    Job DescriptionJob DescriptionThe Assistant Director of Residential Services will primarily be responsible for supported employment and day-to-day residential administration. The Assistant Director is responsible for the safety and protection of individuals enrolled in LVAS residential programs, as well as developing and directing staff to ensure quality care. The Assistant Director will coordinate with the Director of Developmental Programs, Chief Executive Officer and Residential Leadership in creating teamwork among the Program Managers and Team Leaders to ensure competent, cost-effective coverage of all shifts that reduces overtime to a minimum yet maintains LVAS standards.    

    ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

    ·         Provide instructions to staff and consumers as described in the ISP’s
    ·         Develop supported employment programs, and performance improvement
    ·         Model teaching and intervention techniques developed by the team·         Maintain residential documentation and review direct staff for accuracy and timeliness. ·         Confirm with Client Services Coordinator for timeliness of staff documentation, calendars, and goal sheets as needed.·         Develop, train and orient all new hires and internal employees·         Complete new hire shadowing schedules.·         Perform daily supervisions of staff.·         Provide a safe, structured, and therapeutic environment·         Act as a liaison on behalf of LVAS to families/ constituents of our consumers·          Assist in the development of community outreach and program development·         Schedule monthly meetings with Program Managers·         Correspond with Program Managers concerning corrective action plans for staff. ·         Enforce compliance of all LVAS policies and procedures·         Monitor compliance with all State and Local regulatory agencies·         Act as Incident Commander in the absence of the Executive Director·         Schedule, Supervise, train, provide disciplinary action and hire Direct Support Professionals·         Attend ISP, Care Plan, staff, and interdisciplinary team meetings as required·         Approve of staff timesheets.·         Perform other duties as required or assigned QUALIFICATIONS

    Job Title: Quality Management Coordinator

    SUMMARY

    The Quality Management Coordinator of Lehigh Valley Adult Services will be responsible for creating and enforcing a Quality Assurance System for evaluating performance that focuses on structure, processes, and outcomes to reduce or resolve identified problems or implement process improvements. Quality Assurance Programs are to be planned, objective, comprehensive, systematic, measurable, and ongoing. There should be a system of checks and balances for the company through the development of standards with measurable goals, documentation of policies and procedures, staff training, and review of data associated with standards. The Quality Management Coordinator will confirm and enforce adherence to standards and Federal, State, and Statute laws and Regulation. This will also include monitoring activities to assure or improve the quality of care and services.

    ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

    Investigate incidents and manage EIM within HCSIS.
    Identify areas in need of improvement and implement process improvements.
    Develop corrective actions to address areas of deficiencies.
    Follow up monitoring to ensure effectiveness of corrective actions taken to rectify concern(s).
    Review findings with QA Committee members and staff involved in the review process, etc.
    Obtain necessary resources to implement corrective actions.
    Develop educational information, policies, procedures, etc. pertinent to corrective actions.
    Educate and train staff, residents, and families (as necessary) to the upcoming corrective actions (changes).
    There should be a periodic review of residential finance reports or medication records, or any other method for monitoring.
    Assign a monitoring time frame for each area and person responsible for follow up.
    Identify processes for the staff, supervisor, and director to review effectiveness of corrective action (s) and provide feedback on next steps.
    Provide a mechanism for communicating quality to appropriate parties including, but not limited to, administrative and executive staff, residents, family members and staff.
    Serve as a mechanism for communicating accomplishments to appropriate parties including, but not limited to, Board of Directors, administrative and executive staff, residents family members.
    Assign and conduct investigations
    Enter and finalize unusual incidents
    Conducting Satisfaction Surveys
    Adhere to HEDI
    Other duties as assigned.

    QUALIFICATIONS
    Successful completion of FBI fingerprint (as required), PA Criminal Background Check, Child Abuse Clearance, Drug Screen, and complete a Physical
    Working knowledge of EIM/EMC
    Excellent communication skills
    Analytical Skills
    A desire to help others achieve their best

    EDUCATION AND/OR EXPERIENCE

    Bachelors degree with experience in the field (Master Preferred)
    Experience in licensing
    Ability to analyze data and create and review processes
    Experience working with persons that have Intellectual Disabilities: 5 years (Preferred)

    SUMMARY

    The Case Manager of Lehigh Valley Adult Services will be responsible for developing, executing, and managing all day-to-day aspects of the client services by overseeing clinical and administrative operations and managing case files. The Case Manager will facilitate patient care by assessing needs, evaluating treatment options, and coordinating care. They are to act as job coaches and day center support staff, as a leader and supervisor as part of the integral team.. The Case Manager will offer insight of how decisions could impact the program, participants and staff to the leadership team

    ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

    Completing quarterly and annual assessments of new individuals to determine abilities.
    Participating in the individual plan process, development, team reviews and implementation.
    Providing effective leadership in all areas of the adult day center including client-centered care initiatives, personnel management, and quality and safety of programming
    Completing client satisfaction surveys each month
    Scheduling of all medical appointments and follow-ups and attendingappointments as needed
    Ensuring the nutritional guidelines for all individuals are followed as written
    Completion of intakes and maintaining contacts with referral sources as needed
    Training, developing the performance of day center staff and job coaches
    Auditing and addressing staff documentation for timeliness, quality and accuracy while coordinating with the Community Integration Director
    Evaluating staff performance and completing corrective/disciplinary action if required
    Monitoring client services to ensure effectiveness, quality of care compliance and identifies areas of improvements.
    Participating in the referral process
    Ensuring all staff are trained in the individuals plan; to include assessments and health promotions/protocols
    Other duties as assigned.

    QUALIFICATIONS
    Successful completion of FBI fingerprint (as required), PA Criminal Background Check, Child Abuse Clearance, Drug Screen, and complete a Physical
    Excellent communication skills
    Analytical Skills
    Complete knowledge of the IDD and Department of Aging Regulations


    EDUCATION AND/OR EXPERIENCE

    Bachelors in human services Field or similar is required (Maters is preferred)
    Minimum two (2) years supervisory experience
    Understand and comply with Incident Management
    First Aid Certification required; Instructor level training preferred
