PROGRAM BACKGROUND:
To meet the needs of New York City’s ever-changing foster care population, Enhanced Family Foster Care (EFFC) combines and replaces the previous separately contracted Family Foster Care (FFC) and Treatment Family Foster Care (TFFC) programs. EFFC recognizes that all children in care have experienced some level of trauma, that their needs may change over time, and that their strengths must be supported so they can reach their full potential.
POSITION SUMMARY:
A Caseworker works with children and youth who reside in a foster home. The foster parents can either be relatives or individuals who are not related to the children. Caseworkers work as a team with Socio-Therapists to provide supports to the children, the foster parents, and the birth families to ensure that every child is making progress towards permanency.
REQUIRED QUALIFICATIONS:
RESPONSIBILITIES:
Provide intensive support to children, therapeutic foster parents and birth families.Conduct regular casework sessions with each client in accordance with Federal, State, and Local mandates.Conduct regular home visits to therapeutic foster homesMaintain regular contact with birth parents to provide support and problem solveMaintain regular contact foster parents, and/or resource personsHolds bi-weekly individual Sociotherapist sessions with each client and family for their first 3 months in programDocument all contacts and case activities in the State system of Connections, Agency patient tracking systemsComplete documentation within established timeframesPrepare all required documentation on each case including progress notes, psychosocial reports, permanency hearing notes, FASPsAttend legal proceedings such as family court hearings, representing the agency and client’s best interestsActively participate in staff training and supervision meetings.Arrange, monitor and document family and/or sibling visits.Respond to emergencies as needed and/or directedBENEFITS/PERKS:

provides 24-hour residential care and supportive services for youth with a history of trauma and the presence of complex needs. The residential sites throughout NYC and Long Island locations serve a variety of needs including a Specialized Diagnostic Center, Qualified Residential Treatment Programs and Residential Programs for Pregnant and Parenting youth. These residential programs provide youth with structure and supervision designed to provide evidence-based treatment to promote emotional and physical safety and stability.
POSITION SUMMARY:
The Family Team Conferencing Facilitator is responsible for working the residential team to schedule and lead the Family Team Conference that engages members of the family, and their natural support in discussions related to safety, well-being and plans for permanency. The Family Team Conference Facilitator is responsible for facilitating and tracking all agency-led conferences, utilizing the Family Team Conference Model. The facilitator will ensure the safety and well-being of each child and family is assessed through a strength-based lens. Utilizing strong engagement skills, the facilitator will yield positive safety and permanency outcomes for children and families with the input from a multidisciplinary team and various stakeholders.
REQUIRED QUALIFICATIONS:
Master’s degree in Human Services field required with 2 years of experience in casework within ACS programs REQUIRED.Prior experience leading group sessions preferred.Prior experience working with children and families known to the Child Welfare system, impacted by Abuse and Trauma preferred.Strong Engagement, Organizational and Communication skills.Knowledge of and commitment to child welfare best practice standards.Understanding and willingness to commit to the agency’s mission, core values, Sanctuary commitments.Ability to accommodate a flexible work schedule, including evenings.Proficiency in Microsoft Office suiteDemonstrate competency in child development.Demonstrate cultural competency.

PROGRAM BACKGROUND:
Admission to our specialized programs are determined by Family Court recommendations where children are placed in the most appropriate, least restrictive setting. MercyFirst has residential treatment settings for these children and plan for their successful discharge from the moment they are admitted to our programs. While with us, the children, adolescents, attend school on our Syosset campus, receive medical and clinical services. The children and their families work with a multi-disciplinary team to prepare for discharge and to ensure a smooth transition.
SUMMARY:
Under the Supervision of the Sr. Director of Long Island Residential Programs, the Aftercare Caseworker will provide the youth and their family with continued support from day 1 to up to a year after they return home. The goals are successful discharge for both youth and family and assisting in helping children stay out of residential and/or foster care in the future.
REQUIRED QUALIFICATIONS:
Minimum of an MSW or Masters in related field. New grads considered!Prior experience working with adolescents helpful (internship, volunteer, work experience considered)Prior experience with at risk youth strongly preferred.Prior casework experience required.
