    Community Health Care Manager  One (CHCM’s) work with members enrolled in Medicaid to coordinate care and to connect them with essential community resources through partnerships with local physical and behavioral health providers. CHCM’s aim to enhance individual health outcomes and the overall healthcare experience by identifying each person's health and wellness goals and improving their ability to navigate the healthcare system while also working to reduce high healthcare utilization costs. The CHCM One is position provides opportunity to learn care management and gain knowledge for future growth opportunity.  Community Health Care Manager  One report to the Community Health Care Management Supervisor.

    Knowledge / Skills / Abilities 

     

    ·         Skills to communicate and interact appropriately and respectfully across multi-cultural differences.

    ·         Strong critical thinking and problem solving abilities.

    ·         Strong verbal communication skills (both over the phone and in-person).

    ·         Basic Business writing skills.

    ·         Strong customer service skills.

    ·         Ability to multi-task, prioritize, and ability to handle multiple priorities while meeting deadlines.

    ·         Computer skills including Microsoft Office (Outlook/Excel/Word), state administered data collection. systems, and other programs as needed or requested.

    Job Responsibilities/Essential Functions:

    ·         Intake of incoming referrals and provide direct contact within 48 hours of receipt.

    ·         Enter all documentation, contacts, and assessments into multiple databases with a high level of accuracy and timeliness.

    ·         Work closely with supervisors on “campaign” and care plan assignments.

    ·         Monitor members according to guidelines set by NCHA Policy and Procedure under the Care Management guidelines.

    ·         Perform needs assessments and identify risk factors to develop person centered care plans.

    ·         Conduct brief screenings and needs assessments to develop care plans and determine care management levels.

    ·         Provide short-term monitoring, support, and education to promote self-management and preventive care (e.g., well child visits, immunizations, dental, and adult screenings).

    ·         Assist members in understanding and accessing services, benefits, and community resources to address gaps in care.

    ·         Manage and document member contacts using assignment tools.

    ·         Identify and refer members requiring higher levels of care management.

    ·         Track outcomes and report data to support performance improvement and KPI goals (e.g., preventive care, ED reduction).

    ·         Review appropriate cost-effective care and decrease duplication of services for members.

    ·         Understand, communicate, and facilitate member’s complaints, grievances, and appeal processes.

    PEER RECOVERY SPECIALIST TIER 1 - Addiction Response Team (ART) - Hospital Sector

    Peer Recovery Specialist – Tier 1 is a foundational role within the Addiction Response Team (ART) providing peer-based engagement, outreach, and connection to treatment and recovery services through lived experience. Tier 1 Peer Recovery Specialists build core peer support competencies while learning ART workflows, grant requirements, Medicaid expectations, and embedded sector operations (jail, court, hospital/clinic, community).

    The Peer Recovery Specialist – Tier 1 reports to the Peer Recovery Supervisor and indirectly to the Program Manager.

     

    Hospital / Clinic Sector – Role Summary (Tier 1 Peer)

    · Complete facility onboarding and healthcare workflow training

    · Develop understanding of hospital and emergency department workflows

    · Support engagement using SBIRT and AUDIT-C screening tools, as appropriate

    · Develop familiarity with FirstNet (Banner) and EPIC (UCHealth) documentation systems as needed for coordination

    · Provide harm reduction education related to alcohol, THC, and other substances

    · Offer encouragement and peer support to individuals considering MAT or treatment services

    · Support referrals to care coordination and assist individuals with follow-up appointment navigation

    · Participate in outreach within required post-discharge timelines (e.g., 48 hours, 14 days, 45 days) through peer engagement

    Knowledge, Skills & Abilities

    Tier 1 staff are expected to build foundational knowledge and skills through training, supervision, and supported practice.

    · Minimum of two (2) years in active personal recovery from substance use

    · Ability to engage individuals through shared lived experience while maintaining professional boundaries

    · Ability to model recovery-oriented behaviors and reinforce strengths-based, person-centered approaches

    · Foundational understanding of substance use disorders, including common substances, withdrawal basics, harm reduction principles, and overdose response

    · Foundational knowledge of Medication-Assisted Treatment (MAT) and how peers support engagement in MAT

    · Awareness of co-occurring mental health conditions (recognizing signs, not diagnosing)

    · Ability to engage individuals in hospital, jail, court, office, and community-based settings

    · Cultural humility and ability to effectively engage diverse populations

    · Strong verbal and written communication skills (phone, in-person, and electronic formats)

    · Strong decision-making, critical thinking, and problem-solving abilities

    Key Responsibilities

    Peer Support & Engagement

    · Provide peer-based support through lived experience, encouragement, and recovery-focused conversation

    · Build foundational understanding of person-centered, strengths-based, and recovery-oriented language

    · Engage in training and supervised practice of motivational interviewing techniques

    · Support individuals in identifying recovery goals and next steps

    · Promote independence by sharing resources and modeling recovery behaviors

    · Act as an ambassador to individuals experiencing substance use by connecting them to treatment, recovery, harm reduction, and community support resources

     

    Member Engagement, Referrals & Follow-Up

    · Intake and contact incoming referrals within 2–24 business hours

    · Complete intake assessments and reassessments in RDP within required timelines

    · Support short-term monitoring and follow-up

    · Assist individuals with navigating appointments, referrals, follow-up care, and medication coordination

    · Participate in outreach through phone, office, hospital, jail, court, and community-based interactions

    · Attend appointments with individuals and providers, as appropriate

    · Support transitions from hospitals, crisis centers, jails, and courts into community-based care

    · Utilize referral tracking tools in RDP to support continuity of care

    · Participate in required surveys and assessments, including GPRA, BARC-10, and Recovery Domain Assessment at enrollment and follow-up intervals (as applicable to peer scope)

     

    Documentation & Data Compliance

    · Complete peer documentation in the Recovery Data Platform (RDP) within the same or next business day

    · Enter demographics, interactions, embedded locations, outreach events, and referrals in accordance with grant requirements

    · Maintain accurate and timely documentation aligned with ART workflows

    · Maintain strict confidentiality of all participant records and communications

     

    Education & Training

    · High School Diploma or GED (required)

    · Completion of required Peer Recovery Coach coursework (Preferred)

    · Mental Health First Aid (within 90 days)

     

    Licensure & Certifications

    · Valid Colorado Driver’s License and proof of insurance (required)

    Community Health Care Managers (CHCM) work with members who are patients of Sunrise Community Health Center that are enrolled in the Sliding Scale program to coordinate care and to connect them with essential community resources through partnerships with local physical and behavioral health providers CHCM’s aim to enhance individual health outcomes and the overall healthcare experience by identifying each person's health and wellness goals and improving their ability to navigate the healthcare system while also working to reduce high healthcare utilization costs.     Job Responsibilities/Essential Functions:

    ·         Intake of incoming referrals and provide direct contact within 48 hours of receipt.

    ·         Enter all documentation, contacts, and assessments into multiple databases with a high level of accuracy and timeliness.

    ·         Work closely with supervisors on “campaign” and care plan assignments.

    ·         Monitor members according to guidelines set by NCHA Policy and Procedure under the Care Management guidelines.

    ·         Perform needs assessments and identify risk factors to develop person centered care plans.

    ·         Conduct brief screenings and needs assessments to develop care plans and determine care management levels.

    ·         Provide short-term monitoring, support, and education to promote self-management and preventive care (e.g., well child visits, immunizations, dental, and adult screenings).

    ·         Assist members in understanding and accessing services, benefits, and community resources to address gaps in care.

    ·         Manage and document member contacts using assignment tools.

    ·         Identify and refer members requiring higher levels of care management.

    ·         Track outcomes and report data to support performance improvement and KPI goals (e.g., preventive care, ED reduction).

    ·         Review appropriate cost-effective care and decrease duplication of services for members.

    ·         Understand, communicate, and facilitate member’s complaints, grievances, and appeal processes.

    ·         Regular case reviews with supervisor.

    ·         Strict confidentiality of member records and communications following HIPAA Law.

    ·         Expected to develop personal competencies through ongoing education and professional development.

    ·         Focus on meeting individuals “where they are at” in their health journey.

    ·         Educate members on how to navigate the healthcare system.

    ·         Able to work independently and within a team-oriented office environment that may contain interruptions to work due to walk-ins, member needs, supervisor requests, phone conversations, etc.

    ·         Able to manage stressful situations while remaining calm and non-reactive (i.e. working with members who may have severe and persistent mental health issues or substance use disorders)

    ·         Able to utilize positive problem solving in difficult situations.

    Care Coordinators are part of the Addiction Response Team and are responsible for working in collaboration with local providers and community agencies to ensure individuals with substance use are successfully connected with resources to aid in their long-term health, treatment and recovery goals.  Care Coordinators will aid individuals in meeting their goals toward ongoing recovery through education, outreach, connectivity to Medication Assisted Treatment (MAT) and other treatment and recovery services.  Care Coordinators will report to Care Coordination Supervisor.

    Role Summary:

    This position works with individuals who are incarcerated and engaged in Medication-Assisted Treatment to provide assistance and resources for mental health, substance treatment and recovery, housing stability, food security, legal resources, and other needs. This position is focused on assisting these individuals with re-entry into the community. This position works directly with the Larimer County Jail and CorrHealth, the medical provider located in the Larimer County Jail.
