Position Description:
This position is responsible for supporting the mission, vision and values of Lifebanc by developing and leading the team of hospital services coordinators. This position is responsible for developing and maintaining relationships with key hospital leadership staff in the donor service area to implement hospital programs and policies that promote organ and tissue donation. The position is responsible for maintaining a positive donation culture by achieving the donation goals of the organization. The position will collaborate across all hospital systems and will provide support to all Lifebanc team members.

• Collaborates with the hospital services coordinators to develop and implement plans to optimize hospital performance, including, but not limited to, an annual/commitment plan, to maintain positive change in practices related to the entire donation process.
• Provides project management across multiple departments and communicates organizational directives and strategic plans.
• Oversees the agenda for communication of key information through monthly team meetings.
• Cultivates relationships with high level administrators, physicians, nurses and other key hospital personnel to promote and advance a positive donation culture within the service area hospitals to maximize donation opportunities.
• Attends hospital meetings, as needed, to promote and advance a positive donation culture, to assist hospitals in process improvement through continuous development and refinement of policies and procedures, and where applicable, apply industry standards and state/federal law, and build a foundation for mutual feedback to address concerns.
• Collaborates with the Director of Hospital and Family Services in the execution of the strategic plan and ensures compliance with all hospital systems regulatory standards throughout the service area.
• Responsible for developing and executing efforts related to missed opportunities, quality assurance, performance improvement efforts, and after-action reviews.
• Provides input in the Medical Record Review process, ensuring industry standards and data integrity are maintained.
• Responsible for revising and training of hospitals policies and procedures.
• Provides on-call support to clinical and hospital service employees to ensure donation pathways are protected and real-time issues can be addressed to maximize donation.
• Assists in defining and monitoring organizational effectiveness as well as analyzing, trending and reporting all department data.
• Works collaboratively with quality team to review audits and address deficiencies when noted. • Supports the department in the education and coaching of the staff on organ and tissue donation process to optimize outcomes.
• Provide onsite team support to Lifebanc and hospital staff during active cases and/or pending referrals.
• Develop and manage the utilization of customer service skills for the hospital team to identify decision-making individuals who can influence hospital protocols.
• Collaborate with hospital services coordinators to conduct individual/department meetings with key hospital staff.
• Collaborates with key hospital staff members in real-time on all active referrals and/or cases.
• Ensure all required hospital services documentation for assigned hospitals to ensure compliance with industry and regulatory standards.
• Ensures proper documentation of all hospital related activity via Lifebanc Electronic Medical Records system (EMR).
• Work with hospital administration to review, revise, and/or create policies to ensure hospital compliance with state and federal statutes.
• Assists in the execution of strategic plans for hospitals within the donor service area to create and/or maintain positive change in practices related to the entire donation process.
• Performs other job duties, as assigned.
Education and Experience:
• Bachelor’s degree required (health-related field preferred).
• Five plus years of experience in professional environment.
• Three (3) years of OPO experience (preferred).
• Two years of supervisory, management, and training experience.
• Must be willing to work weekends and holidays as needed.
• Must be willing to travel to hospitals within the designated system.
• Knowledge of medical terminology (preferred).
• Exceptional problem-solving skills.

Position Description:
The position of Family Support Coordinator II is responsible for ensuring that all donors and potential donor families are offered the opportunity for donation through collaboration with Lifebanc and hospital staff members. The Family Support Coordinator II serves as a resource and support mechanism for both the donor families and hospital staff during the active donation process and responds to all donor family needs 24/7, in addition, the FSC will support and maintain the mission, vision and values of Lifebanc in their work and job duties.
Essential Functions:
Serves as a partner of the health care team and provides support for end-of-life decisions regarding organ, tissue, and eye donation. Onsite presence for collaboration with the health care team during end-of-life discussions of potential organ, tissue, and eye donors. Identifies and contacts appropriate next of kin of potential organ, tissue, and eye donors in compliance with UAGA CMS, AATB, CMS standards and Lifebanc policies. Discusses the opportunity of organ, tissue, and eye donation with next of kin and obtains authorization/disclosure for organ, tissue, and eye donation. Provides details regarding the recovery process and benefits of donation to the donor familyProvides informed authorization or disclosure to donor families and meets the required elements of informed authorization. Reviews and evaluates donor information to determine family readiness for donation conversations and approaches/disclosures.Obtains and documents a comprehensive medical and social history interview with the next of kin, significant others and/or healthcare professionals of potential organ and tissue donors in compliance with CMS, AATB, FDA, EBAA standards and Lifebanc policies. Accurately documents required information in iTransplant in a timely manner. Provides crisis intervention, counseling techniques and emotional support for the donor family during the organ donation and recovery process in compliance with Lifebanc policies. Provides support and memory making for donor families during the donor case. Supports next of kin with requested follow up such as acceptance/deferral outcomes and/or post recovery notification. Assists with tasks and duties for the Family Services Department in collaboration with the department manager upon request. Works with hospital team to assure details related to the coroner/medical examiner’s involvement are communicated with the NOK such, contact name and phone number. Ensures departmental quality guidelines are maintained through timely and accurate documentation in donor charts to maximize opportunities for transplantation and provides essential customer service both within Lifebanc and outside the organization. Maintains an individual authorization rate that meets the department’s benchmarks which demonstrate best practice skills in consent. Assists the Bereavement Department and other departments in donor family related needs.Performs other duties as assigned.Education and Experience:
Bachelor’s Degree preferred in social work, psychology, health care, funeral industry, education, or ministry. At least 3 years of progressive experience in related field. Experience in dealing with families in grief and/or crisis situations. Appropriate current license pursuant to required education and experience.Knowledge, Skills and Abilities:
Strong organizational skills. Ability to maintain confidentiality. Ability to be on call and work extended hours, including weekends and holidays. Position Description:
The Transplant Coordinator I position is responsible for the facilitation of the organ donation process by providing clinical support through donor evaluation, donor medical management, organ placement and recovery in compliance with CMS and AOPO standards.
Essential Functions:
• Under the direction of the Administrator on Call (AOC), conducts evaluation of organ referrals and collects information to assist in the determination of donor suitability to maximize donation opportunities as required by UNOS, CMS and AOPO standards and Lifebanc policies.
• Covers assigned shifts to assure adequate coverage on a 24-hour basis including nights, weekends, and holidays.
• Responds onsite to referring hospitals within service area within the specified timeframe.
• Reviews and evaluates the patient’s Electronic Medical Record (EMR), to assist in the determination of donation eligibility and suitability.
• Evaluates the patient’s neurological status, consulting with medical staff caring for the patient.
• Serves as a member of the healthcare team and assists the Family Support Coordinators (FSC) and Hospital Services Coordinator (HSC) to identify and approach the appropriate next of kin for organ and tissue donation in compliance with CMS and AOPO standards.
• Facilitates the coordination of the medical management of the donor, following established guidelines as well as medical director guidance to maintain adequate organ perfusion and oxygenation to maximize organ potential.
• Medically manages potential pediatric and adult neurologically deceased donors as outlined in Lifebanc’s Clinical Practice Guidelines.
• Demonstrates a thorough understanding of hemodynamics including respiratory and ventilator management, electrolyte management and fluid balance.
• Draws and/or assists in the donor blood collection for the donor evaluation.
• Participates in all physical donor management in the Lifebanc Donor Care facility (DCF) including but not limited to the collection of specimens, medication and blood administration and ventilator management.
• Ability to recognize and treat cardiac arrhythmias and lead resuscitation as needed.
• Communicates all donor information accurately and timely to ensure transplantation.
• Knowledge of National organ allocation policies and procedures.
• Arranges all logistics of recovery teams including the transportation for organ recovery and organs for transplant.
• Performs other job duties as assigned.
Education and Experience:
• Associate’s degree in nursing (ADN) with current license as a Registered Nurse (RN) with at least two years’ experience in a critical care setting or,
• Current state licensure as a Respiratory Therapist (RT) with at least two years’ experience in a critical care setting or,
• One year of OPO experience and successful completion of Lifebanc’s Professional Development Program “Transplant Coordinator Track” or,
• One year of OPO experience and successful completion of a master’s degree in Transplantation and Donation Sciences.
Certification and Licenses:
• Current state licensure as a Registered Nurse (RN) or current state licensure as a Respiratory Therapist (RT) if required for experience.
• BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support) required; certification must be approved by the American Heart Association (AHA) .
Knowledge, Skills and Abilities:
• Basic computer skills and working knowledge of basic equipment.
• Critical care experience to achieve hemodynamic stability in a potential organ donor as well as the ability demonstrate critical thinking to implement treatment to optimize donation.
• Ability to assume responsibility without direct supervision, multitask and to demonstrate effective decision making within the scope of assigned authority
• Strong interpersonal skills with the ability to interact and communicate effectively with a wide range of people, including medical staff, hospital personnel and donor families.

• Ensures in the execution of organ and donor plans to create and/or maintain positive changes in practices related to the entire donation process.
• Responsible for directing budget creation, expense oversight, and performance targets for the Organ service line.
• Provides oversight into organ recovery activities within the Donor Care Facility or remote recovery if applicable. Works closely with the Director of Facilities on supply, sterile processing, and facility needs.

Education and Experience:
• Bachelor’s degree required (health-related field preferred).
• Previous operating room and sterile technique experience required.
• RN and BSN required.
• CTPC and /or CCTC preferred.

Position Description:
This position is responsible for the facilitation of the tissue and eye donation process through interaction with hospital personnel, coroner personnel, communication with tissue processors and recovery staff as mandated by relevant federal, healthcare and donation industry standards.
Essential Functions:
· Identifies and evaluates all referrals for potential organ, tissue and eye donation as mandated by relevant federal, healthcare and donation industry standards.
· Collects and interprets patient medical and social history information provided by hospital/nursing home staff, coroners or investigators.
· Obtains and enters accurate referral information into iTransplant.
· Triages organ, tissue and eye donor referral calls, contacting the appropriate Lifebanc staff member when needed.
· Requests and accurately reviews medical charts to determine suitability based on processor criteria.
· Confidentially access and navigate various hospital electronic medical record systems to obtain pertinent medical charting.
· Appropriately pre-screen cases with multiple processors when applicable.
· Obtains releases from appropriate coroner/medical examiners and document specific samples and restrictions that they request.
· Communicates case activity to the Administrator on Call.
· Delivers a thorough and accurate report to DSC II and DSC III prior to family approach.
· Serves as the point of contact for funeral homes and Coroner/Medical Examiner offices regarding the donation status.
· Responds to audit findings and processor requests in a timely manner.
· Accurate timely completion of charting error tasks assigned in iTransplant and responds to all other inquiries from the Quality Department required for donor chart clearance.
· Performs all other job duties as assigned.
Education and Experience:
· High School Diploma required.
· An undergraduate degree, diploma, or certificate in the health field or related area preferred.
· Minimum of 1-2 years’ experience as support staff in the clinical health care field such as a Licensed Practical Nurse, EMT, Health Unit Coordinator, STNA, MA, Clinical Technician, or related experience in tissue banking.
Certifications and Licenses:
· CTBS certification preferred
Knowledge, Skills and Abilities:
· Proficient use of computers, email and navigation of applications simultaneously.
· Proficient in data entry accuracy and ability to read and interpret medical records.
· Ability to assume responsibility without direct supervision, exercise initiative, and make responsible decisions under stressful conditions for a positive outcome.
