    The responsibilities of the Certified Nurse Assistant (C.N.A.)/Med-Tech include, but are not limited to:

    Responsibilities:

    Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in this facility and that may be ordered for residents use by the attending physician.Follow the labeling policies and practices of the facility.Ensure that all medications administered are properly labeled.Assist with all medications charting as directed by the Director of Health Services.Accurately measure, record, and report the vital signs of residents.Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal.Assist residents with all applicable ADL's, grooming, light housekeeping including laundry and toileting. Showers and baths as assigned. At times will assist with the application of elastic stockings, slings or support garments.Assist residents in preparing for medical test activities and social programs. Assists in transporting residents to/from appointments, activity and social programs, etc., as necessary.Observe and report the following: presence of pressure areas and skin breakdowns, all injuries including those of an unknown source, including skin tears. Inform the Director of Health Services or Wellness Coordinator of any changes in the resident's condition.
    Experience and Skills:

    Must have current CNA certification by the State of California.Medication Technician CertificatePrevious experience in Long Term Care preferred.Skilled Nursing Facility experience and with residents suffering from Alzheimer's or dementia, a plus.Verification of CPR (BLS) and first aid training.Background clearance, TB screening and successful completion of a health examination by a Los Angeles Jewish Health Nurse Practitioner or M.D.Flexible with working hours. Able to work various shifts.

    Opportunities are available across multiple employment types (Full-Time, Part-Time, Per-Diem) and shifts (6:45 AM - 3:15 PM, 2:45 PM - 11:15 PM, 10:45 PM - 7:15 AM). Shift preferences will be discussed and confirmed during the interview process.

    The responsibilities of the Registered Nurse Supervisor include, but are not limited to:

    Responsibilities:

    Assume administrative authority, responsibility, and accountability for performing the duties of the RN Supervisor.Review the daily assignment schedule to ensure that each staff member and position listed is on duty, assigned appropriate work tasks, and scheduled hours. Additionally, review the supervisory staff assignments for each shift. Address any staff shortages by attempting to fill positions with off-duty personnel first, followed by on-duty staff.Conduct shift management rounds to observe staff performance and ensure that residents' nursing care, therapy, safety, and shelter needs are met according to facility policies and regulatory standards.Ensure medications, treatments, therapy services, and diets are administered as ordered, in a professional manner, with accurate and timely documentation.Receive reports from the RN Supervisor or Charge Nurses regarding any changes in resident conditions, new admissions, discharges, physician call-outs, and required diagnostics (e.g., x-rays).Handle all incoming calls from physicians regarding resident care needs.Monitor, coach, and discipline nursing staff as necessary for failures in adequate, accurate, and timely documentation of care or services. Report any performance issues to the Director of Nursing (DON).Perform administrative duties assigned by the DON, such as completing medical forms, Medi-Cal Treatment Authorization Requests (TARs), PASARR, gathering data for reports and studies, and charting as required for appropriate documentation.Foster a positive rapport and cooperative working relationships with all department members, responding to co-workers with concern and cooperation.Communicate effectively with the medical staff, nursing service, and other department supervisors.Maintain liaison with other department supervisors to ensure adequate planning for social services and activities.
    Experience and Skills:

    Must possess an active, current license to practice as a Registered Nurse (RN) in the State of California.Preferred experience: At least one (1) year as a supervisor in a long-term care facility or two (2) years as a Charge Nurse in a skilled nursing facility or hospital.Must be able to read, write, and speak clearly in English.Must be able to follow both written and oral instructions effectively.

    The responsibilities of the RN include, but are not limited to:

    Responsibilities:

    The responsibilities of the Social Worker include, but are not limited to:

    Responsibilities:

    Assessment/Reassessment: Assists in the completion of an initial psycho-social assessment within 72 hours of a patient admission utilizing input from the patient and patient's authorized representative. Documents individual and group interventions daily in the attendance record. Completes progress notes including psycho-social evaluation weekly or more often, as needed.Documentation: Writes progress note on admission, weekly, upon discharge and as needed with new concerns, significant change of behaviors, response to interventions and when new interventions are needed as well as discharge plans. Completes daily documentation of individual and group interventions as directed.Discharge Planning/Utilization Review: Initiates discharge planning on admission and coordinates the discharge process with the patient, patient's authorized representatives and other resources.Treatment Planning: Assures that treatment plans include appropriate social service interventions within 72 hours of patient's admission and are updated with each change of condition, change in behaviors and at least weekly. Assures that the patient / patient's authorized representatives are involved in development of the treatment plan and sign the treatment plan to acknowledge agreement with the treatment plan.Provides discharge planning and documents medical necessity.Assists patient and patient's authorized representative select an appropriate after care placement when needed.Assist with therapeutic activities, specifically leading 1-2 patient groups per week.Manages unit case load in an efficient manner and coordinates responsibilities with L.C.S.W.Participates in development, review, revision and implementation of policies and procedures for the Acute Psychiatric Unit as needed.Conducts regular unit rounds with LCSW and other interdisciplinary team members as appropriate to identify care and environmental issues and implement corrections as appropriate.Quality Assurance and Performance Improvement: Actively participates in the Acute Psychiatric Unit Quality Assurance and Performance Improvement (QAPI) program. Reviews indicators for social service on a pre-assigned basis, makes any necessary corrections and makes recommendations for improvement to the QA and PI Committee at least quarterly.Assists with coordinating transfers to other acute psychiatric facilities providing involuntary care when needed.Assures security of environment to prevent exit of patients attempting to elope. Implements the facility procedure for elopement if any patient leaves the facility without permission.
    Experience and Skills:

    Master's Degree in Social Work, with acute inpatient geriatric psychiatry experience preferred.A minimum of 1 year of hospital experience is strongly preferred.Strong preference for prior inpatient psychiatric hospital experienceMust be able to read, write, speak, and understand the English language.Basic Life Support (BLS) certification required within (3) months of hire.CPI (Crisis Prevention Insitute) certification required upon (3) months of hire.

    The responsibilities of the Certified Nurse Assistant (C.N.A.) include, but are not limited to:

    Responsibilities:

    Assist residents with ambulatory training and mobility support.Participate in the feeding program, implementing structured techniques.Engage in the Activities of Daily Living (ADL) program.Demonstrate skill and tact in resident interaction and communication.Review care plans to ensure appropriate restorative nursing care is provided.Observe and report any pressure areas, skin breakdowns, injuries (including those of unknown origin), and skin tears to prevent decubitus ulcers. Inform the Nurse Supervisor/Charge Nurse of any changes in the resident's condition.Monitor and report any changes in room temperature, ventilation, lighting, and ensure the room is prepared for receiving the resident (e.g., bed made, name tags displayed, admission kit available).Record all required entries in a timely manner on flow sheets, such as meal percentages, bowel movements, etc., using authorized abbreviations.Inventory and label resident's personal possessions as instructed.Assist residents with daily dental and oral hygiene care, including brushing teeth/dentures and special mouth care.Provide assistance with bath functions (bed bath, tub or shower bath) as directed.Assist with hair care (combing, brushing, shampooing) and nail care (clipping, trimming, and cleaning fingernails/toenails). Shave male residents as needed.Weigh and measure residents, and record vital signs (temperature, pulse, respiration).Participate in fire/safety programs, First Aid/CPR training, and attend a minimum of 75% of monthly meetings and in-services to maintain Certified Nursing Assistant certification (48 hours every two years).Follow established safety protocols in all duties, using necessary safety equipment (e.g., gait belt, mechanical lifts) when lifting or moving residents. Report hazardous conditions and equipment issues to the Nurse Supervisor/Charge Nurse immediately. Demonstrate proper body mechanics/ergonomics in all work practices.
    Experience and Skills:

    Current CNA certification issued by the State of California is required.Experience in a Skilled Nursing Facility, particularly with residents suffering from Alzheimer's or dementia, is a plus.Verification of CPR (BLS) and First Aid certification is required.Background clearance, TB screening, and successful completion of a health examination by a Los Angeles Jewish Health Nurse Practitioner or M.D. are required.
