise in patient care management and medication administrationExcellent communication and team collaboration abilitiesDemonstrated critical thinking and patient education capabilitiesSpanish-speaking a plus
