Job Description:
The Occupational Therapist is responsible for providing skilled rehabilitation services to patients in accordance with the Plan of Treatment. The OT evaluates, plans and administers occupational therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative occupational therapy as prescribed by a physician.
Assess patient's daily living skills and develop therapeutic retraining programs with measurable objectives.
Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient’s level of function.
Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
Review the retraining programs in a manner consistent with the behavioral goals of each patient and within the guidelines of the rehabilitation program as a whole.
Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient and family members.
Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's needs.
Qualifications:
Master's Degree in Occupational Therapy
Current OT licensure, with no sanctions, restrictions, or provisions, in the state of Georgia.
One year of previous OT experience in home health setting (preferred)

About 1st Care Management
1st Care Management provides a patient-centered alternative to traditional office-based care by delivering primary care and care management services directly to patients in their homes. Through in-home visits, Transitional Care Management, and Chronic Care Management programs, we help reduce unnecessary hospitalizations and improve outcomes through continuous, coordinated care.
Role: Remote Medical Doctor (Georgia Licensed)
This is a fully remote, non–face-to-face clinical oversight role. Patient care is primarily delivered by Nurse Practitioners and Care Coordinators in the field and through Telehealth. Physicians provide medical supervision, clinical review, and authorization support within a collaborative care model.
Responsibilities include reviewing patient cases, signing and authorizing orders and treatment plans developed by NPs, prescribing medications as appropriate, and ensuring clinical accuracy and compliance. The physician will also support care coordination, provide clinical guidance to the team, and participate in quality improvement initiatives.
This role is ideal for a physician who is comfortable working in a supportive, oversight-focused capacity within a value-based, home-based care model.

Responsibilities: Manage the transitional and chronic care needs of homebound patients including:
• Assessment and evaluation of current condition• Obtain past medical history
• Review symptoms
• Conduct general physical examinations
• Review and reconcile medications
• Screen for depression
• Screen for alcohol and drug abuse
• Identify diagnoses to support care management and treatment planning
• Recommend appropriate measures, including medication prescriptions and skilled care needs
• Efficiently communicate assessment findings to the office/supervising physician
• Educate patients on disease processes, prevention, medications, and compliance
Qualifications:
• Current Nurse Practitioner license in the state of Georgia with no sanctions or restrictions• Minimum of 1 year of clinical experience
• Clinical background in Adult, Family, or Geriatric specialties preferred
• Experience using Electronic Health Records (EHR)
