SUMMARY OF RESPONSIBILITIES:
The Supports Coordinator provides service coordination and/or targeted case management to persons with severe and persistent mental illness or an intellectual or other disability with or without a co-occurring substance use disorder according to a Plan of Service designed to promote recovery or optimize function and community integration. The Supports Coordinator serves as a link between clients’ primary and other healthcare providers to improve coordination of care and facilitate the ongoing flow of information. The Supports Coordinator advocates for justice, inclusion, and fair opportunity, and links and coordinates services toward the goal of achieving the aims of the Person-Centered Plan. This position supports Antrim/Kalkaska counties.
QUALIFICATIONS:
Education: Minimum of a bachelor’s degree in social work or other relevant human service required, with licensure/limited licensure as appropriate from State of Michigan; Must be either a Qualified Intellectual Disability Professional (QIDP) or Qualified Mental Health Professional (QMHP)
Qualified Intellectual Disability Professional (QIDP) - Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons with intellectual or developmental disabilities as part of that experience) OR one year of experience in treating or working with a person who has intellectual disability; AND is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, registered dietician, therapeutic recreation specialist, a licensed/limited-licensed professional counselor, OR a human services professional with at least a bachelor’s degree in a human services field.
Qualified Mental Health Professional (QMHP) - Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND
is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, therapeutic recreation specialist, licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, a licensed physician’s assistant, OR a human services professional with at least a bachelor’s degree in a human services field.
Experience: One year of working with the IDD or SPMI population. Able to meet requirements as a Qualified Intellectual Disability Professional (QIDP) or Qualified Mental Health Professional (QMHP). Proficient computer skills including Office 365.
Other: Ability to interact with physicians and other healthcare providers in a professional manner. Working knowledge of physical and mental health, chronic health conditions, and behavioral health medications.
ESSENTIAL JOB FUNCTIONS:
· Develop, review, deliver and modify as needed, a Person-Centered Plan of Service in collaboration with the client, parent or guardian, designated advocate, residential and day services support workers, and/or clinical professionals.
· Act as a link between the service provider system including agency staff; the client’s primary care physician, and other community health care and human services providers; and the client, and /or parent or guardian. Assist in the development of natural supports.
· Monitor the quality of services, and the health and safety of the client. Assure that services and supports authorized in the plan of service are in place and are occurring.
· Consult with nurse and/or physician on significant new, or changes in, clients’ physical healthcare needs.
· Coordinate resources and provide assistance during crisis.
· Coordinate residential and financial supports planning to help determine type and scope of supports available for residential suitability.
· Assist in attaining and maintaining benefits, such as Medicaid, SSI/SSD, food stamps, etc.
· Provide information regarding rights, alternatives to guardianship, and guardianship proceedings and paperwork.
· Provide complete, timely, and accurate documentation in the clinical record in accordance with agency policy and procedure, and Medicaid requirements.
· Monitor and ensure that applicable laws, rules, policies, and procedures are followed.
· Assist in reporting, processing, or investigating cases of alleged abuse or neglect.
· Work with agency Recipient Rights Officer and with Michigan Department of Health and Human Services Protective Services Workers or Licensing Consultants as needed.
· Assist in transporting and accompanying clients to medical appointments, evaluations, as needed.
· Comply with agency requirements for continuing education, peer review, credentialing/privileging and Qualified Intellectual Disability Professional (QIDP) and/or Qualified Mental Health Professional (QMHP) requirements.
· May be required to participate in 24-hour emergency services on-call system to conduct or coordinate emergency visits to homes or hospitals, as needed.
· May assist, arrange, or provide in-service training programs for clients, direct support workers, foster care providers, parents, guardians, and interested community members or civic organizations.
· May provide supervision to supports coordinator assistant when applicable.

Ability to walk, bend, stand, sit, lift up to 25 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity. Position requires physical ability to complete and maintain certification in de-escalation techniques which will be a training class provided to the job incumbent. · Demonstrates basic computer skills required for clinical documentation, communication and data entry.
· Ability to visit clients in their residence, educational or employment setting.
· Irregular hours will be necessary periodically.

SUMMARY OF RESPONSIBILITIES:
The Adult Behavior Specialist (ABS) is a member of the Clinical Team and is responsible for completing behavioral assessments and developing positive support plans for individuals with IDD, MI and co-occurring diagnoses. The ABS will evaluate individuals’ psychological and behavioral needs and develop and monitor behavioral treatment plans for individuals who need them. The ABS may assess clients to determine qualifications for available services and will monitor behavior plans, treatments to improve adaptive skills, and the effectiveness of behavioral treatment plans. The ABS provides counseling to clients as authorized, trains direct support staff on positive support plans, instructs about mental illness and levels of intellectual and developmental disabilities and best practice in treating individuals with co-occurring diagnoses.
QUALIFICATIONS:
Education: Master’s degree in Psychology, Counseling, or Social Work with appropriate State of Michigan License. Knowledge and experience in behavioral intervention with adults with intellectual/development disabilities and mental illness using evidenced based practices.
Experience: Experience working with adults is essential to this position. Prefer two years of experience providing services to adults with intellectual/developmental disabilities and mental illness.
Other: Lived experiences with behavioral health issues are valued.
