    Dominion House Residence Counselor - Part Time

    We DO NOT do personal care personal care AND we DO NOT do physical restraints. Everything we do with our clients is verbal engagement and interactions including supportive counseling and teaching.

    Responsibilities of the Residence Counselor are to provide services to adults who have a diagnosis of mental illness and living in a group home. Specific duties of the Residence Counselor included:

    Provide supportive counseling;Providing rehabilitation skills such as teaching of daily living skills including medication management and coping skills, crisis intervention and prevention;Recreation activities for socialization and stress reduction; andMaintaining a healthy and clean environment for our clients ensuring clients are doing their chores correctly and by assisting with cleaning activities. Promote a culture of inclusion and belonging. Provide transportation to clients using the agency’s vehicles.

    Requirements

    Skills/Qualifications for the Residence Counselor include:

    College classes in human services, BA preferred.Demonstrated ability in helping others.Valid and insurable driver's license. Agency vehicles are available for transporting clients. Must have a personal vehicle to use for business purposes when not transporting clients. Experience working with people who have a diagnosis of mental illness or other human service experience.Both the ability to work independently and as a team member.Excellent communication skills both verbal and written.The ability to be organized and have good time management skills.

    Benefits

    · $22.08/hour; 20 hours per week; $22,963.20/annual equivalent plus benefits.

    Service Coordinator at Certified Apartment Program

    Responsibilities of the Apartment Counselor are to provide services to adults who have a diagnosis mental illness and living in individual apartments throughout the Schenectady community. The goal is to work with the client in stabilizing at in his/her apartment while also providing rehabilitative services so he/she can transition to the next level of independence. Specific duties included:

    Providing supportive counseling to assist in stabilizing.Everything we do with our clients is verbal including verbal intervention skills. There is no personal care and no physical restraints.Crisis prevention skills and coping skills.Verbal crisis intervention.Medication supervision by bringing the medication to the clients’ apartment for refills based on the step system and number of days the clients is eligible to have the medication. Although we don’t watch clients ingest the medications, we do a count at the visit to make sure the correct amount of medication is in the medication organizer. In addition, we have a conversation with the clients about taking the medications and teach clients about the medication, side effects, etc.Teaching of daily living skills.Maintaining a healthy and clean environment for our clients to become independent which includes some cleaning. Socialization skills and recreational activities.Promote a culture of inclusion and belonging.     40 hours per week. Monday through Friday, 8:30am to 4:30pm.$22.66/hour; $47,132.80/annual equivalent. Client phone on-call for a 1-week period approximately every 10 weeks. $150 on-call bonus during this period.     Responsibilities of the Psychiatric Registered Nurses:

    Mohawk Opportunities is seeking 2 full-time Psychiatric Registered Nurses. One full-time nurse as a clinical staff member of the Assertive Community Treatment (ACT) Team and the other full-time nurse as a clinical staff member of the Forensic Assertive Community Treatment (FACT) Team. Both teams are a community based multidisciplinary treatment team located in Schenectady, NY. Service provision is designed to promote recovery by improving psychiatric symptoms, preventing relapses, teaching skills, and providing direct assistance and securing necessary community resources. These services may occur in the home, community, or office. Specific duties Include:

    · Conduct person-centered comprehensive health assessments. Develop and implement strength-based treatment plans that are responsive to client needs and facilitate achievement of goals on an as needed basis.

    · Oversee all aspects of medication management in compliance with agency procedures and protocols. This includes administration of injectable medications, managing Clozaril protocols, ordering of medication, overseeing processes for medication distribution, utilizing a medication step system towards client self-management, and managing all aspects of the medication room.

    · Develop and maintain trusting and supportive relationships with clients, families, and community providers. All service provisions shall be judgement free, non-confrontational, yet honest, open, and delivered in a professional, caring, supportive, and growth producing manner.

    · Provide support, treatment, case management, community advocacy, and linkage services, per the treatment plan. Complete required documentation of interventions as well as client’s responses and outcomes.

    · Readily respond to all crisis situations and urgent needs using sound clinical judgement in order to ensure the safety and well-being of the individual. Provide on-call crisis intervention services on a rotating schedule amongst team members. Always act to ensure client’s safety, readily assess needs, provide support, and intervene immediately as needed.

    · Mohawk's programs and offices are multi-level with stairs only. All employees must be able to travel to all levels of the facility in a safe and reasonable time frame. Client services in the community include visits to clients’ apartments which can be on the second and third floors.

    · Provide transportation to clients when necessary, using agency vehicles. In addition, an employee must have a valid NYS driver’s license and must be an approved driver. An employee must always drive in a safe manner, use all safety devices and be medically able to drive. Employees must have access and be willing to use their personal vehicle to attend client/member meetings in the community and meetings outside the office/program.

    Requirements

    Skills/Qualifications for the Psychiatric Registered Nurse:

    Licensed Registered Professional Nurse by New York State. Bachelor’s degree in nursing preferred.Three years’ experience in a psychiatric setting, with at least one year’s experience in an outpatient setting is preferred. Must have a strong commitment to the rights and ability of each person with a mental illness to make their own decisions and choices, live in the community, work, and have access to continuous support and services to enhance wellness.Must be able to effectively work with all people and demonstrate skills and competence to establish supportive and trusting relationships with all individuals.Must uphold individuals’ rights and personal preferences in treatment and treat everyone with respect and dignity. Continuously acting as a guest in individuals’ homes and family homes, is courteous, polite and an active listener.Valid and insurable NYS driver's license. Must have your own vehicle to use for business purposes. Agency vehicles are available for client transportation.

    Dominion House Case Manager

    Responsibilities of the Dominion House Case Manager is the development of all individual service plans for the 12 adults who have a diagnosis of mental illness and are living in a stabilization group home located in Schenectady, NY. In addition, this person will provide supportive, goal-oriented counseling with a focus on decision-making and problem-solving skills and developing and implementing all resident service plans. Specific duties include:

    Development of client service planning to ensure that the client’s services are creative and specifically designed to meet the client’s goals, needs, and desires. Spend individual time with the clients to develop a strong understanding of the clients’ individual goals, needs, and desires and how best for the clients learn new skills. Coordinate regular service planning meetings with the clients and primary counselors.Assist clients in applying for benefit entitlements. Connect clients to all appropriate funding sources upon admission to program, including but not limited to Department of Social Services (DSS) and Social Security Administration (SSA). Advocate with or on behalf of clients to other agencies to ensure access to needed service.Complete financial paperwork and input all pertinent information on case management notes. Communicate regularly with the Finance Dept including requesting client’s checks and reconcile clients’ accounts. Providing supportive, goal-oriented counseling with a focus on decision-making, problem-solving skills; and developing and implementing service plans. Providing supportive counseling to assist in stabilization; crisis prevention skills and coping skills; crisis intervention. Teaching of daily living skills including medication supervision and teaching of medication management. Socialization skills and recreational activities.Maintaining a healthy and clean environment for clients to become independent which includes cleaning.
