    POSITION SUMMARY:

     

    Reporting to the PRP Manager, the Rehabilitation Specialist assists with developing/facilitating groups/skill building modules to support individuals with mental illness in their recovery. The rehab specialist works in a

    group setting coaching and teaching strategies as well as assisting with recreational outings to integrate

    individuals in their community.

     

    ESSENTIAL RESPONSIBILITIES:

     

    • Participate in the development and implementation of skill-building modules.

    • Be knowledgeable of person-centered practices

    • Participate in the development and implementation of the individualized rehabilitation/ service plan;

    provide direct rehabilitative and clinical services to individuals with disabilities; implement an

    intervention, coaching, and teaching strategies in accordance with the plan, the program, and the needs

    of the individual; provide services at multiple locations throughout the community.

    • Identify crisis or potential crises and intervene to facilitate the best possible outcome; maintain calm

    demeanor throughout therapeutic interventions; utilize behavior principles, strategies, and techniques as

    needed in accordance with Humanim training; enforce and abide by program rules, policies, and

    procedures.

    • Assist with planning and coordination of activities and outings based on individual needs/plans; for

    individuals with short-term service plans, assist with developing and implementing effective

    individualized transition plans in preparation for exiting the program to independence.

    • Monitor medications in accordance with Humanim protocols if necessary and if required by the

    department; maintain, protect, and share clinical knowledge of assigned clients as appropriate.

    • Complete all rehabilitation documentation in accordance with program requirements: daily log or

    contact note; monthly progress report; IP/IRP reviews, and assessments; maintain all program

    operations and consumer records in compliance with state and federal regulations as directed by the

    program; complete incident reports as needed within 24 hours of occurrence; complete/submit all

    personnel/ agency documentation within required timeframes.

    • Maintain communications with the treatment team – internally and within the provider community;

    participate in clinical team meetings as needed; document contacts as necessary; Notify supervisor and

    treatment team members of changes in clients’ status promptly; Contact on-call supervisor in case of

    urgent or emergencies after hours.

    • Work as part of a multi-disciplinary team; coordinate and facilitate visits and meetings, as well as report

    progress and exchange information in accordance with program requirements.

    • Attend supervision meetings as requested; attend and participate in all program and agency

    meetings/training/activities.

    • Maintain an overall attendance record of greater than 97%, measured over a minimum of a year; provide

    proper notice of necessary lateness or absence; follow procedures for requesting time off.

    • Maintain positive, professional work behavior with supervisory staff, co-workers, consumers, families,

    funders, employers, and the community; convey a professional image when representing Humanim at

    the agency and in the community.

    • Prepare for and attend meetings on time; successfully complete all required training on time; follow all

    departmental and agency policies and procedures; maintain all emergency and safety requirements.

    • Maintain and provide verification of valid driver's license and automobile insurance annually; maintain

    a safe driving record with an accumulation of fewer than five points; use personal vehicle to transport

    clients to necessary appointments/programs/recreational activities, and other destinations as appropriate;

    Use a personal vehicle to shadow clients on agency transportation if necessary.

    • Provide rehabilitative interventions to promote clients’ use of a variety of transportation resources and

    adhere to all procedures for use of HUMANIM vans.

    • Other Duties as assigned.

     

    PHYSICAL DEMANDS AND WORK ENVIRONMENT:

     

    This position must have the ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations. While performing the duties of this position the employee will constantly operate a computer and other office productivity equipment. This position requires standing, moving, walking, sitting, reaching with hands and arms, stooping, kneeling, crouching, and the ability to drive agency vehicles. Frequently may be required to move/lift to 20 pounds. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.     Position Summary


    The Day Support Specialist provides direct support and supervision to individuals with disabilities, promoting independence, community engagement, life skills development, and vocational preparedness. This position facilitates educational and recreational activities while ensuring compliance with program standards, regulatory requirements, and organizational policies.


    Essential Functions

    Provide direct support, supervision, and assistance to individuals with disabilities in a day support setting.Implement educational training, activities, and lessons that focus on life skills, recreation, career exploration, and vocational preparedness.Attend and facilitate community integration activities that promote learning, independence, and social engagement.Collaborate with the Day Support Program team to cultivate community partnerships and develop meaningful community-based opportunities.Maintain compliance with all county, state, and federal regulations governing human services providers, including participant supervision ratios, documentation requirements, and safety standards.Promote a safe, ethical, inclusive, and professional environment for participants and staff.Assist with special projects, including documentation reviews and audits.Complete timely and accurate daily documentation, progress notes, and program records.Transport participants to and from community integration activities and program-related events as needed.Perform other duties as assigned.


    Qualifications

    Education and Experience

    High school diploma or GED required; Vocational or technical training in a related field preferred.Minimum of one (1) year of experience working with individuals with intellectual and developmental disabilities or in a related human services environment.

    Reporting to the Program Manager of EBSE, the Employment Specialist (EBSE) is responsible to boost Humanim’s Employment Services programs in Maryland by promoting and marketing their services through relationship building with existing and potential referral sources, identifying new referral partners, identifying market opportunities, and raising brand awareness within the designated service area.

     

    This role provides educational information about our programs, creating relationships that allow consumers of Humanim programs to obtain employment in the community. Utilizing all contact methodologies to achieve maximum reach with the ultimate goal of developing trusting and long-term relationships for referrals and employment placement. The Employment Consultant coordinates with the clinical team to recommend additional services consumers may require. This role assists in the development of employment opportunities for individuals with mental health issue, that can include job coaching, follow along supports, ongoing job supports, self-employment support services, career exploration services, co-worker supports and/or pre-vocational/independent living skills activities based on assigned participants’ goals.

     

     

    ESSENTIAL RESPONSIBILITIES:

    Responsible for selling Humanim’s services by developing business relations and business partners with companies in the community to obtain employment opportunities for consumers in part- or full-time competitive jobs.Conduct job analysis at job sites before placement to ensure appropriate job match and compliance with consumer, employer, agency treatment team and funding source(s) needs.Remain knowledgeable and be able to explain ADA/reasonable accommodations to employers and advocate for consumers.Follow policies and procedures for Job Development services and facilitation communication for consumer throughout interview and application process.Facilitate on the job orientation for consumers and ensure consumer remains actively engage in the placement process and re-engage if disengagement occurs.Provide on/offsite job support and PRP visits to ensure proper training for consumers.Responsible for proper documentation and management of progress reports, individual vocational plans (IVP), job development and support addendums and all other agency required documentation.Develop, maintain and actively seek community partnerships, attend meetings and conferences and participate in other marketing and promotion of Humanim’s services.Actively work and maintain relationships with DORS associates to encourage referrals, support consumers referred and work to meeting current and anticipated needs from DORS counselors.Collaborate with departmental and cross functional teams to coordinate treatments, job leads, and ensure all team members are supporting consumers.Responsible for 4 employment placement a year.Develop at minimum 6 employer leads per week.Responsible to document within 24 hours client contact/meetings . Change this to 48-72 hours. This was discussed with Cindy.Follow Fidelity guidelines of 26 community hours(Time spent outside of our Humanim and Vesta Offices, drive time is counted within these hours).Follow clinicians treatment recommendations, meet with each clients therapist and or Psychiatrist every 6 months face to face or via video telehealth to ensure mental health treatment is being considered with employment goalsMinimum of 3 face-to-face meetings per a month with each client on caseload.Minimum of 2 face-to-face meetings per a month with each client looking for employment.All other duties as assigned.

The employee frequently is required to use hands or finger, handle, or feel objects, tools or controls. The employee is frequently required to stand; walk; sit; reach with hands and arms; and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 15 pounds. Ability to spend extended periods sitting at a desk and utilizing a computer. Specific vision abilities required by this position include close vision, distance vision, peripheral vision, and the ability to adjust focus. The noise level in the work environment is usually moderate.
