    Position Summary Reports to the Director of Purchasing and Facilities. Environment of Care (EOC) employees are responsible for maintaining the internal and external condition of Lawndale Christian Health Center (LCHC) properties in excellent condition. An EOC worker also plays a role in maintaining a safe and secure environment for patients and employees as a presence in their area of responsibility. An EOC worker will be called upon to support clinic operations within the skillsets described below. Principal Duties and Responsibilities* * Must be alert to the internal and external condition of all LCHC facilities and take responsibility for resolving an area of need, either individually if necessary skills are present or by working with team members to obtain the necessary resources from other avenues. * Responsible for completing scheduled assignments including checklists and ticket/project/site specific projects as prescribed for each shift * Perform housekeeping checklist according to procedure and meet or exceed sanitation standards prescribed by JCAHO. Corresponding checklist should be completed, signed and turned in at the end of each shift. * Collaborate with Hope House volunteers as needed and support their efforts to provide a welcoming experience for our patients. * Be prepared to assist clinic staff to respond to a security incident. Primary objective is to de-escalate the situation and contribute to establishing an environment where a resolution can be achieved without physical harm to property, patients or staff. * Must be available for overtime in emergency situations * Must be on site or available during snow emergencies * Assist the Purchasing department with package delivery as time permits * Respond to the requests and assignments from facility stakeholders in a timely and professional manner. Facility stakeholders include department and shift coordinators, site managers and other business unit managers. These stakeholders have the authority to provide feedback and request help with projects or operational efforts that require EOC skillsets. Requests deemed unreasonable can be directed to the shift or department coordinator for evaluation.     Position Summary The Clinic Nurse is a registered nurse who is key to providing support for our providers, safe care for our patients, strengthening operational quality, and ultimately improving clinical patient outcomes. The Clinic Nurse assists in the care coordination our patient's healthcare needs, and serves as a teacher, advocate, and navigator for these patients within the context of the care team. This nurse will be responsible for providing direct nursing care, performing nursing assessments, participating in the continuity of patient care, being a servant-leader, and helping improve patient outcomes within the role a clinic nurse. Reports to Director of Nursing, Chief Clinical Officer of Operations, and Providers. Principal Duties and Responsibilities * Provides clinical and administrative support for providers within the care team model. This includes: * Serving as a liaison between providers and their patients * Communicating abnormal lab results and treatment plan to patients as outlined by the provider and/or protocols * Providing patient education in areas such as chronic disease, medications, pregnancy * Completing medication reconciliation, new patient intakes, and OB intakes as appropriate * Working with other members of the care team to coordinate patient care, referrals, DME orders, care management and hospital/ER follow-up * Executing nursing care under direction of the provider such as: IV infusions, medication injections, wound care and dressing changes, and assisting in procedures * Navigating medication changes and prior authorizations due to insurance requirements under standing orders and the guidance of the provider * Obtaining pre-authorizations and/or RQIs for diagnostic testing as required by insurance * Manages the clinical phone line. Assists pharmacies, responds to patient requests, conducts triage and conveys messages to and from providers, clinics, specialists, and other care team members. * Directs the assessment of acutely ill patients who present to the clinic in person or via phone to ensure efficient work flow. * Provides nursing care to patients who present through the Nurse Visit Schedule. This may include, among other types of visits: MAT care, Latent TB evaluation, INR level checks with Coumadin titration, BP checks, wound care, transfer of scripts, patient education, medication administration, triage, pregnancy tests/counseling, HIV rapid tests and conveying test results. * Collaborates with medical support staff and assists with prioritizing clinical tasks to ensure efficient work flow. Assists with training and competency verification of medical support staff. * Participation in Care Management and Chronic Disease Management efforts in collaboration with the Population Health Department possibly including home visits, consultation with Complex Care Coordinators, specific patient education and assessment efforts, other tasks as they arise. * Ensures all emergency equipment and clinic use medications are properly stocked, stored and logged (as applicable) for the care of patients at their clinic site. * Participates in care team meetings, monthly nurse meetings, clinic-wide quarterly meetings and other meetings as directed by the Director of Nursing. * Documents all activity appropriately in the Electronic Medical Record.     Position Summary

    The Behavioral Health Care Coordinator is responsible for clinical and administrative support tasks related to facilitating behavioral health and psychiatric care at Lawndale Christian Health Center. Responsibilities include coordination of clinic and telehealth Behavioral Health consultation visits, project management of initiatives related to behavioral health and psychiatry, development of clinical resources for patients and providers, and data collection and analysis of service line utilization and quality outcomes.

    Principal Duties and Responsibilities *

    • Coordinate with care teams to facilitate on-site BH handoff process, including following up with care teams members regarding workflow concerns, updating materials and signage in consult rooms and provider areas, and assisting with orientation of new providers and MAs to use of BH services, schedules, and telehealth equipment.

    • Facilitate BH telehealth visits, as needed, including coordination of triage schedule, confirming patient availability/interest for scheduled visit, and administration of secondary diagnostic assessments (i.e., PCL-5, DAST, MDQ, ASRS, etc.), and.

    • Provide clinical and technical support to Behavioral Health treatment groups (i.e., THRIVE Teen Group, Stress Management Group, etc.) as needed, including curriculum development, co-facilitation, patient outreach, registration, training of BH staff on telehealth platform, troubleshooting technical concerns, coordinating communication with care teams regarding groups (i.e., schedule updates, flyers/signage, EMR quick text, outreach to Site Managers and PCPs, etc.).

    • Assist in the development of EMR form content to improve efficiency and quality of documentation for BH, psychiatry, and medical providers.

    • Assist in the development and updating of provider EMR schedules and templates, as needed.

    • Develop, organize, and maintain clinical resources for patients and care team members related to mental health concerns (i.e. patient education handouts, condition-specific referral guides, clinical decision support tools, etc.)

    • Compile items for intra- and interdepartmental communication in monthly departmental newsletter.

    • Coordinate internal pediatric psychiatry consultation requests, gather data related to case review, meet weekly with psychiatrist to facilitate case review.

    • Assist with planning and coordination of events, meetings, and trainings for BH department and external partners, including scheduling, purchasing supplies, preparing agendas, and capturing meeting notes.

    • Run productivity reports monthly to highlight areas for operational improvement.

    • Collect and report program-specific data used to monitor clinical quality and work toward achieving CQI program goals.

    • Perform other duties as required to help LCHC accomplish its mission.

    Position Summary The Behavioral Health Care Coordinator is responsible for clinical and administrative support tasks related to facilitating behavioral health and psychiatric care at Lawndale Christian Health Center. Responsibilities include coordination of clinic and telehealth Behavioral Health consultation visits, project management of initiatives related to behavioral health and psychiatry, development of clinical resources for patients and providers, and data collection and analysis of service line utilization and quality outcomes. Principal Duties and Responsibilities * * Coordinate with care teams to facilitate on-site BH handoff process, including following up with care teams members regarding workflow concerns, updating materials and signage in consult rooms and provider areas, and assisting with orientation of new providers and MAs to use of BH services, schedules, and telehealth equipment. * Facilitate BH telehealth visits, as needed, including coordination of triage schedule, confirming patient availability/interest for scheduled visit, and administration of secondary diagnostic assessments (i.e., PCL-5, DAST, MDQ, ASRS, etc.), and. * Provide clinical and technical support to Behavioral Health treatment groups (i.e., THRIVE Teen Group, Stress Management Group, etc.) as needed, including curriculum development, co-facilitation, patient outreach, registration, training of BH staff on telehealth platform, troubleshooting technical concerns, coordinating communication with care teams regarding groups (i.e., schedule updates, flyers/signage, EMR quick text, outreach to Site Managers and PCPs, etc.). * Assist in the development of EMR form content to improve efficiency and quality of documentation for BH, psychiatry, and medical providers. * Assist in the development and updating of provider EMR schedules and templates, as needed. * Develop, organize, and maintain clinical resources for patients and care team members related to mental health concerns (i.e. patient education handouts, condition-specific referral guides, clinical decision support tools, etc.) * Compile items for intra- and interdepartmental communication in monthly departmental newsletter. * Coordinate internal pediatric psychiatry consultation requests, gather data related to case review, meet weekly with psychiatrist to facilitate case review. * Assist with planning and coordination of events, meetings, and trainings for BH department and external partners, including scheduling, purchasing supplies, preparing agendas, and capturing meeting notes. * Run productivity reports monthly to highlight areas for operational improvement. * Collect and report program-specific data used to monitor clinical quality and work toward achieving CQI program goals. * Perform other duties as required to help LCHC accomplish its mission.
