Role Essential Responsibilities:

Administer client medications and documents accordingly. Maintain current knowledge of house and client activities. Transport clients to and from appointments, intake pickups, discharge drop-offs, etc.Ensure all passengers in the vehicle are compliant, safe, and non-volatile.Plan work tasks to coincide with programming requirements. Set priorities in monitoring clients, making house rounds, documentation of pertinent information and completion of other assignments. Highly dependable and can be counted on to work independently, having the ability to work with little direction. Maintain an up-to-date knowledge of program philosophy and policies and procedures, 245G requirements, house operations, including client rights and confidentiality. Assist counselors with intake/admission procedures, orientation of new clients and co-workers. Encourage clients to achieve their goals by functioning as a positive role model. Participate in client recreational activities with a positive attitude. Serve as a resource for clients through knowledge of services and opportunities in the community. Documentation of client information in the communications log and completion of other forms.Objectively document client related topics in client files and communication logs. Northstar Mental Health Services works alongside the rest of the Northstar team in providing mental health services to those experiencing emotional distress and other mental health concerns that are interfering with their lives and overall well-being. Our integrated team of professionals work with clients individually, couples, with family, and in group settings. We are looking for individuals that possess the ability to provide psychotherapy to clients and help them to become mentally sound, emotionally balanced, socially adjusted, physically well, and spiritually alive.
Key Result Areas:
For residential clients, fulfill the mental health component of the co-occurring licensure requirements, which includes completing the initial mental health screen of all new clients within the first 3 days of arrival into program (GAIN-SS), and subsequently completing the diagnostic assessment (DA) within 10 days. Maintain a minimum of 25 direct client contact hours per week (for full time), or averaging at least 60% of available time as direct client contact if part time. This will include a combination of 1:1 or group psychotherapy, as well as facilitating co-occurring substance use groups. Complete all substance use documentation within the day of service, and mental health documentation within 72 hours of rendering the service. Fulfill expectations on maintaining documentation tracking of clients in the residential site. Attend weekly multidisciplinary clinical staffing meetings. Attend weekly departmental clinical consultation meetings. Number of grievances/maltreatment reports is no greater than 1 per month.Pass quarterly peer review internal documentation review processJob Duties and Responsibilities: (Essential Functions)
Adhering to MN State Statutes, code of ethics, insurance requirements, and Northstar’s policy and procedures.Maintain resident/client records in accordance with Northstar’s policy requirements.Complete continuing education courses in current treatment concepts and methods as required by the State of Minnesota to maintain professional license.Attend in-house training, all scheduled Northstar/Bridges staff meetings, and consultations.Hold a caseload of clients, providing all components of a psychotherapeutic episode of care from DA through termination.Coordinate care with all providers that are on the client's care team.Other duties as assigned.Requirements:Required Skills and Experience: (Minimum requirements in terms of educational background, work experience, licenses/certifications or other knowledge skills and abilities)
Must be a Licensed Mental Health Professional in the State of Minnesota to be considered for the position. ( LP, LICSW, LMFT, and/or LPCC) provisionally licensed (LPC, LAMFT, LCSW) or qualify as a clinical trainee. Provide individual therapy, group therapy, case management, crisis intervention that addresses the goals established on the treatment plan.Document services provided in a weekly note, individual note, and/or information note. Job Duties and Responsibilities: (Essential Functions)
Supervise residents to assure their health and safetyProvide peer support to those needing support.Meet with clients on a regular basis, or as needed after the initial meeting to discuss routine recovery issues from a peer perspective. Provide quality program services to the individuals served in a professional manner.Complete required documentation according to program standards and timelines.Provide guidance, encouragement, education and skill-building around wellness planning, crisis planning, and self-advocacy to support the clients' recovery goals.Act as a liaison between the clinical team and clients to ensure each client's needs and preferences are understood and honored whenever possible.Provide services that help clients identify their strengths and to use their strengths to reach their treatment goals, and identify and overcome barriers to participate in community resources.Assist in planning and supervising recreational activities for clients.Maintain confidential client records. Essential duties and responsibilities include the following (other duties may be assigned):
Administer client medications and documents accordingly. Maintain current knowledge of house and client activities. Transport clients to and from appointments, intake pickups, discharge drop-offs, etc.Ensure all passengers in the vehicle are compliant, safe, and non-volatile.Plan work tasks to coincide with programming requirements. Set priorities in monitoring clients, making house rounds, documentation of pertinent information and completion of other assignments. Highly dependable and can be counted on to work independently, having the ability to work with little direction. Maintain an up-to-date knowledge of program philosophy and policies and procedures, 245G requirements, house operations, including client rights and confidentiality. Assist counselors with intake/admission procedures, orientation of new clients and co-workers. Encourage clients to achieve their goals by functioning as a positive role model. Participate in client recreational activities with a positive attitude. Serve as a resource for clients through knowledge of services and opportunities in the community. Documentation of client information in the communications log and completion of other forms.Objectively document client related topics in client files and communication logs. Be diplomatic and exhibiting sensitivity toward other staff and client’s feelings, needs, values, and opinions. Complete clerical assignments such as faxing, copying programming materials, stocking packets/forms, typing documents in Word and updating Excel spreadsheets. Assist in food preparation, and household cleaning duties as needed. Adhere to attendance and absence policies. Punctual in reporting for work and shift change reports. Participate in rotational and/or off-shift schedules, including a holiday rotation schedule, as applicable.Able to work additional hours/off-shift as needed.Requirements:Valid Driver’s License with acceptable driving recordCPR/First Aid Certification or attain within 30 days of employmentMust be able to pass a background check Knowledge of all MN Statutes related to providing services for the population NBHN serves, and willingness to learn, understand, and carry out regulations with integrityMust understand and follow NBH's Drug and Alcohol Policy Must be at least 18 years old.
