Summary of Position: Performs a variety of general and project cleaning tasks to maintain resident areas, offices, corridors and public areas to ensure environmental safety, cleanliness, functionality and ambiance.
Essential Duties and Responsibilities:
Maintains established departmental policies, procedures, and objectives, quality assessment and improvement programs, and safety, environmental and infection control standards.Works from a schedule that outlines and defines frequency and timing or tasks. Leads works with cleaning crew or assigned projects. Performs duties of housekeeper when not assigned project work.Empties trash, biomedical waste and recyclables and transports to central collection site. Transports from site to compactor/recycling dock. Operates compactor. Packages biomedical waste.Strips and refinishes floors, extracts carpets. Dry mop and wet mop floors.Completes work orders in a timely manner.Moves, removes, and relocates furniture, furnishings, equipment, etc. as directed. Set-ups for meetings, activities and special programs.Completes initial cleanings on vacant units successfully.Communicates effectively and tactfully with adult and older adult residents/patients recognizing their age, culture, needs, abilities and physical condition. Attends department & AV meetings and inservice programs.Understands and follows universal precautions as it pertains to their department.Performs all duties in a manner that demonstrates safety, respect, care and concern for our residents/patients/clients and staff. Reports to clinical staff any concerns that might warrant attention on behalf of the resident. Maintain a healthy, clean, safe environment for the population of Ashlar Village.Operates vacuum cleaners and some mechanical floor/carpet machinery.Washes window draperies. Summary of Position: The recreation therapist will be required to evaluate, plan, implement and document according to the CT DPH standards as well as the expectations of the Director. This individual will create therapeutic recreation programs that benefit each resident’s health, functional status and overall quality of life. The individual will be responsible for the successful provision of recreation and leisure in 1:1 sessions, small groups, large groups and house wide facility events. This individual will maintain a diverse calendar of programs to meet the social, emotions, spiritual, cognitive, and physical needs of each individual resident. This individual will need specific experience with programming for residents with dementia.
Essential Duties and Responsibilities:
Maintains adherence to Masonicare Customer Service expectationsDevelops and implements individualized treatment plans which are age and culturally relevant; and address each resident’s social, emotional, physical, psychological and cognitive status for the residents/patients.Plans and implements 1:1 intervention, small group activities, large groups activities, community trips, special events and entertainment; and motivates residents to participate. Transports residents to/from programs. Completes and maintains documentation standards including assessments, participation records, care plans, notes, MDS, etc.Communicates effectively and tactfully with residents and recognizes their age, cultural diversity, needs, abilities and physical condition. Education/Certification: Preferred: Bachelor’s Degree in Therapeutic Recreation, and/or Certification through NCTRC as a Certified Therapeutic Recreation Specialist. Minimum Requirement of Associates Degree with TRD certification is required.
Key Competencies: Strong Dementia care background as relates to Therapeutic Recreation. Strong time management skills required. Ability to develop creative programming and enhance the skill sets offered by the Therapeutic Recreation Department is desired. Must be able to operate a computer and maintain electronic medical records. Interest/experience in technology is a plus. Excellent communication skills, knowledge of appropriate TR interventions, strong documentation and organizational skills are required. Ability to be flexible with schedule on occasion to provide programming is required. Must be a team player!

Clinical Assessment Specialist (DAY & EVENING SHIFT)

Summary of Position: Responsible for the coordination of the Minimum Data Set (MDS) care planning and the financial component associated with MDS.
Essential Duties and Responsibilities:
1. Implements and monitors Reimbursement Assessment Instrument (RAI) system and processes related to continuous flow of services from admission through treatment, reassessment and discharge for all patients/residents of designated care units.
2. Determines all Assessment Reference Dates (ARD) and creates/monitors MDS schedules for patients/residents based on input from other disciplines. Communicates with Interdisciplinary team daily for necessary changes in time frame.
3. Collaborates with the Interdisciplinary team s to coordinate the planning, development and implementation of a comprehensive care plan.
4. Oversees compliance with Medicare Certifications for all Medicare A / PPS patients/residents on the designated units.
5. Regularly prepares and distributes Medicare Notices for patients/residents on the designated units.
Communicates with Finance/Billing office regarding Medicare status and coverage as needed. Participates in monthly triple check process for all medicare claims.Collaborates with the Health Information Department (HIM) to ensure appropriate documentation is submitted to government/private pay agencies in conjunction with the MDS process.Maintains flexibility in work schedule/assignment to meet the needs of the Clinical Reimbursement department to include on call/weekend coverage.Participates in educational programs, inservices, meetings, workshops and utilizes current literature to enhance professional skills and knowledge. Maintains current working knowledge for all changes in the RAI system and PPS.Minimum Qualifications:
Education: Graduate of school of professional nursing.
Experience: Five years clinical experience with knowledge in financial, regulatory and legal aspects of healthcare. Previous experience with MDS, care planning, clinical reimbursement including PDPM and current Medicare reimbursement processes are required.

RAC-CT certification preferred.

The Unit Secretary (US) serves as the pivotal person in the routine operations and smooth running of the care center on a daily basis under the direction and supervision of the Clinical Nurse Manager.
Essential Duties and Responsibilities:
1. The US organizes and coordinates the daily activities of the care center related to physician visits, monthly reviews, annual History & Physical's, consultant visits, patient/resident appointments, lab and x-ray appointments, LOA's, etc, staff assignments and schedules. She/he will maintain the daily nursing staffing sheet and assist in staffing and scheduling activities as directed by the CNM.
2. The US will transcribe physician orders into the Meditech Order Entry module accurately and efficiently and communicate all urgent or emergent orders to the appropriate staff to ensure the safe and effective delivery of patient care.
3. The US will perform all clerical duties necessary to maintain a complete, organized and current medical record and will demonstrate full understanding of responsibilities of all procedures relating to the medical record such as: admissions, discharges, transfers, deaths and purging. The medical record will meet all facility and regulatory standards as demonstrated by compliance with monthly medical record audits.
4. The US serves as a resource to all staff, physicians, patients/residents, families and visitors and communicates information promptly and accurately in a professional and sensitive manner. She/he is able to prioritize work in order to provide prompt and efficient service and demonstrates flexibility in her/his working schedule to best meet departmental needs.
5. The US assists in the containment of costs through the requisitioning of supplies and equipment, maintenance and repair services and preparation of related forms in order to keep accurate inventories of supplies and equipment.
