    Program Manager - Health - Recuperative Care

    Position works at Facility

    The Program Manager is tasked with ensuring effective operations of NHF recuperative care facility. Direct reports include Housekeeping, Direct Care staff, Direct Care Supervisors during the coordinator’s absences.


    Responsibilities and Initiatives

    To help NHF meet its growth goals, the Program Manager will:

    • Oversee the quality of guest/patient services provided meet NHF standards by implementing and monitoring day-to-day operational systems and procedures in accordance with the company’s standards of operation; coordinate and assign daily routines and coverages of operation team, providing coaching and training when needed.

    • Develop, implement, and consistently monitor health and safety measures including but not limited to, hazard communications, evacuation procedures, incident management.

    • Collaborate with LVN Nursing Manager to properly dispose of medical waste.

    • Coordinate with internal and external partners for special events held at the facility.

    • Conducts weekly site safety inspections and coordinates with housekeeping to ensure necessary repairs are completed.

    • Collaborates with interdisciplinary team to coordinate guest’s quality of care, ensure all staff supervise permissible to accurately utilized bed board application to determine census cap for operational planning (i.e.) room changes, meal planning etc.

    • Collaborate with Director and Leadership of any pertinent information regarding the operations on a regular basis. Maintain, manage, and review department budget with Director.

    • Flexibility to adapt to schedule changes and meet the on-call requirement to appropriately triage issues and provide solutions within program operations.

    • Ensure that all direct reports execute immediate crisis intervention/emergency assistance to guests and/other staff members.

    • Ensure all incident reports and/reporting, Emergency Action Plan (EAP) documentation are reviewed and meet compliance. Provide training as needed.

    • Assist Director in developing and maintaining a system of education, in-services and training for all staff and people supported.

    • Collaborate with Guest Leads or designee to complete weekly inventory for operations department.

    • Complete and ensure all operational software is being utilized to complete daily tasks.

    • Coordinate with Administrative assistant to ensure all supply orders are placed and received to include any issues with transportation.

    • Create an environment of growth and development by delegating duties to the team, assign special projects as needed to operations staff, etc.

    • Complete and conduct additional tasks/duties as assigned to meet the needs of the facility, notwithstanding, primary responsibilities.

    • Review and approve all timecards operations department, that includes but not limited to Housekeeping, Direct Guest Support Team, and Department Leads, in a timely matter. This includes making any timecard corrections, having all employees approve timecard prior to manager approval.

    • Ensure that all operations staff members are compliant with CA labor laws such as breaks, lunch periods, not working off the clock, etc.

    • Collaborate with People and Culture department on any issues or questions as needed. • Conduct monthly department meetings in collaboration with monthly staff meetings and/engagement.

    • Conduct yearly performance appraisal, bi-monthly one-on-one supervision with direct reports.

    • Conduct corrective actions plans as needed for staff, up to and including termination with the support of People and Culture. • Work in collaboration with community liaison/security guard to ensure safety of guests, staff, volunteers/interns, and visitors on the property. Work with approved Staffing agencies for temporary temp support. Must maintain clear communication with all agencies on performance and replacement.

    • Trains staff in the use of security and surveillance systems.

    • Observe department guidelines in conducting periodic review of the active staff and potential needs for training and or performance notices.

    • Self-identify potentially unsafe systems/processes/situations and takes initiative to report to the Director.

    • Complete and conduct additional tasks/duties as assigned to meet the needs of the facility, notwithstanding, primary responsibilities.

    • Always follow NHF policies and procedures

    RequirementsBachelor's degree preferred.4-5 years’ supervisory experience with demonstrated experience of coaching and mentoring staff in related field.Familiarity working with LAHSA regulations.Knowledge of OSHA regulations preferred but not required.Current BLS certification strongly preferred but not required.Prior working experience in residential treatment facility, mental health facility, substance abuse treatment center, or homeless shelter preferred. • Excellent verbal/written communication skills required.Must have the ability to multi-task, problem solve, meet deadlines, follow through, and manage a team in a fast-paced environment.Ability to communicate effectively with all levels of staff, guests, and visitors.

    Medical Coordinator/LVN

    The Medical Coordinator works in our Recuperative Care Program to provide post-hospital care, while creating a healthy, safe, and home like environment for guest quality of life. The Recuperative Care Program provides quality and compassionate care and empowers those we serve.

    Responsibilities and Initiatives

    Guest Services Associate

    Behavior Support Specialist

    Behavior Support Specialist (BSS) utilizes a behavior analysis method to help provide behavioral improvement for recuperative care guests and others with developmental and behavioral challenges. By observing and responding to different behaviors, Behavior Specialists provide guidance and may intervene to recuperative care guests with the intention of helping to alter behavioral challenges in an observable way. The end goal for Behavior Support Specialists is to help guests make changes in behavior that allow them to more easily manage social interactions and situations.

    Responsibilities and Initiatives

    To help NHF meet its growth goals, Behavior Support Specialist will:

    Study guest behavior and apply ABA principles.Collaborate with the Interdisciplinary Team (IDT) and Director of Recuperative Care to create a robust behavior support plan when needed.Respond appropriately to different situations common among guests experiencing homelessness and others with behavioral and developmental challenges.Utilize key communication skills to provide effective feedback to recuperative care guests.Effectively communicate positive feedback to recuperative care guests.Review and document guest progress through staff communication, charting and completing required reporting.Collaborate with guests’ medical provider team to incorporate behavior treatment plan into the recuperative care guest lives.Communicate with family, GSAs, nursing, social services and other team members and partners to ensure they are aware of how to assist guests when needed.Train both staff and guest in implementing behavior treatment plan during the recuperative care guest stay and post-transition.Be able to recognize and respond to critical improvements in guest behaviors as well as crisis intervention.Become familiar with and use behavioral redirection techniques.Know how to respond to negative behaviors appropriately and professionally.Provide written documentation on each recuperative care guest involved in an incident.Participate, advocate and support guests and staff during VCBH meetings and appointments, as needed.Consult with external psychiatrist, medical team and guests to assist with psychological medication regimen for maintenance and stability.On call availability to attend and address crisis onsite.     Responsibilities and Initiatives

    Ensure safety and security of recuperative care facility. Conduct daily facility and equipment inspections. Conduct constant room checks throughout the day.Perform guest intake responsibilities, ensuring that all guests are greeted and provided with a detailed orientation of program guidelines, when applicable. Develop rapport with guests and garner guest feedback.Create, lead, and/or implement engagement activities with our guests, when applicable. Build rapport and trust with our guests through constant engagement and checking in. Attend to our guests’ requests.Assist nursing, social services and external partners with guest appointments on-site or check ins.Assist management in accurately maintaining bed availability and documentation.Assist Lead staff or Program Management with supplies inventory check/ log. Maintain accurate record keeping including but not limited to daily shift notes, incident reports, Emergency Action Plan drills, etc. when applicable.Respond to and provide positive intervention encounters for guest concerns.Attend all meetings and training that are aligned with the role which includes shift hand off, and trainings such as, but not limited to; BLS, Crisis Prevention, Harm Reduction, Trauma Informed Care, DEI, etc.Perform laundry, clothing requests and mealtime duties and in the absence of the environmental site associate perform housekeeping services (if applicable).Must be flexible to adapt to schedule changes and/or work additional shifts to meet the needs of the facility, notwithstanding labor laws.Must model and project a positive professional image and adhere to the organization dress code.Perform additional assignments to meet the needs of the facility, notwithstanding primary responsibilities.Always follow NHF policies and procedures.

    Requirements

    High School Diploma or equivalent required.Minimum 2 years caregiver experience in a Healthcare or Recuperative Care environment (preferred).Ability to perform and prioritize multiple tasks effectively and efficiently.Must be able to work a schedule with five (5) eight (8) hour shifts.Valid CA driver’s license.Current CPR and First Aid certification.Excellent oral and written communication skills.Ability to communicate effectively with all levels of staff, guests, and visitors.Working knowledge of Microsoft Office Suite.

    Physical Requirements

    The duties as described will required frequent standing and walking. The employee will frequently bend/stoop, squat, reach above the shoulders, twist, and turn, kneel, and push/pull.
