    What You’ll Do:

    ·         Provide basic nursing care to residents within the scope of the nursing assistant responsibilities and perform basic nursing procedures under the direction of the licensed nurse supervisor

    ·         Assist residents with daily care, including bathing, grooming, hygiene, and placement of adaptive equipment

    ·         Keep residents dry (change gown, clothing, and linens when wet or soiled

    ·         Handle linens and make beds according to infection control guidelines

    ·         Assist residents with bowel and bladder functions

    ·         Assist residents in preparation for and transporting them to appointments, activities, and social programs

    ·         Assist with lifting, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts, meeting specific resident safety needs.

    ·         Assist residents in ambulation with or without self-help devices as instructed

    ·         Performs restorative procedures as instructed

    ·         Accurately measure and record heights and weights, temperatures, pulses, respirations, and blood pressures as instructed

    ·         Empty urinary drainage bags and record amount in the record as indicated; report any irregularities to nurse

    ·         Ensure call lights are within reach of residents and answer call lights promptly

    ·         Ensure that residents who are unable to call for help are checked frequently

    ·         Report all changes in residents’ condition to supervisor as soon as practical

    ·         Maintain cleanliness of resident units, central bathing area, treatment and supply rooms, workrooms, and linen closets; clean bedpans, urinals, and commodes; wash basins, sanitize bathtub after each resident’s use, clean floors

    ·         Perform other related duties as assigned.

     Requirements:

    Certified Nursing Assistant (CNA) in good standing in relevant state

    Previous experience in long-term care or senior living (preferred)

    Compassionate, reliable, and patient-centered

    Strong team player with excellent communication skills

    Ability to lift up to 25 pounds - physical work is a primary part of the job

    Able to stand, bend and walk for extended time periods

    Qualifications:

    What You’ll Be Doing:

    ·         Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

    ·         Plan nursing interventions for each resident based on individual needs and monitor the implementation

    ·         Evaluate the effectiveness of care interventions, identify problems and trends and develop alternative interventions.

    ·         Conduct family and resident education

    ·         Administer medications and treatments timely and according to center policy

    ·         Transcribe physician’s orders and chart progress notes in an informative, factual manner that reflects the care administered as well as the resident’s response

    ·         Identify and report changes in condition to supervisor, physician and family

    ·         Accurately identify skin changes and follow skin management protocols

    ·         Order and report diagnostic tests timely

    ·         Follow Pharmacy policy and procedures for ordering and delivering medications.

    ·         Coordinate resident’s rehabilitation program with therapists to ensure rehabilitation needs are met

    ·         Monitors that CNA documentation is completed daily

    ·         Ensures CNA assignments are completed on time

    ·         Perform additional nursing care responsibilities as required and based on State Nurse Practice Act.

    Requirements:

    ·         Active state RN license

    ·         CPR/BLS Certification

    ·         Experience in Long Term Care or Skilled Nursing preferred

    ·         Excellent technical, assessment and documentation skills

    Job Summary

    Administers prescribed medications and treatments as defined by state regulations 1 to residents and maintains related medical records under supervision of Nurse Supervisor.

    Ensures that all care is provided in privacy

    Ensures all protected health information is kept confidential

    Reports all complaints made by residents to supervisor

    Reports all allegations of resident abuse and/or misappropriation of resident property Safety and Sanitation

    Follows established safety policies and procedures

    Observes safety needs of residents as indicated in care plan

    Wears and/or uses safety equipment and supplies when indicated and properly trained to use

    Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situations

    Position Responsibilities

    Administers prescribed medications and treatments as defined by state regulations in accordance with approved techniques and notes time and amount on resident’s chart.

    Verifies identity of resident receiving medication and records name of drug, dosage, and time of administration on specified forms and records.

    Presents medication to residents and observes ingestion or other application, or administers medication, using specified procedures.

    Observes resident, records significant conditions and reactions, and notifies Nurse Supervisor or Physician of resident’s condition and reaction to drugs, treatments, and significant incidents.

    Documents reasons prescribed drugs are not administered and reports to nursing supervisor

    Records and restocks medication inventories.

    Prepares equipment and aids physicians during treatments and examination of residents.

    Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and determines progress of residents.

    Checks residents in section regularly; Answers call light promptly; Follows through with resident problems or needs by delivering care or reporting to supervisor depending upon level of ability and nature of resident need. 

    Performs any miscellaneous work assignments as may be required. 

    Certified Medication Aide Education 

    Certified as required by state law.

    Experience

    Previous experience in long-term care preferred. Physical Demands

    Lifting 25 pounds maximum with frequent lifting and carrying of objects weighing 25 pounds; Reaching, handling, feeling, seeing.

     Working Conditions

    Inside with well-lighted, well-ventilated areas with some exposure to disagreeable conditions.

    ·         Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration. 

    ·         Help coach staff as necessary.

    Requirements:

    Current LPN license in KY

    CPR/BLS certification (or willing to obtain)

    Previous LTC or SNF experience preferred, but new grads welcome!

    Positive attitude, reliability, and strong clinical judgment

    Salary/ Wage Range
