The Clinically Integrated Network (CIN) Care Management team will work in collaboration with the client, physician(s), and other resources to ensure adherence with plan of care to achieve an optimal level of wellness for each client. The CIN Care Manager will perform duties as a registered nurse, case manager, and an educator to assess, coordinate, and educate the high risk and rising risk client population to achieve their optimal level of wellness.
