    Job DescriptionJob DescriptionSUMMARY: The medical assistant will work within their scope as defined by state and federal guidelines. The MA oversees the operations of rooming and checking in patients and the general flow of back office. The MA communicates effectively with all members of the team including but not limited to providers, fellow MA’s, nurses, provider, patient navigators and patient registration specialists.   • Job DescriptionJob DescriptionSUMMARY:Renders comprehensive health care within the limitations of professional licensing to patients of the Los Angeles Christian Health Centers. Assesses needs and prescribes treatment. Actively supports the mission of LACHC: to follow Christ by loving and serving our neighbors through comprehensive, quality health care. ESSENTIAL DUTIES, RESPONSIBILITIES, AND PRIVILEGES include:Under supervision of staff physician, perform evaluation and treatment of patients in the outpatient setting at Los Angeles Christian Health Centers sites. Assess medical needs and compile care plan, addressing acute needs, chronic conditions, and preventive health maintenance. Assessing needs may include assessment of spiritual, emotional, mental and social needs. Furnish/transmit appropriate medications and treatments. Perform in-office procedures such as incision and drainage, arthrocentesis, nail removal, skin biopsy, and cryotherapy.Productivity standards must be met 17 patients per day.Work within clinical privileges that are clearly delineated either through a practice agreement (Physician Assistant) or Scope of Practice agreement (Nurse Practitioner).Keep accurate, concise, and thorough documentation of all medical history, exam findings, assessment,treatment/interventions, and education.Arrive to work punctually at the start time. Punctual arrival of licensed providers is essential for the processing and preparation of patients.Complete documentation and lock progress notes in a timely manner. The clinic requirement is to lock all progress notes within 7 calendar days.Review and electronically sign off on all laboratory results, diagnostic results, and outside consult notes in a timely manner, ensuring patient is contacted for follow up if clinically appropriate.Initiate and follow through on specialist referrals and e-consults (electronic consults), communicating with specialist providers as necessary, in the provision of whole- person care.In a “float” capacity, serve patients empaneled to other providers as Primary Care Providers. Work with the care team and other staff at LACHC to serve as a patient-centered medical home for our patients.During and after covering shifts for primary care providers, communicate with the primary care providers about follow up needed for particular patients, to ensure ongoing care by the primary care provider and their clinical care team.If maintaining their own patient panel at a given site, serve as the primary care provider for a panel of patients. As a provider, work with a care team and other staff at LACHC to serve as a patient- centered medical home for our patients.Conjointly supervise medical assistants along with the Medical Assistant supervisor.Participate in LACHC meetings including morning “team huddles”, Provider Meetings, and All-Staff Meetings. Actively participate in spiritual care for patients and staff which may include prayer, and referral to community resources.Provide compassionate care for patients who, depending on the clinic site, are predominately homeless, may suffer from mental illness, and often lack resources to care well for themselves.Actively participate in Continuous Quality Improvement activities at LACHC, including teaching other providers on selected topics, implementing team- based care, participating in peer reviews, evaluating clinical performance measures, and implementing improvements using a Plan-Do-Study-Act method of stepwise changes.Meet productivity standards for outpatient primary care at LACHC, including an expectation of 17 patients seen per usual 8 hour day; actual productivity may vary per site, and depending on type of visit (in person vs. virtual (Telehealth, telephonic or audio-video).Periodically serve “on call” for the clinic, answering urgent phone calls from patients or other agencies and responding to critical lab results in the off-hours for one week approximately every 10-15 weeks.Network with other health care agencies and county facilities.Assist in provision of health screenings (including TB, diabetes, hypertension) at outreach events.Maintain active enrollment in Medicare, Medicaid, Managed Care, and other payor programs as needed for clinical operations, completing credentialing paperwork in a timely manner to remain actively enrolled in these programs.Other duties may be assigned. QUALIFICATIONS:To perform this job successfully, an individual must be able to accomplish each essential duty satisfactorily. The MA will work in the street medicine setting to provide quality and compassionate care to people experiencing homelessness. The MA communicates effectively with all members of the team including but not limited to providers, fellow MA’s, nurses, provider, patient navigators and patient registration specialists. The Housing Coordinator is responsible for working directly with patients to complete comprehensive housing assessments, develop collaborative housing goal plans, to secure permanent housing, and provide intensive support for up to one year after move-in.ESSENTIAL DUTIES and RESPONSIBILITIES include:· Participate in high-touch, in-person, outreach strategies to continually engage with patients referred for Housing Coordination and/or Tenancy Support Services.· Working knowledge of Housing First and tenant rights under Fair Housing.· Clear understanding of nonprofit and community resources for individuals experiencing homelessness in Service Planning Area (SPA) 4.· Understanding of Trauma-Informed Care, Motivational Interviewing, and Harm Reduction strategies, including certification is Narcan administration.· Synthesize complex information from assessments, trainings, and research to implement up-to-date, evidence-based interventions for impactful housing navigation and tenancy support.· Demonstrable expertise in condition(s), and strategies to assist the Medi-Cal Enhanced Care Management populations of focus: Individuals and Families Experiencing Homelessness; High Utilizers; Adults with Serious Mental Illness and/or Substance Use Disorder; Incarcerated and Transitioning to the Community; Individuals at Risk for Institutionalization and Eligible for Long Term Care.· Maintain updated progress notes, assessments, and data in the Electronic Health Record.· Complete necessary and accurate Medi-Cal billing in the Electronic Health Record for services provided.· Apply critical thinking skills and sound decision-making capabilities, often under pressure and in complex situations· Ability to remain flexible, responsive, accessible, and adaptable.Housing Navigation· Collaborate with patients facing homelessness to discuss the housing processes, options, and eligibility, tailored to the patient’s preferences and needs.· Enter all appropriate data into the Homeless Management Information System (HMIS), following the criteria set out by HUD for data elements and the workflows set by the Los Angeles Homeless Services Authority (LAHSA).· Assist patients to navigate the Coordinated Entry System.· Complete and record VI-SPDAT assessments.· In addition to VI-SPDAT assessments, conduct an annual Risk Assessment and Housing Goal Plan to guide service needs and strategic interventions, including contributing needs to secure and maintain housing.· Understanding of application, approval, and terms of housing vouchers available to individuals experiencing chronic homelessness; assist eligible patients in applying to vouchers for rental assistance.· Work collaboratively with patients’ Lead Care Managers to provide updates, progress, and barrier(s) to stable, permanent housing.· Provide patient assistance with obtaining copies of birth certificates, identification, and other documentation, as needed, for successful housing entry.· Provide community-based accompaniment services to clients as needed.· Explore alternative housing settings and resources, such as low-income housing, independent application processes for supportive housing opportunities, shared housing, interim/bridge settings, and shelter availability.· Identify and work with community resources, coalitions, projects, and partnerships to ensure seamless access to and delivery of housing support services.· Support patients with the housing application process for housing voucher(s) and/or Permanent Supportive Housing requirements.Tenancy Support· Assist patients in increasing income; educate patients on establishing and maintaining a personal budget for housing maintenance.· Conduct in-home visits to ensure patient safety and wellness.· Provide assistance with transition to housing, including helping to orient patient to apartment expectations, upkeep, and navigation of community resources.· Provide support and intervention with property managers to ensure collaboration, avoid eviction, crisis intervention, patient wellness, and advocacy in the success of tenancy.· Educate patients and landlords, as needed, on Fair Housing Regulations and accommodation needs, including unit modification(s).· Remain accessible to both patients and landlords in order to address concerns, engage in problem solving, and avoid possible eviction.· Assist with annual re-certification needs.· Ensure accurate documentation of services in the Electronic Health Record.Other duties as assigned.QUALIFICATIONSTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
