Are you an experienced RN with exceptional nursing skills and enjoys working with patients in a home setting? We are looking for individuals that have experience providing the following nursing services:
Perform Health AssessmentsSchedule and perform Supervisory Visits to other skilled nurses (RN/LPNs), certified nurse assistants (CNAs) and personal care aides (PCAs)Provide Care CoordinationJob Description:
Responsible for providing skilled nursing care and services to patients in their place of residence, and may case manage nursing and non-skilled care and services when assigned. All nursing services will be performed according to compliance with state and local laws/ regulations, the Nurse Practice Act and all other healthcare accreditation standards.
Initiates and identifies client's needs through assessmentProvides nursing services based on the plan of carePrepares necessary items for treatment and proceduresImplements and uses current standards to prevent spread of infection, instructs and may prepare medications as appropriate for the patient's care and safetyMakes treatment recommendations based upon the patient's acuity level and goal potentialExcels at the admissions process including same-day electronic documentationFlexible to accomplish admission with time, travel time or geographic constraintsThis clinician provides and documents skilled care on all visitsDocumentation skills must meet agency standardsPosition requires driving throughout dayOther Requirements:
Able to follow regulation guidelines and deadlines with submission of clinical paperworkComputer skills are a must, typing skills for reporting is needed and to update documentation in online portalDemonstrates willingness and ability to work with electronic input and telecommunication devicesOrganized and timely with clinical paperworkExcellent clinical skills and demonstrated ability to manage time wellExcellent communication skills; must be able to communicate in a manner that is understandable to the staff and general public verbally and in writingHow to Apply:
