    Job Title/Position: Registered Nurse

    Reports To: Director of Nursing

    Job Description Summary

     The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience.  The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

     

    Essential Job Functions/Responsibilities

     

    Patient Care

    Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs.  Provides a complete physical assessment and history of current and previous illness(es).

    Provides professional nursing care by utilizing all elements of nursing process.

    Assesses and evaluates patient’s status by:

    Writing and initiating plan of care

    Regularly re-evaluating patient and family/caregiver needs

    Participating in revising the plan of care as necessary

    Initiates the plan of care and makes necessary revisions as patient status and needs change.

    Uses health assessment data to determine nursing diagnosis.

    Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.  Includes the patient and the family in the planning process.

    Initiates appropriate preventive and rehabilitative nursing procedures.  Administers medications and treatments as prescribed by the physician in the physician’s plan of care.

    Counsels the patient and family in meeting nursing and related needs.

    Provides health care instructions to the patient as appropriate per assessment and plan.

    Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.

    Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

     

    Communication

    Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.  Records pain/symptom management changes/outcomes as appropriate.

    Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.

    Communicates with community health related persons to coordinate the care plan.

    Teaches the patient and family/caregiver self-care techniques as appropriate.  Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.  Works in concert with the interdisciplinary group.

    Provides and maintains a safe environment for the patient.

    Assists the patient and family/caregiver and other team members in providing continuity of care.

    Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.

    Attends interdisciplinary group meetings.

     

    Additional Duties

    Ensures that arrangements for equipment and other necessary items and services
    are available.

    Supervises ancillary personnel and delegates responsibilities when required.

    Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes.

    Fulfills the obligation of requested and/or accepted case assignments.

    Actively participates in quality assessment performance improvement teams and activities.


     

    Position Qualifications

    Graduate of an accredited school of nursing.  One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.

    Current licensure in State and CPR certification.

    Bachelor’s degree, with one (1) year of hospice care experience preferred.

    Management experience not required.  Responsible for supervising hospice aides.

    Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

    Prolonged or considerable walking or standing.  Able to lift, position and/or transfer patients.   Able to lift supplies and equipment.  Considerable reaching, stooping bending, kneeling and/or crouching.  Visual acuity and hearing to perform required nursing skills.

    Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

    Job Description Summary

     

    The social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.

     

    Essential Job Functions/Responsibilities

    Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.

    Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.

    Assesses for, and educates interdisciplinary group, on any special needs related to the culture of the patient and family. Includes communication, role of family, space, and any special traditions or taboos.

    Maintains clinical records on all patients referred to social work.

    Educates patients and families on, and assists in, preparation of advanced directives.

    Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.

    Provides information to patients and families/caregivers and community agencies.

    Serves as liaison between patients and families/caregivers and community agencies.

    Maintains collaborative relationships with organization personnel to support patient care.

    Maintains and develops contracts with public and private agencies as resources for patient and personnel.

    Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.

    Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.

    Actively participates in quality assessment performance improvement teams and activities.

    Assists family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities.

    Other duties as delegated by the Clinical Supervisor.

    Position Qualifications

    A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education

    OR

    A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3).

    Minimum of one (1) years’ experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.

    Experience in a hospice care preferred.

    Demonstrates good verbal and written communication, and organization skills.

    Possesses and maintains current CPR Certification.

    Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

    Job Summary: This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction.     Essential Functions:

    Assesses the patient's physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical assessment with patient/family and assures completion within appropriate time frame. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.

    Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psycho social, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patients status.Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.Documents assessment, planning, implementation and evaluation in the patient record.
