Position Overview:
The Activities Assistant organizes and participates in person-specific activity programming, including one-to-one interventions, small group facilitation, and large group presentations.
Key Responsibilities:
Utilize the environment to provide both comfort and stimulation to the resident population.
Foster a sense of community within the community environment.
Lead and participate in individual and group activities.
Utilize a variety of models to achieve individualized programming such as pet therapy, music therapy, art therapy, etc.
Assist with decorating the facility and preparing for special events and seasonal parties.
Develop and post monthly calendar as a planning resource for residents and families.
Participate in recreational programs for residents confined to their rooms.
Promote volunteer efforts in the community.
Maintain records of activities and attendance.
Qualifications:
High school diploma or equivalent preferred
Activity Assistant Certification preferred
CNA certification preferred

Position Overview:
The Weekend Supervisor is responsible for directing the operation of a nursing unit in a long-term community.
Key Responsibilities:
Perform general management functions such as hiring, disciplining, and evaluating care team members.
Plan and facilitate meetings and committees to address resident care issues/concerns for the unit.
Manage area of responsibility with the goal of achieving and maintaining the highest quality of care possible.
Participate in developing, implementing, and evaluating programs that promote the recruitment, retention, development, and continuing education of nursing staff members.
Contact physicians as needed/necessary and ensure physician interventions are timely and appropriate.
Review staffing patterns and census of nursing units and reassign personnel when necessary to ensure staffing meets resident needs and budgetary controls.
Monitor the clinical operations of the unit and resident’s conditions and ensure that appropriate and quality care is administered.
Obtain medications, supplies and medical records needed to provide safe, efficient, and therapeutic are to residents on a continuing basis.
Assist in the orientation of new personnel, monitoring their skills and guides and observe care team members that may need assistance with procedures.
Qualifications:
Graduate of accredited school of nursing.
Licensed as an LPN or RN in the state employed; license must be active, valid, and in good standing.
Valid CPR teaching certificate preferred, but not required.
Must hold an active CPR license or the ability to obtain within the first 30-days of employment. CPR license must be kept active and current throughout employment.
Three years of nursing experience, of which one year in a long-term care environment.

Position Overview:
The QMA is a member of the community nursing team whose responsibility is set up, administer, and record certain prescribed medications for residents in the community under supervision of a licensed nurse and as allowed by state and federal regulatory agencies.
Key Responsibilities:
• Prepare and administer regularly prescribed oral, eye, nasal, and ear medications to residents. Administrations must be administered safely and documented.
• Prepare and administer topical medications to intact skin.
• Administer PRN drugs, as allowed, when authorized by a license nurse.
• Check physician orders against medication forms.
• Maintain cleanliness and proper storage of all drugs while maintaining a clean working environment.
• Keep medication cart locked while unattended.
• Care for and position all resident tubing and bags as instructed.
• Make rounds and report needs and conditions to the charge nurse as applicable.
• Administer vital signs and document properly.
• Keep medical records current by charting pertinent resident conditions timely and routine charting as scheduled, with co-signature by a licensed nurse.
• Order medications from pharmacy per physician order.
• Use appropriate drug reference if ever unsure or unclear as to drug being administered.
• Count all drugs on Individual Control Drug Records with on-coming charge CTM at shift change.
• Answer call lights, bed/chair sensors, and security care bracelet (if applicable) alarms promptly and courteously.
Qualifications:
• Certified as a Qualified Medication Aide in the state employed; certificate must be active, valid, and in good standing.

• Re-teach resident to perform ADL skills.
• Provide restorative care to prevent pressure sores, contractures, and complications.
• Participate in Continuous Quality Improvement and ensures follow-up occurs.
• Converse with resident and family; assist them on adjustment to the community.
• Ensure adequate hydration and nutrition of resident; report deviations from norm to the Charge Nurse promptly.
• Perform blood pressure, pulse, respirations, intake/output and weights accurately.

Position Overview:
The ADNS assists in managing the department of nursing and
assumes responsibility delegated by the Director of Nursing Services (DNS).

Assist in interviewing applicants and recommend hiring of staff and scheduling.
Participate in orientation and in-service education training.
Participate in performance reviews and personnel files; enforce the personnel policies of the community, including disciplining and terminating care team members.
Assist DNS in ensuring compliance with regulations for documentation from physicians and nursing personnel and ensuring that all regional policies/procedures are implemented.
Assist with care planning process.
Make rounds daily to direct the work of care team members and to observe for hazards, work duty problems, and ensure services are delivered to clients consistent with needs.
Assist with Continuous Quality Improvement and corrective follow-up actions.
Follow up daily on each unit to ensure changes in status are reported and recorded and that services are provided.
Under the direction of the DNS, complete chart audits and report results and progress, and makes recommendations for improvement to the DNS.
Ensure proper infection control techniques are utilized.
Maintain effective communication with families, residents and community staff.
Qualifications:
Current RN license by State Board of Nursing in state employed; license must be active, valid, and in good standing.
Experience in the long-term care or working with the geriatric population preferred.
Must have at least three years nursing experience and one year supervisory experience.
Must hold an active CPR or ability to obtain within the first 30-days of hire; CPR must remain active and current throughout employment

The PTA collaborates closely with the interdisciplinary care team to ensure consistent, resident-centered therapy. Key Responsibilities:
Clinical Support and Treatment Implementation
Deliver physical therapy interventions as outlined in the treatment plan developed by the supervising Physical Therapist.
Assist residents with therapeutic exercises, gait training, and functional mobility activities to improve strength, balance, and independence.
Monitor resident responses during treatment sessions and communicate progress, concerns, or changes to the Physical Therapist.
Support resident and family education under the direction of the Physical Therapist to
promote safe and effective carryover of therapy strategies.
Maintain a safe, supportive, and engaging therapeutic environment that encourages resident participation and comfort.
Participate in interdisciplinary team meetings and contribute to care planning discussions to ensure coordinated, resident-centered care.
Documentation and Compliance
Accurately document treatment sessions, resident performance, and observations in
accordance with facility policies and regulatory standards.
Adhere to HIPAA, OSHA, and infection control protocols to ensure compliance and resident safety.
Collaboration and Operational Support
Maintain open communication with the supervising Physical Therapist, interdisciplinary care team, and resident care staff to support continuity of care.
Assist in maintaining therapy equipment and supplies, reporting any issues promptly to
ensure a safe and efficient therapy environment
Qualifications:
Associate’s degree from an accredited Physical Therapist Assistant program
Current state licensure as a Physical Therapist Assistant
1–2 years of experience in a skilled nursing facility or geriatric care setting preferred
Familiarity with PDPM and SNF therapy documentation standards is a plus
