    Registered Nurse Case Manager - Specialized Home Care Summary: We are looking for compassionate Registered Nurses (RNs) to work one-on-one with our patients in their home environment to establish and oversee all aspects of case-managing patient's care. Case management involves creating a Plan of Care for patients and following physician orders that lead to outcomes of increased patient independence and satisfaction. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care. Responsibilities: Assumes responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care. Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources. Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes. Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals. Participate in on-call rotation as reasonably assigned. Qualifications: Minimum of 2 years of nursing experience with a preference for 2 years in an acute or a post-acute care setting. Active State Registered Nurse licensure is required. Proof of current CPR certification and valid Driver's license. Primary Responsibilities: Determine physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition. Provide and complete all proper documentation. Administer physical therapy treatments as ordered by a physician. Direct and oversee treatment given by physical therapy assistants. Evaluate the effects of physical therapy treatments and the fit of prosthetic and orthotic devices by observing, noting, and evaluating patients' progress, and recommending adjustments and modifications. Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan post-discharge (e.g., home exercise programs; recommending and / or providing equipment; recommending outpatient follow-up programs). Participate in weekly case conferences and effectively communicate about patient care across disciplines involved.
