The ideal candidate will be dedicated to patient-centered care, able to work independently, and excel at clinical assessments and care coordination.  
    Responsibilities Assess patient needs and develop personalized care plans (e.g., wound care, medication management).Provide skilled nursing care consistent with physician orders and agency policies. Administer medications, treatments, and therapies as prescribed. Monitor and evaluate patient progress and make necessary adjustments to care plans. Educate patients and their families on health conditions, medication, and self-care techniques. Maintain detailed and accurate patient records and documentation. Collaborate with physicians, interdisciplinary care teams, and other health professionals. Qualifications
     Current and unencumbered Registered Nurse (RN) license in the state of practice. CPR/BLS certification (or ability to obtain upon hire).Experience in home health care preferred (minimum 1–2 years often preferred). Strong communication and organizational skills. Ability to work independently, manage a daily schedule, and travel between patient homes.  Read Less
