Key Responsibilities
Perform comprehensive patient assessments and ongoing evaluationsDevelop, implement, and revise individualized care plansAdminister medications, injections, and IV therapies as prescribedProvide wound care and monitor healing progressMonitor and manage chronic conditions (e.g., diabetes, heart failure, COPD)Educate patients and family members on disease management, medications, and safetyDocument all patient care accurately in electronic health records (EHR)Communicate changes in patient condition to physicians and care teamsCoordinate care with physical therapists, occupational therapists, social workers, and other healthcare professionalsEnsure compliance with agency policies, state regulations, and federal guidelinesParticipate in case conferences and quality improvement activities
Required Qualifications
Active Registered Nurse (RN) license in the state of practiceGraduate of an accredited nursing programCurrent BLS/CPR certificationMinimum of 1–2 years of clinical nursing experience (home health experience preferred)Valid driver’s license and reliable transportationAbility to work independently and manage time effectively
Preferred Qualifications
Experience in home health, hospice, geriatrics, or medical-surgical nursingStrong wound care and IV therapy skillsKnowledge of Medicare/Medicaid regulationsOASIS documentation experience
