    AGENCY BACKGROUND:

    PROGRAM BACKGROUND:
    The Performance and Quality Improvement (PQI) Department is responsible for the agency's information, program and operational quality, compliance, program data tracking, reporting and quality/performance improvement process across MercyFirst. PQI ensures MercyFirst meets all regulatory and contractual program performance and facility requirements across the agency. Led by the Chief Quality Officer, PQI staff support programs the above areas along with conducting continuous quality improvement activities such as case record reviews, utilization reviews, stakeholder surveys, KPIs and outcome measures.

    POSITION SUMMARY:
    The FTC Facilitator is responsible for helping to identify issues that may threaten the safety of children at home, address the status of behaviors or conditions that present risk of abuse or maltreatment to the child, and to help family members create a plan that will address these risks while promoting the advancement of a permanency and/or stabilization plan for the child/family. The FTC Facilitator will utilize a strength-based perspective in guiding a processes that is solution focused, and maintain a neutral and/or objective perspective while guiding the problem-solving solutions raised by staff, children, and/or family members.

    Reporting Relationship: FTC Supervisor

    Qualifications:

    Bachelor’s in social services (e.g., social work, psychology, sociology, etc.) with 2 years casework and 1-year group work experience and/or 1-year supervisory experience.Knowledge of child welfare and understanding of systemic approaches for engaging families in team conferences and regulatory procedures and practices for child welfare facilitation preferred.Effective negotiation skills and the ability to develop effective working relationships with programs and clients.Flexibility, and good engagement and assessment skills, including strong oral, writing, and organizational skills.Experience working with multiple stakeholders and develop collaborative relationships.Detail-oriented and able to keep accurate, well-organized records; able to work independently but know when to seek assistance.Travel may be required to facilitate conferences at MercyFirst program sites for Foster Care, Residential, and Prenatal/Postpartum/Parenting (PPP) programs.Proficiency in Word, Excel, PowerPoint, and the ability to learn city state and federal data systems including NYS’ CONNECTIONS case management system, as well as MercyFirst’s internal case management systems as necessary.

    Responsibilities:

    Facilitate and coordinate Family Team Conferences (FTCs) in both Family Foster Care (e.g., EFFC, Therapeutic Foster Care), and Residential (e.g., PPP programs) to ensure case plan decisions reflect MercyFirst values and principles.Maintain fidelity to the FTC model: Establish an atmosphere conducive to facilitating an FTC that is strength-based, solution-focused, and engages all stakeholders in a participatory process to assist in reaching consensus; raise objective questions to initiate “straight talk” among the stakeholders and guide the problem-solving process.Communicate with direct supervisor to troubleshoot decisions prior to cancelling or rescheduling an FTC if a stakeholder will not be present or arriving late for the conference.Assure all stakeholders have the FTC summary report, which is inclusive of next steps, timeframes, and any referral source(s).Anticipate and communicate with the FTC Coordinator any incident that occurred during an FTC that may require additional administrative intervention such as identified safety issues, reasons that should deem a removal, or concerns that may have not required reaching consensus but may impact attaining the goals set forth on behalf of the case.Attend and participate in weekly supervision to report on the FTCs held, and raise concerns that are pertinent to the follow up of supervisors and staff in addressing the needs and goals identified at each conference.Practice flexibility in work schedule to assure FTCs occur.Complete other assignments as needed or designated by the FTC Coordinator and Chief Quality Officer.Submit weekly calendar to Unit Supervisors/Program Director in family foster care, preventive, and residential programs to reflect all FTCs that will be facilitated.Ensure timely documentation of FTC summary report in CNNX to be contemporaneous with when the conference was held; also enter information within agency’s internal system of record, i.e., ASARA as required, and track outcomes via reporting.Ensure that solutions and/or next steps are measurable, have time limits, and are matched to the needs of clients by identifying and outlining when and how to accomplish next steps to achieve programmatic goals.Track all FTCs held and support quality improvement-related data collection and analysis, including providing reports on a monthly basis to family foster care, residential, PPP, and preventive programs.Meet with program staff to ensure that the programs are meeting facilitation targets, including providing training, support, and technical assistance.

    PROGRAM BACKGROUND:

    To meet the needs of New York City’s ever-changing foster care population, Enhanced Family Foster Care (EFFC) combines and replaces the previous separately contracted Family Foster Care (FFC) and Treatment Family Foster Care (TFFC) programs. EFFC recognizes that all children in care have experienced some level of trauma, that their needs may change over time, and that their strengths must be supported so they can reach their full potential.


    POSITION SUMMARY:

    A Caseworker works with children and youth who reside in a foster home. The foster parents can either be relatives or individuals who are not related to the children. Caseworkers work as a team with Socio-Therapists to provide supports to the children, the foster parents, and the birth families to ensure that every child is making progress towards permanency.


    REQUIRED QUALIFICATIONS:

    PROGRAM BACKGROUND:

    MercyFirst’s A Safe Way Forward Program (SWF) provides child welfare involved families affected by intimate partner violence (IPV) with comprehensive prevention services including therapeutic case management and evidence-based intervention modalities to address IPV. This joint prevention-clinical services model offers comprehensive trauma-informed services to the whole family, including the survivor of IPV, the person causing harm (PCH), and to children exposed to the IPV in the home. Range of services includes in-home visitation, crisis intervention, case management, advocacy, individual, group and family counseling, psycho-education and support, and referral to other appropriate services. The SWF program will serve a total of 65 families residing in Brooklyn.

     

    POSITION SUMMARY:

    The Therapeutic Case Manager is responsible for screenings and assessment, engagement, safety planning and coordination of services and treatment for members of the family system experiencing IPV. Approach to service is family-centered, trauma-informed, strengths-based and emphasizes both short-term and long-term goals to restore and maintain Safety for all family members, Protect children from future exposure to IPV, and promote Healing and Improved Family Relationships. The qualified candidate will be responsible for adherence to reporting requirements and timelines and meeting programmatic outcome measures.

     

    REQUIRED QUALIFICATIONS:

    PROGRAM BACKGROUND:

    Enhanced Hard-to-Place (HTP) Group Home program, providing specialized residential services to adolescents, between the ages of 13 to 18, in 2 gender-specific homes, with a maximum capacity of 8 in each house. The group home offers community-based, home-like atmosphere while providing a structured and therapeutic environment to meet the residents’ needs. The program is designed for individuals with a history of behavioral difficulties that cannot be successfully maintained in their family home setting, a history of multiple unsuccessful placements, as well as victims of abuse/maltreatment and trauma. The program provides individual, group, and family therapy with 24-hour supervision within a highly structured therapeutic milieu


    SUMMARY:

    This position is responsible for daytime administrative oversight of programming within the ACT Program. The qualified candidate will provide onsite supervision and support to the Youth Support Counselors and residents in the two group homes. During their shift, this individual will be responsible for the reporting and documentation of all critical incidents, including notification to external stakeholders and families. Additionally, this individual will be the point person for responding to emergencies and coordinating with Program/Clinical Administration on-call as needed during their shift.

     

    REQUIRED QUALIFICATIONS:

    Bachelor’s Degree or High School Diploma with 5+ years of experience in residential programming administration; Excellent Leadership, Organizational, Supervision, Communication and Analytical skills.Knowledge and training in working with adolescents & families; Knowledge and training in working with direct care staff.Understanding and willingness to commit to the agency’s mission, core values and Sanctuary commitments.Valid New York State Driver’s License, satisfactory driving record and ability to travel in and around the Long Island region.Schedule will include evenings/weekend hours.

     

    RESPONSIBILITIES:

    The Day Supervisor will report directly to the Assistant Director of Long Island Residential Programs and provide after-hours supervision to the two sites.The Day Supervisor will be responsible for continuous monitoring of video camera surveillance in the programs.This individual will be responsible for coordination with the clinical department for all clinical assessments needed during their shift.The Day Supervisor will also be the point person for directing transportation and unit staff as needed and documentation of all critical incidents.

    SUMMARY:

    The Psychiatric Nurse Practitioner shall serve as a member of the clinical staff and provide psychiatric input to the treatment planning team. Prescribe and monitor children’s medications and educate families of benefits, risks, and side effects.

    POSITION SUMMARY:

    The Housing Specialist is responsible for working with birth parents and youth in our Family Foster Care and Residential programs. The Housing Specialist will help our clients secure and obtain a permanent place to live. The ideal candidate will have experience working with at risk youth and have a strong knowledge base of the following: NYCHA, Supportive Housing, Section 8, and other related programs.

    POSITION SUMMARY:

    Provide assistance and support to employees regarding their benefits (medical, dental, FMLA, etc.). Provide required recordkeeping via HR computer system as well as insurance carrier's computer system. We are a team, looking a team member. Together we support all of HR's administration needs.     MercyFirst’s A Safe Way Forward Program (SWF) provides child welfare involved families affected by intimate partner violence (IPV) with comprehensive prevention services including therapeutic case management and evidence-based intervention modalities to address IPV. This joint prevention-clinical services model offers comprehensive trauma-informed services to the whole family, including the survivor of IPV, the person causing harm (PCH), and to children exposed to the IPV in the home. Range of services includes in-home visitation, crisis intervention, case management, advocacy, individual, group and family counseling, psycho-education and support, and referral to other appropriate services. The SWF program will serve a total of 65 families residing in Brooklyn.

     

    POSITION SUMMARY:

    The Therapeutic Case Manager is responsible for screenings and assessment, engagement, safety planning and coordination of services and treatment for members of the family system experiencing IPV. Approach to service is family-centered, trauma-informed, strengths-based and emphasizes both short-term and long-term goals to restore and maintain Safety for all family members, Protect children from future exposure to IPV, and promote Healing and Improved Family Relationships. The qualified candidate will be responsible for adherence to reporting requirements and timelines and meeting programmatic outcome measures.
