As a WIC Registered Nurse, Nutritionist or Registered Dietitian, you will provide clinical and nutritional assessment, education, counseling, and services to participants in the Women, Infants, and Children Supplemental Food Program (WIC). Maintain patient/client confidentiality and ensures compliance with HIPAA and WIC policy and procedures.

ESSENTIAL DUTIES & RESPONSIBILITIES
Screens, assesses, and evaluates client’s nutritional status during client interview.-Determines eligibility of clients for WIC Program based on nutritional status
-Performs heights, weight measurements, as well as hemoglobin.
-Documents summary of dietary assessment, client goals and plans in client record.
-May be required to screen applicants for income eligibility in the WIC program.
Verifies applicant residency, income, identification, and Medicaid status. Completes and
reviews client registration information for participant’s electronic WIC record and sets up EBT
accounts
-Ensures certification documents and other records are accurate and complete prior to submission.
-Maintains documentation in client’s electronic record according to WIC guidelines.
-Issues required notification to ineligible participants.
Provides a Patient centered nutrition counseling approach and education to WIC participants:-Counsels high-risk participants on an individual basis.
-When appropriate, provides diet planning, counseling, and instruction to other WIC clients on individual and group basis at various Clinic locations correlating nutrition needs with general health status, culture patterns, personal preferences, and economic factors.
Promotes breastfeeding to prenatals. Works with postpartum clients who are experiencing difficulties with breastfeeding.-Makes referrals to lactation consultant as needed.
Provides referrals to community programs for specialized nutrition services/food assistance or other referrals as needed. Consults with multi-disciplinary team to assure continuity of care between programs and a comprehensive health care plan.Participates in follow-up care of clients, including by telephone, when appropriate.-Maintains documentation on all contacts made with WIC clients.
Actively participates in efforts to maintain and/or increase Clinic’s assigned caseload.Meets or exceeds individual productivity goals on a daily basis.Participates in health fairs and community outreach as assigned.Prepares lesson plans, visual aids, bulletin boards, pamphlets, and literature useful for nutrition education/classes.Checks email on a regular basis to remain current on any changes that may impact nutrition department activities. Attends and participates in meetings and inservices as required. Participates in professional development activities. Serves on committees as requested. Performs duties in accordance with NHC Standards of Conduct and Mission Statement. Education / Training
Graduate of an accredited School of Nursing, Bachelor’s Degree in Dietetics, Nutrition, or Registry Dietitian Eligible.
Experience
A minimum of one year of teaching experience maternal and child health or public health setting preferred.
Licensure/Certification
CPR certification as required by NHCI, or if an RN, an Indiana State Registered Nurse License.
Physical Requirements
Prolonged sitting, infrequent to occasional walking or standing. Requires full range of body motion, manual dexterity, and eye-hand coordination sufficient to assist clients and operate office/electronic equipment. Requires corrected vision and hearing to normal range to record, prepare, and communicate appropriate reports. Requires ability to lift and carry items weighting up to 35 lbs. Requires exposure to communicable diseases and bodily fluids. Requires ability to work under stressful conditions.
Work Environment
Normal clinic environment and offsite in various types of environments. Work may involve irregular hours, including evenings or weekends. Requires traveling outdoors in all types of weather. Involves frequent contact with other staff, physicians’ offices, and clients. Contact may involve dealing with sick, angry, or upset people. Frequent exposure to communicable diseases, toxic substances, and other conditions common to a Clinic environment. OSHA Category I – Tasks involve contact with blood or body substances.

The schedule is Monday: 8:00am – 5:00pm, Tuesday 8:00am – 5:00pm, Wednesday: 10:00am – 7:00pm, Thursday: 8:00am – 5:00pm, Friday 8:00am – 5:00pm.

As Phone Nurse, you will:
Works in conjunction with other staff to promote a “team effort”.Take complete messages and relay them to physician and/or other staff accordingly.Return patient phone calls in a timely fashion, relaying advice and instructions from the physician.Assist Clinical Manager in developing telephone policies and standing orders established at NH. Work in conjunction with Clinical leadership team to promote the medication refill policy and aid in improving patient continuity and compliance with their care plans.Monitors and responds to triage calls on the nurse phone line.Process patient’s request accordingly.Consult with providers or other staff as appropriate. If a patient request requires medical assistance, create telephone encounter for provider in Electronic Health Record (EHR) requesting guidance from provider.If call warrants a triage, the caller’s stated symptoms will be assessed to determine the appropriate level of care required to safely meet the patient’s medical needs. When appropriate, provide home care instructions using the approved, written guidelines as well as approved reference material provided. Document in Electronic Health Record (EHR) any action taken.Refer patients to appropriate medical facility or provider for care, utilizing approved guidelines as indicated by individual client profiles.Thoroughly complete documentation utilizing the appropriate computer software in compliance with the approved policies and procedures.Responsible for daily maintenance of the refill line.Create a telephone encounter for the provider or on call provider in the Electronic Health Record (EHR).Requirements:
Indiana State Licensed Practical Nurse / Registered Nurse License required. CPR certified. TB certified.Prefer two years of professional nursing experience in family practice, pediatrics, and/or ob-gyn. Prefer experience with electronic medical records.Education Requirements:

As Clinical Manager, you will:
Oversees the day-to-day operations of the nursing department:-Coordinates work assignments and schedules of the nursing staff to ensure optimum patient flow.
-Actively participates in patient complaint procedures to resolve operational problems.
-Ensures compliance with NH policies and procedures, Federal/State regulations (ex: OSHA, HIPAA), and other grant requirements.
Ensures department is staffed with qualified, competent employees:-Interviews prospective employees and makes recommendations for hire.
-Provides all new hires with proper training and orientation to scheduling and the medical front desk.
-Evaluates staff performance and recommends merit increases, promotions, and disciplinary actions.
Ensures department is functioning well as a team:-Communicates departmental goals and sets “expectations of performance” for each team member.
-Identifies, analyzes, and facilitates resolution of any issues which are a barrier to the team achieving its goals.
-Plays a vital role in the success of NH employee communication by keeping both staff and administration informed of any changes, ideas, concerns, etc.
-Analyzes the resources (staffing, equipment, technology, etc.) available to the department and when appropriate, recommends changes to improve efficiencies.
Prepares staff timecards for payroll processing. Monitors staff attendance, tardiness, and overtime trends.Performs supervisory duties in accordance with the Managers Standards of Conduct and NH Mission Statement.Requirements:
Minimum of five (5) years of experience in the nursing field. Supervisor experience preferred.Experience in medical office management preferred.Experience in electronic medical records required. Experience with childhood immunizations a plus.Education Requirements:
Graduate of an accredited school of nursing required. Bachelor’s degree preferred. Indiana State Registered Nurse or Licensed Practical Nurse license required. CPR certification, TB certification.This position is full-time with benefits. Support patient’s health by reviewing information with patient’s, report test and examination results, ensures provider notes and signatures are documented for all orders, consultations, and exams.Aid patient’s in administering pregnancy tests, urine tests, hemoglobin, lead levels, glucose tests, EKG, HgbAlc, non-stress tests, hearing tests, retinal photography, and vision screens.Assist patients with continued education on their health status, immunizations, preventative health, laboratory tests, personal hygiene, birth control, physical exams, prenatal, and post-partum care.Requirements:
