    The Adult Mental Health Targeted Case Manager (TCM) provides culturally appropriate support for adults with Serious and Persistent Mental Illness (SPMI). The primary goal is to help clients access needed medical, social, educational, vocational, and other services related to their mental health needs while addressing all social determinants of the client’s health. This community-based role requires the Case Manager to meet clients in their homes and community settings to promote stability and independence.


    2. Essential Duties and Responsibilities

    Conduct comprehensive functional assessments when meeting the clients and their support, and update them semi-annually.

    Develop a collaborative individual community support plan with the client and their support network, and update it every 180 days to reflect current goals and needs.

    In a person-centered approach, refer the client to community resources and natural supports identified in the assessment and support plan to meet their needs.

    Contact the client monthly to review their progress towards support plan goals and address any concerns or barriers.

    Partner with clients to identify strengths, cultural values, and self-identified goals to promote recovery and community integration.

    Connect clients to crisis intervention resources within 24 hours of a reported crisis.

    Accept and process all county referrals, including those for individuals under civil commitment.

    Serve as an advocate for clients to ensure they receive the benefits and services for which they are eligible.

    Maintain accurate and timely documentation of all client contacts, assessments, and plans in the designated Electronic Health Record (EHR) system.

    Adhere strictly to the Minnesota Government Data Practices Act (MGDPA), HIPAA, and the Minnesota Health Records Act. Provide every client with a "Notice of Privacy Practices" and "Tennessen Warning" at initiation.

    Record client-related activities (assessment, ICSP development, referral/linkage, monitoring) accurately. Ensure invoices are submitted within required timeframes.


    3. Qualifications and RequirementsMinimum Qualifications

    Must meet one of the following criteria:

    A bachelor’s degree in one of the behavioral sciences or related fields (psychology, social work, nursing, etc.) from an accredited college or university, AND at least 2,000 hours of supervised experience in the delivery of services to adults with mental illness; OR

    A graduate degree in one of the behavioral sciences or related fields; OR

    Is a registered nurse without a bachelor's degree, but who has at least three years of experience in the delivery of services to adults with mental illness.


    Skills and Competencies

    Must adhere to the company’s professional boundaries and clinical ethics policies, including the strict prohibition of dual relationships.

    Ability to use company-issued mobile devices and navigate the EHR platform's user interface.

    Must have a valid driver's license, reliable vehicle, and maintain required insurance levels for community travel.



    5. Work Environment and Physical Demands

    Hybrid Work Structure: This position involves both office-based administrative work and community-based client visits. The company reserves the right to adjust work arrangements based on performance and/or business needs.

    The Waiver Case Manager provides comprehensive, person-centered case management services to individuals receiving Minnesota waiver services. This role ensures compliance with all applicable state, county, and federal regulations while advocating for client needs, coordinating services, and maintaining accurate and timely documentation.

    The ideal candidate is organized, responsive, culturally competent, and able to manage a dynamic caseload while meeting documentation, billing, and communication standards.

    Key Responsibilities


    1. Client Services & Case Management
    Conduct intake interviews and eligibility reviews for prospective clients.Complete comprehensive assessments, social histories, and required diagnostic documentation.Initiate client contact within five (5) business days of assignment and complete an initial face-to-face visit within ten (10) business days.Develop, implement, and regularly update individualized service plans in collaboration with clients and interdisciplinary teams (IDTs).Serve as a client advocate, ensuring client rights, informed choice, cultural responsiveness, and ethical service delivery.Assist clients in identifying, accessing, and coordinating medical, social, financial, educational, and waiver services.Facilitate or participate in IDT meetings and ensure appropriate coverage if unable to attend.Monitor service effectiveness and recommend adjustments as client needs evolve.2. Documentation, Compliance & Communication
    Maintain accurate, complete, and timely case notes documenting all client-related activities.Obtain and renew Consent for Release of Information every six (6) months.Provide required notices (e.g., Notice of Privacy Practices, county grievance forms) at intake and required intervals, ensuring proper documentation.Maintain organized and audit-ready client files in accordance with MNCCS and county standards.Review incoming documents within required timelines and complete all follow-up actions promptly.Respond to phone messages within three (3) business days and maintain appropriate voicemail messaging.Review and respond to emails daily, prioritizing urgent matters same day.

    3. Billing & Productivity Expectations
    Accurately track and record all billable and non-billable activities in compliance with waiver billing rules.Meet organizational productivity standards, including maintaining at least 34 billable work hours per week.Review client budgets monthly and notify the supervisor of concerns or discrepancies.Submit required admission, discharge, and status-change documentation to supervisors and billing staff in a timely manner.Maintain daily task trackers reflecting work activity and billing compliance.4. Administrative & General Duties
    Submit expense and mileage reimbursement requests accurately and within required timelines.Attend required staff meetings and review meeting minutes if absent.Maintain current auto insurance documentation as required by MNCCS.Prepare and provide casebooks for audits when requested.Perform other job-related duties as assigned by supervision or leadership.

    Position Requirements & Work Conditions
    Valid driver’s license and ability to meet MNCCS insurance requirements.Reliable transportation for required job duties.Daily local travel within assigned service areas.Inability to maintain driving eligibility may result in reassignment if available.

As a contractor, you will manage your own schedule and caseload while ensuring all services meet DHS regulatory standards.


    Primary Responsibilities

    Provide one-on-one rehabilitative services in the client’s home or community, focusing on:

    Basic living and social skills.

    Community intervention and integration.

    Medication education and symptom management.

    Household management and financial literacy

    Transitioning to community living.

    Collaborate with the Mental Health Professional to implement and track progress on Individual Treatment Plans.

    Maintain high-quality, HIPAA-compliant progress notes. Ensure all documentation is completed within 24–48 hours of service to meet billing and audit requirements.

    Help clients identify triggers and develop coping strategies; provide immediate assistance or referrals during mental health crises.

    Communicate effectively with the client’s broader care team, including case managers, psychiatrists, and family members.


    Mandatory Qualifications

    To qualify as a Mental Health Practitioner in Minnesota, the contractor must meet one of the following criteria:

    Has at least 4,000 hours of supervised experience in the delivery of services to adults with mental illness; Or

    Holds a bachelor’s degree in a behavioral science/related field and has at least 2,000 hours of supervised experience.


    Additional Requirements:

    Must pass a Minnesota DHS Background Study.

    Must have a valid driver’s license, reliable vehicle, and active auto insurance.

    Must complete the required 30 hours of ARMHS-specific training if not already completed.


    Contractor-Specific Details

    This is a 1099 Independent Contractor role.

    Compensation: Typically paid per billable hour (range: $25.00 – $40.00/hr, depending on experience and whether travel is reimbursed.

    You are responsible for your own tax withholdings, health insurance, and laptop/mobile phone.

    You will participate in mandatory clinical supervision (monthly or as required by your license/statute) provided by the agency’s Mental Health Professional.


    Preferred Skills
