    Job Description Job Description Position: Program: Department: Reports to: Salary Category: FLSA Status: Associate Manager, Clinical Nursing (LVN) Nursing Health Services Nurse Manager $105,119 - $110,651 100% Full-Time Exempt DEPARTMENT SUMMARY: As the nation s largest and most experienced provider of LGBT medical and mental health services, we help LGBTQ people lead healthier, happier lives, by keeping their minds and body well. All services are free or at a low cost. We re also one of the few Federally Qualified Health Centers in the nation with providers who specialize in primary care for LGBTQ people and with a research team working to advance the care and treatment of people in our community. JOB SUMMARY: The Associate Manager, Clinical Nursing (LVN) provides leadership and oversight for Medical Assistant staff, ensuring high-quality patient care and compliance with regulatory standards. Key responsibilities include supervising staff, managing clinical operations, maintaining adequate staffing and supplies, and supporting staff development. The position oversees quality improvement efforts, participates in audits and policy development, and collaborates with other Health Services leaders to maintain a positive clinical environment. ESSENTIAL FUNCTIONS Leadership Oversight Provide direction and guidance to Medical Assistants Collaborate with Health Services leadership to align nursing programs with patient care and regulatory standards Ensure adequate staffing levels and clinical coverage to meet care and compliance needs Oversee supply management related to patient care, including ordering and inventory of medications, equipment, and supplies Maintain a positive clinical environment in partnership with supervisors and other managers Staff Management Development Lead human resources functions for direct reports, including performance feedback and evaluations Develop and implement staff training to meet continuing education, contract, and team development goals Ensure clinical staff practice within defined scopes of practice Conduct or participate in regular team meetings to support communication and staff engagement Quality Compliance Provide Quality Management oversight across Health Services divisions and applicable Center areas Participate in the Quality Risk Management Committee and ensure timely monitoring and reporting Contribute to or lead special project teams to address quality or operational concerns Participate in audits and support preparation of corrective action plans Ensure policies and procedures comply with regulatory, licensure, and contractual standards Policy Procedure Management Participate in Nursing Unit Level Policy and Procedure Committee to maintain current documentation Ensure staff understand and adhere to updated policies and procedures Maintain documentation to ensure compliance with Center policies and external requirements Organizational Engagement Attend Center and Health Services meetings as required Perform other duties as assigned QUALIFICATIONS Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people A passion for the Center s work and its mission to make the world a better place for LGBTQ people Current California LVN license required Current AHA CPR certification Minimum of three years of experience in direct patient care, preferably in an ambulatory care setting Experience in outpatient nursing management preferred Working knowledge of infectious diseases, e.g., HIV/AIDS, STIs, and state/ federal reporting requirements Basic working knowledge of care coordination and navigation throughout the managed care process i.e., prior authorization, specialty care coordination, patient assistance program Strong communication, organizational, and interpersonal skills with a high level of attention to detail Proficiency with Microsoft Office, Electronic Health Records, and other software systems Ability to be self-motivated and dedicated to initiating and completing complex tasks Ability to handle multiple tasks, be detailed oriented, and maintain strict confidentiality of medical information Able to work well independently with minimum supervision Bilingual skills, a plus (English/Spanish fluency preferred) Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment The Los Angeles LGBT Center is an Equal Opportunity Employer and is committed to fostering diversity within its staff. JOB SUMMARY: Under supervision of the Program Manager, Housing Stabilization, the Housing Stabilizer I for the Intensive Case Management Services (ICMS) program is responsible for meeting contract requirements while assisting clients with supportive services with a client-centric approach. The ideal candidate possesses excellent skills that are sensitive to the challenges of people experiencing homelessness and a range of medical and behavioral health issues they face as they move into and maintain permanent supportive housing. Services will be provided to support quality care in compliance with local, state, and federal regulations and accreditation standards. ESSENTIAL FUNCTIONS: Act as primary contact for high-acuity clients throughout their full length of participation in the program; Complete a comprehensive intake and Bio-Psychosocial assessment of new participants to help determine needs, eligibility, interests, and goals; Assess and implement necessary interventions to reduce barriers to housing; Link clients to Medical Home and supportive services such as mental health, health, substance abuse, employment, and education both at the Los Angeles LGBT Center and with external service providers; Provide linkage to resources such as DPSS, Social Security, Medi-Cal, and act as liaison for clients with those resources; Match clients with appropriate housing opportunities through field-based housing search, including but not limited to short term crisis housing, federally assisted housing, substance abuse programs, and low-income housing; Provide assistance with the completion of all applications for housing placements; Provide linkage to resources assisting clients in completing applications for financial assistance programs; Work with clients and their current and previous supports to develop quarterly individual care plans to help them meet initial and intermediate goals in the areas of education and employment, housing stability, financial stability, positive healthy outcomes, and permanent connections; Complete quarterly 5X5 assesments including transitions of care documentation following medical appointments and in-patient hospital care; Collect eligibility documents as required; Conduct ICMS required home visits to ensure clients are successfully maintaining their permanent housing; Work effectively and creatively as a member of an interdisciplinary team to support the needs of high-acuity clients; Attend and present clients in weekly case conference meetings to coordinate services with internal and outside service providers; Maintain statistical client data, including entering client data into HMIS Clarity and CHAMP databases as determined by contract requirements; Maintain complete and accurate files documenting all care and services provided in the client s Electronic Record; Connect with participants and landlords; Use standard office machinery including personal computers and printers in performance of duties; Assist in data auditing tasks, prepare reports as required, and submit in a timely manner; Participate in staff meetings, agency meetings, planning meetings, and other meetings as needed; Comply with requirements of OSHA, HIPAA, and other applicable regulations; Other duties as assigned; JOB QUALIFICATIONS AND EXPERIENCE: A passion for the Center s work and its mission to make the world a better place for LGBTQ people; Knowledge of or experience working with the LGBTQ community and familiarity with issues of relevance to LGBTQ people; Minimum 3 years experience providing direct mental health or intensive case management services with people experiencing homelessness; Mental health-related field, preferred or Bachelor s degree in social work; Knowledge of homeless populations and related systems, programs, and Federal policies and guidelines under the Coordinated Entry System (CES); Access to reliable transportation and the ability to be insured; AHA CPR certification; Training in Motivational Interviewing and Trauma Informed care, a plus; Ability to have a whatever it takes attitude when working with challenging individuals; Excellent verbal and written communication skills, including strong organizational, detail, and interpersonal skills; Excellent Computer skills and knowledge of Microsoft Office, word processing, Electronic Health Records, and other software systems; Ability to be self-motivated and dedicated to initiating and completing complex tasks; Ability to handle multiple tasks, be detailed oriented, and maintain strict confidentiality of medical information; Able to work well independently with minimum supervision; Bilingual skills, a plus (English/Spanish fluency preferred); Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
