    Job DescriptionJob DescriptionNext Care Hospice is a fast growing hospice agency that provides compassionate care to our patients and their families during the end-of-life journey.

    We are committed in providing continuous training to our nurses with growth opportunities across the clinical and administrative tracks.

    We are currently in need of an experienced Per-Diem Hospice RN to give skilled nursing to hospice patients at home and senior facilities. One year minimum experience is required. The RN must have experience in hospice care and be able to bring each visit to completion with recording of the notes in the EMR system (Hospice MD)

    Our current starting per-visit rates are as follows:

    Supervisory Visits: $80/visit
    Re-Certifications: $110/visit
    New Admissions: $250/admission

    We are also hiring salaried full-time or part-time RN Case Managers.

    Our hospice patients are located in Orange County and surrounding areas. The RN is expected to conduct supervisory visits, recertifications and new admissions.

    You will be responsible for the assessment, diagnosis, and treatment of the assigned patients.

    The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. You will be compassionate and cool-tempered. You will be able to follow health and safety guidelines faithfully and consistently. You will initiate and coordinate the hospice care plan, supervise CHHAs and LVNs and provides skilled nursing hospice and palliative care services provided to our patients. The RN will work cooperatively with the attending physician, Hospice Medical Director, and other members of the interdisciplinary Hospice team. The RN regularly reviews and updates the plan of care, performs prescribed medical treatments (including pain management and symptom control), conducts assessments and evaluations and provides education and supportive care to patient and caregivers. Must be able to utilize computers and be comfortable with EMR (Hospice MD). Hospice MD training can be provided.

    QUALIFICATIONS:
    Current RN license within the State.
    Hospice experience required
    Knowledge of EMR software

    We are seeking an experienced and dynamic Hospice Agency Liaison to join our team. The Hospice Liaison serves as the connecting link between patients, families, and hospice care services, ensuring a smooth transition and personalized care plan tailored to each patients unique needs. This role involves educating and consulting with healthcare providers, patients, and families about the benefits and services provided by hospice care, facilitating a deeper understanding and acceptance of this compassionate approach to end-of-life care. By working closely with medical professionals and care teams, the Hospice Liaison helps to coordinate care that honors the patients wishes, promotes comfort, and supports the emotional well-being of both patients and their loved ones during a sensitive time. Through their efforts, they play an instrumental role in making hospice care accessible and understood, ensuring that patients receive the dignity, respect, and quality of life they deserve.

    Hospice Liaison Job Duties

    Facilitate communication between patients, families, and the hospice care team to ensure understanding of the hospice philosophy and the care plan.Assess patient eligibility for hospice care based on medical criteria and communicate findings to healthcare providers and families.Provide detailed explanations of hospice services and benefits to patients, families, and referral sources, such as hospitals and physicians.With the support of the Intake team, coordinate the initial care plan by scheduling the first visit from the hospice team and ensuring all necessary medical equipment and medications are in place.Develop and maintain professional relationships with referral sources, including attending meetings and participating in community events to promote hospice services.Document all patient interactions, referrals, and outcomes in the hospices CRM system to ensure accurate tracking and follow-up.Train and support new staff members on the hospice liaison role, including best practices for patient communication and referral source management.Participate in interdisciplinary team meetings to provide updates on patient referrals and to discuss strategies for overcoming barriers to hospice care access.Qualifications:
    Bachelors degree in Marketing, Healthcare Administration, or related field preferred.Minimum of 3 years of experience in healthcare marketing, specifically within hospice or palliative care settings.Proven networking experience across hospitals and healthcare systems.Strong communication, presentation, and interpersonal skills.Ability to work independently and as part of a team.Familiarity with EMR software and marketing tools is a plus.Compassionate demeanor with a passion for improving the quality of life for patients and their families.Benefits:
