    What YOU'LL Do:

    Be a Proactive Health Advocate: Ensure individuals receive all necessary routine and preventative healthcare by scheduling, coordinating, transporting, and communicating effectively with external healthcare providers and internal partners at the agency.

    Monitor and Manage Health Status: Provide ongoing monitoring of health, quickly recognize changes in condition, and maintain thorough, accurate healthcare documentation and records to promote the person's best possible health.

    Empower Through Education: Deliver essential health-related training and education like infection control and medication administration to the Med Aides and support staff and the people we serve.
