    Position Summary

    A clinical competent registered professional nurse who delivers patient care through the nursing process of assessment, planning, intervention, implementation, and evaluation. Prescribes, delegates and coordinates the nursing care provided to patients throughout the health system. Effectively interacts with the patient, significant others, and health care team members while maintaining the standards of professional nursing.

    Responsibilities

    Handle multiple concurrent patient complaints, concerns and issuesFormulate nursing care plans.Administer medications within the scope of RN practice.Assist physician with invasive/ non-invasive procedures performed in the department.Acquire and maintain current knowledge of all equipment and procedures/treatments generally used in the care of emergent patients.Document/Communicate pertinent information regarding patient condition.Provide direction and supervision to LPN's, POCT's and other ancillary staffPerform crisis intervention.Monitor and ensure the accurate and timely completion of physician orders.Perform defibrillation, cardioversion, and external pacing effectively/safely.Provide assistance in order to provide efficiencies in the ED and get the patient's length of stay in the ED decreased.Perform other duties as requested.

    Requirements

    Current R.N. licensure for Ohio and maintain.Must have strong critical thinking skills.BLS Certification (Mandatory)ACLS Certification (Mandatory within 6 months)PALS Certification (Mandatory within 6 months)Previous nursing experience preferredAbility to work independentlyLMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.

    Position Summary

    Serves as the department director for the Inpatient Case Management Department. Responsible for case management, utilization review, discharge planning, social services, and dietitians. The director is responsible for planning, organizing, and overseeing all department functions. Manages relationships with payers, physicians, hospital referral sources, and case management staff.

    Responsibilities

    Perform strategic planning activities and daily management of operationsEnsure timely and effective transitions of patients, including discharge planning and coordination of care with other healthcare providersIdentify areas for improvement in case management processes, develop new workflows, and implement changes to improve efficiency and patient outcomesMaintain strong communication with physicians, nurses, and other healthcare professionals, as well as insurance companies to ensure seamless care coordinationInitiate and engage in process improvement activitiesMaintain compliance with relevant regulations and accreditation standards, including any updates or changes that may directly impact services provided by case management such as discharge planning, identifying and reducing hospital readmissions, promotion of effective utilization of services and reimbursement for care providedOversee medical necessity utilization review, appeals and denials management related to medical necessity audit activities (RAC, MAC, commercial payer denials, internal reviews/audits, etc.)Train and motivate case management staff to accomplish department goals and objectivesDevelop and oversee the annual case management budgetPrepare and evaluate reports on the department’s activitiesDevelop and update policy and procedures for case management operations, ensuring adherence to organizational standards and best practicesLead utilization review committee meeting, providing meaningful information and suggestions for improvements

    Requirements
