  • PHYSICAL THERAPIST - PRN

    Lakeview Specialty Hospital & Rehab is growing We are ready to add several quality-minded, compassionate, and knowledgeable Physical Therapists to our experienced and collaborative Therapy team in support of our program expansion. DEPARTMENT FUNCTION/JOB OVERVIEW Responsible for planning and administering physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability. Responsible for delivering and/or delegating quality patient care from admission through discharge, including patient and family education/training. Responsible for maintaining accurate and timely documentation. Work will be performed in an ethical and legal manner following organizational policies, processes, protocols, and procedures. Primary Responsibilities: Performs initial and on-going assessments of patient's condition. Establishes, revises, and evaluates a plan of care which is appropriate to problems identified and involves the patient/family. Provides or delegates physical therapy treatment to patients, including but not limited to therapeutic exercise, gait training, functional activities, pain management, physical agents, hydrotherapy, and appropriate utilization of group therapies to facilitate patient recovery. Records pertinent patient information to document changes and comply with legal and ethical standards, including daily charting, team summary, insurance justifications, initial and discharge summaries, plan of care, and patient exercise programs. Records daily treatment charges and equipment billing to insure fair and accurate financial records. Analyzes and revises treatment programs to insure continuing appropriate intervention. Modifies therapeutic interventions to accommodate the age of the patient population. Instructs patients and families/caregivers in functional activities and exercise programs to facilitate follow up on discharge. Follows instruction from lead therapist and/or director to perform other functions as assigned to achieve the goals within the department and the program. Must perceive patient relations as a primary focus, and continually strive to satisfy the needs of every patient in all areas of the work force. License / Education Requirements : Current State of Wisconsin Physical Therapist license Degree from accredited educational institution Current CPR certification Preferences Preference will be given to candidates comfortable in evaluating and treating patients within inpatient, inpatient rehab, and outpatient settings. Preference will be given to candidates available to work during the work week and weekends. Previous professional or clinical education experience in a neurorehab setting is strongly preferred but not required.   • To provide physical therapy intervention to clients served and to be a productive member of the physical therapy department. Key Responsibilities: Patient Care: Participates with the treatment team in the development, implementation, assessment, and documentation of individualized patient treatment plans under the direct supervision of a physical therapist. Qualifications: Skill, Knowledge and Abilities: Requires a thorough knowledge of human anatomy, philosophy, and the effect of treatment procedures on body function and structure. Knowledge of neuro-developmental treatment and motor control and understanding of neurological dysfunction and its effect on motor performance. Education: Completion of physical therapy assistant program from an accredited school and CPR certification.     Are you looking for a challenge? Join the Lakeview Hospital team as an Occupational Therapist! Safely provide occupational therapy services to in-patients within the long-term acute care and rehab hospital. Evaluate and treat patients with medically complex and neurological diagnosis to assist them with reaching maximal level of functioning. PRN weekday and weekend scheduling opportunities available. Key Responsibilities: Provide individual and group therapy to patients as ordered by physician. Develop Occupational therapy treatment plans which are specific to the special needs of the patients within the program. Provide consultation on problems related to Occupational Therapy, specific treatment techniques, and other programming issues to promote understanding of Occupational Therapy principles and team involvement in patient care. Assume responsibility for supervision of Occupational Therapy Students as assigned. Maintain open communication with all members of the treatment team and present self as a team player. Qualifications: 1. Skill, Knowledge and Abilities: Must be to communicate both in writing and orally. Good interpersonal skills. 2. Education: OTR or MOT in Occupational Therapy 3.
