We are seeking a Home Health Care Worker to become a part of our team! You will provide routine healthcare to patients at the patient's home.
Responsibilities:
Provide routine individualized care to persons with disabilitiesMonitor and report changes in patient health statusProvide personal care, such as bathing and dressing patientsProvide specialized care that focuses on non-medical services for membersRecord notes on me member progress on daily goals and the resultsTeach family members ways to care for patientQualifications:
Experience in hospice, home health, or other related field serviceMust have reliable transportation and clean MVRPositive and patient demeanorAbility to build rapport with patientsAbility to handle physical workloadExcellent written and verbal communication skillsQuality Customer service to membersRequirements:
