Job DescriptionJob Description The role of the Registered Nurse (RN) is to work collaboratively with providers and the care team to provide safe, efficient, and effective care to empaneled patients. Under the clinical direction of the Chief Medical Officer (CMO) or designee, the RN (1) facilitates health and disease management, (2) uses evidence-based care to improve quality and patient outcomes, (3) assists in transitions of care, and (4) is responsible for assessing, planning, implementing, and evaluating an organized program for staff education directed toward the achievement of professional excellence. Serves as a point of contact for nursing and clinical support service staff education. Contributes to the creation of clinical resources and instructional leadership for outreach programs directed to health care professionals. The following information provides an overview of the skills, qualities, and qualifications needed for this role. Essential Functions: Performs short evaluation of the patient situation to estimate severity of illness and/or injury including learning about chief complaint and determines next steps. Sends those with high-risk chief complaints such as to Emergency Department (ED) immediately. Shall schedule those patients with non-emergent symptoms as soon as possible and may provide appropriate home care advice until patient is seen by a provider. Maintains follow up of all patients directed to the ED per established protocols. Acts, when designated, in Ask a Nurse capacity, handling routine information requests from patients. Acts as a clinical leader in the department and is a clinical resource for support staff. Responds to emergent patient needs and conducts triage as needed. Participates in assessment and evaluation to sign off support staffs' competency skills checklists. Functions as a liaison with community health professionals to promote professional/education programs and services. Designs, conducts, monitors, and evaluates educational programs for new and current nursing and clinical support staff. Recommends direction of program focus. Assesses educational needs of the learner and develops plans for learning experiences appropriate for meeting recognized needs. Maintains regular and predictable attendance. Performs other duties as assigned. QUALIFICATIONS Minimum Qualifications: Registered Nurse with associate degree in Nursing or related field. At least two years of professional experience. AZ Registered Nurse License in good standing. Current CPR certificate.
