The nurse Care Manager (CM) is responsible for reviewing and approving Plans of Care, CANS-NY assessments, and UM review for HCBS services. The CM is also responsible for care management for medically fragile children and supporting the coordination of services related to inpatient, homecare, specialty medications, and DME according to UM requirements set forth by the "office of Health Insurance Programs Principles for Medically Fragile Children. This role will work closely with various operational, medical management, and IT departments across the organization. The successful candidate must have healthcare experience and knowledge in planning membership and claims data, able to work autonomously and collaboratively with other Health Analytic team members, and with various subject matter experts and stakeholders across the organization.
